The ex-husband was from Bernville, PA, which is about 20 minutes from me. What a psycho. Those poor little girls. You know what, people just should not trust anyone these days and never ever allow them to be put in such a situation. And that means making sure you never go anywhere alone with someone. Of course, that wasnt much alone considering they were in a public place. I dont know. The world is just sick. I personally am looking forward to the end of it to be honest. There are jsut too many people suffering these days.

Washington - How would you feel about your personal health information flowing freely over the Internet between public health officials, healthcare providers, insurance and data clearinghouse companies, and others – without your permission?

If this doesn't sound like a good idea, it's time to become informed about federal health privacy law.
Today, when Americans visit a healthcare provider for services (including dental and eye exams), they receive a form with a title such as "Notification of Privacy Rights." Many assume that signing the form guarantees that personal information won't be shared with third parties. But the form offers no such guarantees. And neither does federal law.
In fact, the privacy rule established under the Health Insurance Portability and Accountability Act of 1996 (HIPAA) legally permits healthcare providers to share patients' information with more than 600,000 health- and data-related entities – without a patient's consent. Yet the notification form doesn't clearly explain this.
Individuals control their information when they give consent; they don't with notification. When you sign a notification form, all you are doing is acknowledging its receipt. The HIPAA notification form offers no control over who sees your information and instead just tells you about some of the entities that can access your information, rather than asking for your permission.
Consequently, many physicians and other healthcare providers are urging Congress to strengthen privacy rights. They know firsthand that the HIPAA rule fails to ensure true confidentiality.
"...[T]he regulations under [HIPAA], which were intended to extend patient privacy as we moved from a paper-based system of medical records to a digital system, are a sham. HIPAA allows the routine release of personal health information without patient consent or knowledge, and even over a patient's objection…" stresses Dr. Janis G. Chester, president of the American Association of Practicing Psychiatrists.
As the ACLU put it, "HIPAA has so many medical privacy loopholes, it makes Swiss cheese look solid." The organization also points out that under existing federal regulations, the term "privacy" hasn't been well defined. The ACLU is urging Congress to define medical privacy as "patient control of electronic medical records."
Moreover, these organizations and others are lobbying for privacy amendments to key health Internet-technology (HIT) bills currently being considered in Congress.
Lack of privacy has serious consequences. It fosters making personal health information a commodity that businesses sell and trade in the marketplace, notes ACLU. Weak privacy rights also interfere with doctor-patient relationships. When drafting the HIPAA privacy rule, the US Department of Health and Human Services (HHS) noted that "Privacy violations reduce consumers' trust in the healthcare system and institutions that serve them." The ACLU noted recently that at least one third of Americans are not sharing their complete personal medical histories because they feel their privacy will be weakened in the name of efficiency. Additionally, without strong privacy rights, individuals can't take steps to adequately protect themselves from bad, lost, stolen, or misused data.
Meanwhile, more and more personal data is being collected during routine healthcare visits, including information about marital and sexual matters. A married woman (wedded for over 30 years) and mother of two adult children, said she was appalled when asked during a routine visit if she preferred men or women. She stressed that while she "has nothing to hide," she doesn't think it's anyone's business what her sexual preference is or when her first sexual encounter was (which is often asked during exams).
What's more, it is becoming easier to share healthcare information with just a click of a mouse. As HHS has noted, "Until recently, health information was recorded and maintained on paper and stored in the offices of community-based physicians, nurses, hospitals, and other healthcare professionals and institutions.... Today, however, more and more health care providers, plans, and others are utilizing electronic means of storing and transmitting health information…. In a matter of seconds, a person's most profoundly private information can be shared with hundreds, thousands, even millions of individuals and organizations at a time."
Do Americans really want the intimate details of their lives and families shared so easily without their consent? If not, they need to urge Congress to establish stronger privacy rights. Tinkering with HIPAA won't do it. That would just keep a lot of people busy rewriting regulations that don't guarantee privacy. Rather, Congress needs to pass a new law that defines "privacy" and upholds the precious ethic of consent. The new law should guarantee individuals' freedom to decide whether to be part of electronic medical-record and genetic databases for years to come.

Does anyone know how to set Page Length in Windows 97? See inside
New account, they need the page to stop at 8.9 or 9.1, and so I have to put in a hard page break.
BUT, does anyone know how to set the page length to be shorter automatically? Something I can create a template or whatever, and when I type on that account, the page length will automatically break at 8.9 and then I will know to put the hard page break in when the machine does?

