the MTSO's offer.  If you are a well-trained, knowledgeable medical Transcriptionist with experience and you are looking for an IC job or an employee position with an MTSO, then you are worth more than 8 or 9 cpl.  If all you do is sit and transcribe all day long, you are worth more than 8 or 9 cpl. 


I really don't know what you're getting at.  Not everyone has the capabilities, time, or money to start their own service.  Not everyone is good at pounding the pavement and finding their own accounts.  I'm not a salesperson to say the least.  I lucked into my own personal accounts and if I hadn't, I'd be working for one of the MTSOs, but I'm not.  My full-time employee status job is with a hospital for the benefits only and it pays hourly with incentive.  Right now, that's the only employee status job worth taking is through an actual hospital, NOT an MTSO.  At least at the hospital, you get a decent wage and get great benefits.  Oh, there are a few good MTSOs out there, but they are very few and far between.

If you're a potential employee...
...it has to do with HIPAA as well as Workers' Comp, in addition to ID issues.  For HIPAA, your home office has to be completely out of sight from the anyone living in or visiting your home, preferably in a separate, dedicated room, or in a common room but with a partition.  Sometimes at-home MTs don't get this, i.e., it's not okay for little Sally to come sit next to mom while she's transcribing; it's a HIPAA violation.
If you're IC on one account and employee on another..

Rather than paying self-employment tax, can I just increase what is being deducted as an employee? 


I'm going to be working about 2/3 of my production as an employee, then an IC account which will account for another 1/3 of my total income. I've always been either just an employee or just an IC, and I'm wondering about how to handle self-employment tax in this situation.


Anyone here been-there-done-that?  Would appreciate hearing your thoughts.  Thanks!

On some of my own accounts, I make MORE than $47/HR...sm
remember it's not what you charge per hour, it's what you charge per line and how many lines you produce in an hour. With your own accounts, charging somewhere around 14 cpl, you can make well over $47/hr - though probably not consistently 8 hours per day...

Yes, and on some accounts we're not even

allowed to make complete sentences.  On our verbatim accounts if the doctor dictates something like patient to OR, under GA, 3-cm incision, that's exactly how we have to type it. 


Back in the day we would never consider that appropriate for a medical document that may some day have to be used in a court of law.  But, lately we have to just throw all common sense out the window and type what they want.  It sure does hurt the line count, but even worse, it hurts the profession in that it seems we're not allowed to use our brains...just type.

Overtime has to be approved by manager. I have always been an SE and could work as many hours as I wanted to, such as when I had a day of bad dictators I could work until I got my daily line count. Just wondering if all states had this law. I am working in CA.

You're going to have to make a change.

If you can try to work an hour or so earlier or later to get in some lines when her son is sleeping.  I find that if my children/DH are awake I want to be with them, even if just watching TV, so I'm more productive when they are sleeping.  If you think this new account situation is just a temporary thing and in a month or so you won't have to work so much then just have a talk with son and explain the situation and make an effort to spend some quality time with him.   Starting with any new account is takes a while to get productive, but if this new account is too difficult, ask to try another account or change companies.  


I gave way too much to my job and not enough to my family, even though I had to work, I wished I could do it all differently.  My son has seen pictures and home videos of me hold him in my lap and reading to him and he can't believe I used to do that.  I allowed my work to take priority.  I can't go back and undue the past, but I've learned from my mistakes and working hard to correct things now.

The way I figured it, to make $60k a year at 40 hours a week you have to type at least 300 lph and make 9 cpl.   Not many 9 cpl offers these days and only experienced, long-term MTs can do 300 lph on a regular basis.


Bet they didn't put that in the ad.

SE status instead of IC? For me,SE status is heaven -with part of taxes paid and flexibility in my schedule. Have you ever applied to MDI-MD? They only accept qualified, experienced MTs and stress quality in their transcription. From what I read of your qualifications, it sure would be worth contacting them.

Per "see mssgs" comment regarding age discrimination later on in this discussion, I'm 64 - no problem getting hired at a line rate higher than average. Age is not a factor with MDI-MD - knowledge and quality is !!!

and a lot of the time we do the shopping together. We try to make sure that the majority of us like something before we decide on it for dinner. And we're always trying new things and have found many new favorites that way. However, if there's something we're planning on having that one of us doesn't like, I'll make a simple alternative to it (like maybe fish, etc.)

I certainly can't criticize the kids for not liking everything we like, or tell them to go without if they don't like it, because half of the time it's ME who doesn't like the planned dinner. For example, they all love steak and I can't stand red meat.

