If you want to be able to work at home, you'll have to do the type of work that national services have, which is primarily acute-care hospital and large clinic.

You may be able to handle this work, but because you have done only mental health for 19 years, an employer is not going to assume that you can.

You might want to review acute care and specialty dictation to make sure that you CAN deal with that type of work. Then, apply with an eye toward assuring an employer that you can do the work.

If you discover that you are far from being able to do operative procedures and medical specialty reports, then you may find it necessary and even profitable to take a reputable MT course, like Andrews or M-Tec. That will almost guarantee you a job the minute you finish the course, which should take little time because you have the foundation skills already.
