If I may be so bold as to clarify this statement.
SR can be compared to a dog. You need to put a lot of quality time into "training" it to do what you want it to do, and you have to continue the training process long after the dog has learned the commands.
SR makes mistakes for two simple reasons. First, it doesn't understand human language. As the Dragon training module says, it doesn't understand the difference between "Ice cream" and "I scream." You (the dictator) have to teach it the difference. And if it makes a mistakes, you have to correct that mistake at some point in time. You can do it as soon as the mistake is made or later on in the day. But, just make sure you correct the mistake. If you don't, then the mistake will be made over and over again.
This is the likely scenario at facilities that install SR for their doctors and/or nurses. Just dictate and that's it ... "we'll handle the rest." And that's where the MT comes in. (Excedrin headache #29).
If you want to guarantee improved accuracy, create a situation whereby the dictator(s) have to spend their time training the product ... continuously ("here's an error, now please correct it ... and another one ... another one ... etc.").
The only "training" that's going on is directed at the MT. Unfortunately, no one is learning anything. What I read is a lot of kvetching but little to no action.
There's an old expression that defines insanity ... doing the same thing but expecting a different result.
