This sounds crazy, but it worked for me, albeit for a different reason. I had a nerve injury in my heel some years back (unrelated to MT work). I discovered that many things, including foot pedal use, were actually more comfortable if I were wearing high heels....

So even though I mostly transcribe in flip-flops or slippers nowadays, for a while there, I transcribed in pajamas and 2-inch heels. Again, you might have to play with the angles and distances under the desk; but it's worth a try. Assuming, of course, that you even own a pair of heels.... :)

Neat trick
Not sure if it will help but it has helped me a ton.  If using Express Scribe, change the setting for your forward pedal to 67% and 77%.  This slows down the dictation w/o distortion.
