got all ASR reports today and I learned the patient's extremities had "no good hernias" and in the next report they had "no anemia."  One patient suffered from "adequate abuse" (meant to be alcohol abuse).

Dictated: No adenopathy of the supraclavicular and cervical chains
ASR: Elizabeth is not suppose

and for some reason the word "thumb" shows up in ASR all the time - when the dictator says "umm..."

all in a day's work