In Windows, how do I check my computer's IP address or physical (aka MAC) address?
Windows NT, 2000, and XP
To see what IP address, default gateway, and subnet mask your computer is using:
- Click the
Start button and select All Programs (XP) or Programs (NT and 2000).
- Select
Accessories, then Command Prompt (2000 and XP), or Command Prompt (NT).
- At the command prompt, enter: ipconfig
- To close the command prompt, enter: exit
For other IP information on Windows, including the physical address, at the command prompt, enter: ipconfig /all | more
This will give you information regarding your computer's TCP/IP setup, including:
Host name
DNS servers
Physical address
IP address
Subnet mask
Default gateway
WINS servers
The IP address will be a series of numbers following the pattern xxx.xxx.xxx.xxx, for example, 156.56.27.32.
The physical address, which many people call the MAC address, will be a series of six hexadecimal number pairs following the pattern xx-xx-xx-xx-xx-xx, for example, 00-40-A4-F3-C0-01.
Note: If you have more than one active network adapter, you will see multiple entries here. Many laptop users nowadays have at least two adapters: one for the regular Ethernet adapter, and one for the wireless 802.11b or 802.11g card. Some computers will have three entries or more (for example, for either an infrared or a Bluetooth adapter). Sometimes it's easy to distinguish between the two (for example, one of them will say "wireless" next to "Description:"), but other times it's not (the description will give only a manufacturer and model). You may have to consult the manufacturer's documentation for your computer to tell which section of the ipconfig/all | more output refers to which network adapter.
Windows XP, without using the command prompt
Alternatively, in Windows XP, if you want to find this information and want to stay within the graphic user interface, or for any reason do not want to use DOS commands, you can find this information by doing the following:
- Right-click the
My Network Places icon, and then select Properties.
- Double-click the
Local Area Connection icon. Occasionally, there will be a number on the end (e.g., Local Area Connection 2); it depends on your computer's configuration. If you are looking for the MAC address for a wireless adapter, the icon will normally be named Wireless Network Connection.
- Click the
Support tab.
- Click the
Details... button.
The physical address (MAC address) and IP address will be listed in the window that appears.
Windows 95, 98, and Me
To check your computer's IP address or physical address, follow the steps below:
- Click the
Start button and select Run... .
- Type
winipcfg and click OK.
- From the drop-down menu, select the adapter being used for your connection:
- Select
PPP Adapter if you're dialing in.
- Select the Ethernet adapter if you're connected to a LAN or a broadband connection.
- Select the wireless adapter if you're connected via wireless.
The IP address will appear in its labeled field. The MAC address will appear in a field labeled Adapter Address.
Alternatively, you may follow these steps:
- Click the
Start button and select Settings, then Control Panel.
- Double-click
Network. Select TCP/IP, then click Properties.
The IP address appears under the IP Address page tab.
Note: If you are using Dial-up Networking (i.e., PPP) or DHCP (campus housing at Indiana University Bloomington uses DHCP), the IP address is dynamically assigned by the terminal server, and Obtain an IP address automatically should be selected.
Note: You can use the Get Connected web site to register your Windows XP Professional, Windows XP Home, Windows Vista, or Mac OS X (10.3 and later) computer to use the IU network in campus housing. Computers running Windows 2000 or Linux, as well as game consoles, must be manually registered. For help, contact the UITS Support Center at IUB by calling 812-855-6789 or at IUPUI by calling 317-274-4357. You cannot register Windows 95, 98, or Me on the IU network in campus housing.

I'd be up front with them and just flat out tell them that you'd rather they didn't contact your current employer. Most companies understand that. Offer plenty of references so they have a way to gauge your work.

usually on my note pad or a patient list, makes it interesting too. But she doesn't usually stay long, and if the dogs are in she hides ---her and the Husky are mortal enemies, granted it does not help that he tried to eat/kill her a couple weeks go-- our signals got crossed and my husband let the dogs in before I could up up the gates to ensure her safety. Luckily I was right there and was able to grab the dog before anything really bad happened. So my husband learned an important lesson that day...communicate with me when you let the dogs in, or put up the gates first.
Had a drive-by shooting right in front of my
home office window in July. Unfortunately, a baby got shot in the head. I thought it was someone setting off cherry bombs or something - the pops were too close together I thought for gunfire. It was 2 girls and a stroller and they didn't even realize the baby had been shot at first. It was a big howdy-do. The cops were out here for hours, blocking off the streets so they could look for shell casings. A couple of news trucks too, although the subsequent TV news reports were somewhat inaccurate regarding the location and details. Never found out what happened to the baby.
Anyway, right after this happened, the guy and teenager kid across the street stayed away from their house for about 2 weeks after, coming by periodically to check on the mail. The cops would occasionally call on the house during that time. I never did find out what the deal was and if there was connection between these people and the shooting, but I always worry about a gang thing or a home invasion thing and they get the wrong house (mine).
OK. Now there are construction guys out front
using a big saw to cut through our new concrete sidewalks that have already been dug up twice since being put in this spring. I can't hear to work. They didn't even warn us of any construction or water shutoffs. How many times do they have to dig up the same old spot this year before they get it right?

