code 561410 is document preparation services, and it is about as close as you are going to get, I think. There is nothing specifically 'medical transcription' listed.

As far as the W2, I'd contact the company and make them send me the correct W2. You may get into some trouble if you have two W2's and only pay taxes to the one state, so make them fix it.
