Pt name in the report itself?

I thought I had read somewhere that the pt name should not be in the body of the report. Can anyone tell me if that is a law/regulation/etc.. or just a style guideline? I have a new acct I am assigned to and all of the docs give the pt's name while dictating the body of the office note/procedure note.
If it is a HIPAA thing or some sort of regulation, I need to know. If it is merely style, I will leave it.
I just want to do my job right so if anyone knows for sure, please let me know.

Flagging a report
What is the standard way to flag a report if you did not understand a dictated word? The AAMT BOS states to leave a blank space. If you had to flag a report for that reason what is your procedure?

A patient's name is almost required in the report
to help with filing the report into the chart. Now whether or not it a copy of that record can be sent out with the patient's name is a different story depending on several factors. For example, all identifying information must be removed by a hospital's tumor registrar before submitting the information to the College of Surgeons who monitor cancer trends and treatments.
Many services are steering away from the name of the patient being in the body of the report as a general safeguard, which is a good thing, but this is not mandated by any law.
Per Line versus Per Report
In a 7-hour day I can type 2000 lines or 200 reports. Most companies expect 1200 lines during a full-time shift, so that probably would equate to 120 reports if you are going by the 1200 lpd standard. Another way to look at it...the average "decent" (not exceptional) MT should be able to type 20 minutes of diction in 60 minutes. I don't proof after typing, I proof while I am typing.
