You start inhouse, probably in a clinic but maybe a hospital if you can get on. Then you get your experience before going home.
Do you know why they want the crucial 2 years plus in experience? Because if you are new, you still do not have all the skills and education to do this job for acute care at home and be productive enough. You require too much training and mentoring at this point to do a quality/quantity job from home on your own.
Try inhouse. This is a career and you must be willing to put in what it takes to secure it for you. The 2 years inhouse will be the best learning environment for you, especially in a hospital setting.
