I think you need to see it differently ...
FIRST, we do not KNOW for sure the new MQ plan; it may be just be for particular offices at that.
Secondly, IF (big IF here) it is as some suggested based on 98% QA score and less than 15% of your work going to QA ... remember, this:
---that 98% is ONLY on the handful of reports chosen for actual QA'ing of your work;
---15% of your total work going to QA because of blanks is a LOT of your work; something is wrong if you are sending 15% of your work to QA for blanks and you need more training.
IF (again, big IF) the plan is designed the way one poster seems to think it is and everyone now ASSUMES it is ... you will still get paid if you don't meet this criteria -- you just won't qualify for production incentives.
The whole point is to REWARD those who have high skill and production in hand.
Not everyone will qualify. That is NOT just at MQ but at ANY place where there is a standard. Not everyone makes the cut.
Everyone CAN make that cut, though in varying degrees, by making a dedicated effort and commitment to improving their quality, learning ESLs better, learning to use tools/techniques for increasing their production, and mainly by letting go of this attitude that the company is out to get you.

Maybe it's the extra sounds that they charge for? I've been using Home Typist for several weeks and never got a notice it was only a trial version. Trying Googling Home Typist 1.40 and make sure that's the version you downloaded. I think you can find it at freedownload.com
If you're a fast typist,
(250-300 or more lph) you're probably going to lose money. If you're a slower typist, you might break even. It also depends on the job. Some of them take longer to edit than to just straight type.

If you're writing in a manner which is provoking in people, then I would say the problem lies with you. Perhaps you are expressing yourself in a confrontational manner?

Sorry - but I think you could have handled it differently
I am assuming you are an IC - which means who you work WITH is your biz and no one else's - and no one's business if you took on a second contract. An IC works for who she wants when she wants and within reason sets her own deadlines. A company understands this and does not give you a 2 hour TAT unless you request it or can handle it and you have an understanding with them

If you really want this job, I think I would low ball it with a clause that says when his business picks up and you get more work that you will raise your rates.
If you are getting in on the beginning of this company, there will be room for advancement. You might want to discuss with him that aspect, whether it be an office manager or supervisor, etc. In that case, he might be offering benefits.

and we are not typists either, we are transcriptionists and now medical editors. if you ever worked for a newspaper or book publisher or any magazine, editing is what it's all about.
As for *poor typer* - I type 100 wpm.......but am so much faster with VR because I have XML formerly known as HTML experience for 10+ years and worked for a newspaper years ago...it all came in so handy when it came to even learning VR/ASR 2 years ago.....

One thing I have done is designate an amount of money I will spend on me (be it travel, clothes, makeup, cologne, entertainment, etc.). I worked that figure into my budget. It is part of what I earn on a regular basis so I don't have to worry about doing what you described.
I used to do that. It got really old really fast. I felt I was overworked all the time (not that I don't feel really stretched sometimes now but it is a rare occasion and it always has to do with something other than work when I do). Seemed like I was never going to "have enough" to go on vacation without killing myself.

Stedman's is organized a bit differently sm
and that is probably why I didn't like Tessier's at all. I have other word books besides Stedman's and I don't like them either! The only thing I like as well would be the MPI and I hardly ever use mine now.
In all fairness, I probably Google more than I do anything. If I am looking for a ronguer or scissors or suture or needle, I use the M&S word book to find that. If I am looking up a new drug-eluting stent, I google, BUT I can make what I am looking for so specific that I find less than 100 sites for most things. I don't ever file through pages of it because that is useless too.
I dunno...only ever do OPs, have for a long time and it is to the point where I rarely have to search for anything. Despite my advanced age, my near photographic memory is still highly functional. I thought that would be the first thing to go, but it wasn't. In other ways, my brain is a junkyard. I can remember a piece of equipment I transcribed only once 4 years ago, but I can't remember where I put my purse or keys!

Since I don't believe fighting the Information Age is a good strategy (sort of like hunters-and-gatherers laughing at people sticking roots in the ground, to my mind) I'm signing up for classes to broaden my skills. I'm also looking into reactiving my appraiser's license (in case of future need. I appraised lots of foreclosures in the Reagan era and would definitely prefer to pass now).
Back to the healthcare industry, I would ask you, how does having IT performed on this continent fit into improving the delivery of good affordable healthcare to all of us? What has to be done so that this does serve that purpose? How do we fit into it? Best wishes for sure.

I have had that problem with some whispering dictators, so I mute the program itself just for that dictator.
It's a shame if you can't use it. I get a real rhythm going and my day just flies. I don't think I can type without it now.

