But for "Thank you for referring this patient to me and allowing me to participate in his/her care" I would do "tyfrtptmaa" (using the first letters of the words in order) because that would be enough to bring it up under the IT phrases.
In the phrase I would have "Thank you for referring this patient to me and allowing me to participate in @@ care" because the @@ is the marker I can get right to in DQS and then fill in his/her.
Also, I would make expansions for the smaller parts of the sentence "Thank you for referring this patient" because sometimes he might say only that part or change it to "Thank you for referring this patient to me. I will see her again next week."
I always try to think lazy - think in as few letters as possible, but still in some way that makes sense and you don't have to spend lots of time remembering what your shortcuts are. Eventually you will find it is second nature - it is just learning to think differently that seems daunting when you are starting out with the expansions.
One of the things I love about Instant Text is that you can have the same expansion shortcut for things because it brings up all of them and then you can choose the right one - tyfrh can stand for "thank you for referring her" or "thank you for referring him" and the same shortcut will bring both up, making it easier to work the system.