I think I would leave it up all year and decorate it for the different seasons and leave it on all night. I used to know a lady a long time ago, giving away my age here, who had one and had it all decorated with crystal glass ornaments and red garland. I liked it but my mother said it looked like a tin can.
How did they get past the front desk and
where was security when they didn't take no for an answer the first time? Having worked in a psych facility (which has a higher level of patient confidentially) and tyring to be "forced" into giving information not able to be given, it became natural to just say no. NO ONE on my watch got the first hint of information they were not entitled to. People may want information but getting it is something different.

You don't need a macro. Just add a semicolon in front of
your yo abbreviation so autocorrect will back up and take out that space. Then type 34;yo and it will expand to 34-year-old.

Front-end/back-end systems...
"Front-end" SR means that the original dictator's speech is turned into text. "Back-end" SR means that someone listens to the original dictator, redictating and "cleaning up" the original dication. This "sanitized" redictation is then fed into the SR engine.
The main disadvantage of "front-end" systems in the healthcare setting is that they must try to adapt to the speech peculiarities of many different dictators, whereas a "back-end" system only has to deal with the speech patterns of one dictator, namely the person who does the redictation.
Back-end dictation adds a layer of human intelligence to the conversion of speech to text because the person redictating presumably catches errors, eliminates hesitations and miscues, squares up ambiguous sentence structures, etc. - things that front-end dictation cannot do or does very poorly. As such, while the back-end speech engine might still cause some errors that require keyboard correction, there should not be nearly as many as front-end systems produce. And, a back-end SR application can be less sophisticated than a front-end system because it is not being required to deal with so many vagaries of human speech. In fact, a back-end SR application does not need artificial intelligence at all, whereas front-end SR really does.

Does anyone out there know of a website where I could get some practice dictation? Have been away from transcription for a few years and want to get my "ear" back and some speed before trying to get back into it. Thanks for your help.

I was wondering if anyone has any ideas on what you should charge per line for a Family Practice of 4 physicians? I am interested in working for this practice that currently has been doing their own transcription (oh heaven help! lol), but did not know what would a fair rate to charge per line...thanks for any input!

Practice dictation sm
This is a crazy question but I've had to take time off and feel really out of the loop. Never worked from home before. Is there any company who would forward some sample dication to let you kind of play with for awhile before you formally test for them? At this point, I don't think I'd pass the test although I've had plenty of acute care and specialty experience. I know it sounds really unprofessional but I thought I would at least ask. Hate to fail, spent too many years being a perfectionist, I guess. Are there any sites you could recommend for someone returning to the profession after a few years taking care of family business?

I know it hurts at the moment, but it will help you in the long run to have a bigger variety of dictators and specialties. When I was doing radiology there were only about 15, and all but 2 were quite good. Now doing acute care again I may not have a repeat all day. It does make a big difference in production.
Practice tapes from?
I have heard practice tapes some give out that are almost perfect dictation- definitely not what you run into when you transcribe. Are you doing the "goodie" tapes or are they the real McCoy? The practice ones I have heard put in their periods, paragraph. Ha, very seldom hear that in real. What kind of work will you do, speciality or the 4s? I have my equipment as fast as it can possibly go and my salary has gone down in the past 4 years 20,000$ from the constant outsourcing the companies are doing. My speed is between 130-140 wpm and I make good only because I have my nose to the grindstone most of the day. By the way, independent as well but my company asked me for certain hours and which days. I have seen numerous posts on here saying you do not have to do it -but I wanted to work for this particular place and I accept it.

Practice tapes are real; slurring run-on sentences, gurgling muffled words, you name it. But, she gave me the printed reports to go with them, so if I really get stuck, i can look at them and find the answer. If you don't mind my asking, how long have you been in this, and what's your cpl? You work for an agency? Yes, my speed is concerning me. Thanks for your reply.
If you are an employee at this practice sm
then it is not your decision who they hire. It is probably in your job description to train new employees. You may want to ask for a copy of that before heading into Dr. Whozewhatsee's office to complain. The other thing is even though you have a background on this girl that does not mean she is terrible (innocent until proven guilty). In my opinion, when I read your post, I am hearing "this job is mine, mine, mine, and mine". "I am to make all of the decisions because this job is mine, mine, mine." Sort of like a 2-year-old. In saying that I'm sure it will really make you mad. I am hearing from your post that you really do not want help but you want to be off to have surgery, etc. The practice cannot just back-up their work but unless you are a supervisor (25+ years does not make you that), it is the decision of the office manager on who to hire. You may be better off with this young girl as opposed to an outside service. The outside service may very well take your job by underbidding and letting the practice know that they can get the job done and do not take off for back surgeries, etc. You may want to keep the waters smooth until you get back. I would do my best to keep this girl going on a steady pace. JMO. Good luck.