Different types of errors are weighted differently ...
so more serious errors count more, less serious count less. I have 8 reports a month QA'd. My QA person listens to the dictation while reading my transcribed report and marks errors and classifies each error. The total is weighted on a percentage.
Last month on my 8 QA'd reports, I had only 1 error and it was a style format error (I used a "dash" as the physician dictated but the BOS2 says to use 2 dashes if dashes are being used and BOS2 prefers using semi-colons). That was the only error of any kind I had and it was not serious. My monthly QA score (because of how this type of error was weighted) was 99.9%. I have had a few times of 100% but I have never had below 99%. My QA person gives me a full explanation of why it is an error and sometimes marks information that is not counted against me but is given as "information only". Noncritical errors are only counted once in a document and critical errors are counted as often as you have them (ex: if you used perineal instead of peroneal --that is a critical error).

...the program that makes typewriter sounds. The typewriter sounds are cool, but how do you hear your dictator over the sound? I have turned the volume down, but still can't hear the dictator over the typewriter sounds. I was hoping this program would help increase my speed (rhythm).

Every version of Word seems to do auto numbering differently. sm
The last two versions have a number list style that will keep numbers at the left margin and do the hanging indent automatically. Alt-Shift-left arrow will take your list back to the left margin if you don't use the style but want to use automatic numbering.

FTP Client
I still don't know what an FTP client is that was asked in the original question?

The client always wants a little something for nothing.....so I give them a free line if there is only one word on the line...I give them a 68-72 character line (spaces I do count). I have never upped them in 9 years, but on my 10th anniversary I am upping them to 13 CPL. I also give them their 5 line headings for nothing as it's only 2 Keystrokes for me. They are good to me so I bake for the offices a couple of times a year and give the offices a gift during the winter holidays.
I do for one client
One of my clients requested that I charge this way. We do 10 words = 1 line, so basically a penny per word. From what I have figured, I'm doing okay when compared to the character count way of charging on this account. I think they requested charges calculated this way because of being ripped off by their last MT. Once I send their files, they can check the word count and keep a running tab to compare with my statement. They're satisfied and I'm satisfied, so it works well.

Call the office of your client and have them fax/e-mail to you samples of their prior surgeries. There's certainly nothing wrong with asking for this.

The client can unzip using WinZip or whatever program they have installed, but MP Tools is so easy, some clients may want to use it. I have a couple who are interested in purchasing it just for the printing function, which works really well.
I had a client ask me to look
They provided a patient list for the date of service, but if the patient was not on there, then I had to view on-line the past week of patient lists to look for the name. Yeah right - that took 15 or 20 minutes sometimes uploading and viewing. So now, they get an inaccurate spelling of the patient name with a comment when I send there report, pt. not on list. Hello? Like the low wage isn't bad enough, and then add in that kind of junk. It is pathetic.

BOS is a style guide, which I do not own. However, I aim to please the client. So, if the doctor wants to spell something his own way, then so be it. He went to medical school, not me! A book of style means absolutely nothing to a client who wants things a certain way. End of story.

wants. Most do I would think, but some want them in a paragraph form and not captialized. Need to check with your supervisor or client.

And ASR is NO WAY 3 times faster...that will never happen. Most transcriptionists type the speed of the dictator to begin with. ASR could be a great tool for those of us needing a break from transcription, but it does not warrant ripping off our cpl's..ESPECIALLY when the client is charged the same...the have NO WAY of knowing whether the report is goes to ASR or not. Lastly, an ASR Editor needs YEARS of transcription experience, becuase to insure a correct report

Unless specified by the client, I only
expand in diagnoses, procedures, impression, etc. I always type what they say when it comes to drugs, you don't change things like two a day to b.i.d. unless directed to do so by the client specifically. Also, on my client we are not allowed to use slang, so if they say "sat" we expand to saturation, bili is changed to bilirubin etc. A good MT never takes it upon herself to change something that is dictated UNLESS they are specified to in certain instances by the client or it something is wrong--then if it is changed you flag it for your QA.
I think relistening to a certain client until you
are very used to them is a good idea. Just put it on high speed and go over it. That won't take near as much time. I am QA and I can most certainly tell the difference between those who relisten or at least reread and those who do not.

A client approached me AFTER both HE and I had quit working for this one particular MTSO...I worked for him, she found out and tried to sue ME for $20K.
I sent her back a play $20K bill and told her that was about as much as her "lawsuit" was worth. Never heard another word from her.
Own client question
Those of you with your own clients. What do you do when you want to go away for the weekend, or get sick? I can see this as being about the only reason to stay with a company. The ability to get someone else to pick up your accounts for a few days is appealing.

Client rate
It depends on the rate in your area, but I would charge no less than .12 per 65 char line. I think this rate is not too high and not too low. You have be careful about charging too much as someone will undercut you. Practicount is a line count program that will allow you to charge for headers, footers, bolded areas, underlines, and so on. This way if you have to drop your price in order to get the client you can charge for other things that will make up the difference.