We try to include enough of a variety so that everyone's happy. If one person isn't crazy about the main dish, then more than likely they're going to eat more salad and veggies or maybe a sandwich with it. It all works out. :)

Make sure there aren't other taxes that you're overlooking.

Some communities/counties have occupational privilege tax, 911 tax. Some sales require you to collect a sales tax on your service. The best advice is to check with a tax preparer AND visit your state's revenue website to make sure you're not missing anything.

Well, ya can't make $50k a year unless you're willing to give up your life and be chained to S

you PC 12+ hours a day.  I can't recommend any particular school as I didn't go to one of the "online" schools.  I learned in an actual classroom which I recommend over any of these online schools.  Another thing I recommend is working in an office for at least six months, but more like a year, because there is a gigantic learning curve once you actually get a job.  There are just things that schools can't or don't teach, things that can only be learned on the job from other MTs who have been doing it longer than you.


Don't get me wrong.  It's great to be able to work from home.  Personally, I don't think newbies should be allowed to work at home right out of the gait.  There is some much they don't know.  They need help.  They need to be able to ask questions and one of the best things about working in the office as a newbie, is being able to ask the seasoned MT next to you to listen to something for you.


Having said all that, I think MT is slowly dying.  It's become difficult to make a decent living in this profession.  I'm one of the few lucky ones.  I work at-home for a hospital.  I get paid hourly, get incentive, and have all the benefits of being a FT employee.  Oh, and they can't take my benefits away if I don't make my quota.


You can't support a family on 7 or 8 cpl or even 9 cpl when the dictation is the crap they won't send to India.  I would suggest finding another profession.  The trend in medical coding is that all coders will be working from home soon what with everyone going to the Electronic Medical Record.  I would go into coding.  You may end up working in the office for a while, but I don't see coding being outsourced to India any time soon.

You're likely to make more money on the lower rate because easier to understand and can probably
faster and need less time to look things up, etc. Just my 2 cents.
Excuse me but federal taxes are paid as employee by employee
The ONLY difference in IC is you pay the ENTIRE amount of the SS which is the 15.25%, as an employee you only pay 7.75%.  And you can easily make up that difference in deductions.  As an employee you just have someone else manage your withholdings and as an IC you do it yourself.  I have much more usable income being an IC and not an employee and I am strict about putting aside what I need to.  But everyone always says you have to pay your own taxes, well your employer does not PAY them for you -- except 7.75% of SS, they merely withhold it for you.  As an IC you are the employer. 
How about zillions of accounts. MQ has these accounts so overloaded even the new hires dont have any
work. I would love to read some of these reports and then they expect perfect QA and no blanks. Did they lose their minds completely.

The accounts are old and she keeps her accounts TRUSTING she hired the right MTs
who if in doubt will ask questions regarding reports. No one can find all mistakes in all reports and that is human to have a report with perhaps a typo. You think anyone reads through all the reports of a national company before they get to the hospital? Not a chance. Some random QA is supposed to correct all mistakes? Not hardly. Doctors/dictators make mistakes too. We all do. So do QA people. So the thing is this person hires good people who are trustworthy, the hospital likes the way she does their account and life goes on quite nicely w/o hovering editors/QA people. BTW, who QAs the QA people on your accounts. Ever wonder?
You're working to make money, not to spend money.
These people should be ashamed of themselves taking advantage of people this way!
You an IC, statutory employee or employee?
That designation, and/or forcing the IRS to designate you, can make a diff. Are you a corporation? Partnership? You need WAY more help than we can give you.

otherwise the posts on here as far as I can see telling it like it is. I think a lot of the posts are so young they just don’t have a clue as to what it means to be an employee. You do as your employee wants (I am talking reason now, not stupidity as far as sexual advances, etc) or else you find another company that fits your personality better. You do not confront, you do not tell your employee no, that is their job, not yours. This is such a simple thing for people who have been in the work force for any period of time.

That should not be the case. Sounds like you need new accountant with strong knowledge of IC deduction.  We save a several thousand on what we pay in taxes based on what I can deduct as at home IC, $2000 off the top for what I pay my child to help with office chores, as an example.  The measly 7.xx% I pay that an employer would pay is well worth my independence in being my own boss.

Marla in So. California pays 11 cpl for a 55 ccl, Oracle pays 9-1/2 cpl, MDI-MD 11 cpl, JLG 10 cpl for gross line, etc.


Of course, these are rates negotiated and paid to qualified MTs.
