We have had it for almost a year now. We live in a relatively rural area, more than 50 miles from even a medium size city and we have cable internet. The phone service is only as good as your internet service. Cable internet is relatively new in our town and we pay for the highest speed they have but still it can get slow. If your internet is slow, your phone service won't be very good. It is usable for us but there are periods when the person you call can't hear you, mostly after storms and such. My cousin has had it for 2 years and lives in a metro area and has never had this problem, so I tend to think it's because we are remote. I use it for work as well with very little problems.
