My aunt "trusted" the trusted old surgeon who told her not to worry, we'll deal with it IF it becomes an issue. Well, it did but to late to treat and she died within 3 years.

WORKING INHOUSE NOT ALWAYS SO WONDERFUL, EITHER.
I worked in house for the only hospital corporation available in my rural area. I expected to learn a lot. I actually learned very little with regards to transcription. I learned a lot about hospital politics. They had some of the worst dictators there, and the HIM director did not like confrontation. Also, the main task given to me was to accomplish a line rate equal to 1200 per day with spaces, at which time they would set me up to work at home. Unfortunately, I only had 6 months in which to accomplish this. I was flatly told that if I did not accomplish this production rate, I would be fired. Before that happened, I found a job online, and will never return inhouse again. I also have to say that I don't miss driving 300+ miles per week in order to do that inhouse job. Also, I wasn't thrilled with the prospect of the hospital's tech person visiting me ad lib at home.
Now, I can't say that I'm thrilled with this profession. I have the same concerns/complaints as the original poster of this thread. However, comparing inhouse, with inhome, I do believe that I've learned more and gained more valuable experience through the inhome process. Yes, I have worked many hours and more than 1 job at a time. When I research previous jobs by a doctor, I also notice that previous jobs have been submitted with lots of errors in them. I have shared my concerns with my manager and believe that nothing was done. I have also complained about poor dictators and why, oh why, are these doctors still allowed to conduct themselves so poorly? When I worked in the hospital, I noticed that the HIM director let the doctors do whatever they wished because she was afraid to confront them, like they were Gods or something. It is my opinion that this is a major problem in our industry with regards to the people who are the liaison between the MT and the doctor. Somewhere, a doctor has a supervisor, office manager, director, hospital administrator, etc. If a doctor is in private practice, he may care more about the quality of his product. But, if a doctor is affiliated with a hospital or other facility, he might just be doing a job and as long as he/she can get away with it, he/she will be as sloppy as he/she wants to be.
It is also really frustrating to be told that you have to do 150 lines/hour or more, or lose benefits or be fired or whatever. At the same time, it takes time to really produce a good report, given the deficiencies in the dictations. I panicked about this in the beginning, as I was doing about 50 lines per hour. I'm happy to report that it does get better. I also quit the job with the production quota.

That patient's life can be altered for good or bad depending on what is on that medical record. That is what has been forgotten over the years and is still yet to be learned by alot of newbies. Personally I think one of the worst things that has happened in this industry is the salaries based on cpl! I wish that places would go back to paying by the hour and pay good decent wage for your experience and knowledge.
I realize that the sheer volume of what we do is what has caused things such as production pay, paying by the line, etc to come around but it has also created a mindset of "how many lines can I type and how much money can I make" totally forgetting about the patient.
Years ago MTs in MR Departments were responsible for things like getting HPs on charts for preops in a timely fashions (that was one of the most important things) as the law states you cannot put a patient under anesthesia without that HP on the chart. There was none of this business about not liking certain work types, ESL dictators, OP reports, etc. etc. Seemed like there was more pride in the profession then.
Keep doing what you are doing as you do it. You are still helping the patient's whether anyone realizes it or not, you know it. The next life you impact could be one of your own family or friends.

and so sorry about your child. But, when I was inhouse the very same thing happened to me. It was terrible. But I was a single mom and needed every dime...And I agree with treating others with kindness and respect... and treating ourselves with kindness and respect too. I always have the saying do no harm... but then I forget about myself.. and I blame myself for everything instead of forgiving. For those of us working as an MT for a long time, or those who are new, we need to remember to forgive ourselves as well as others, b/c we live just once and there is no use wasting time down in the dumps. All things pass and we learn from every experience. Best to do no harm no matter what! Thanks for making me think today... usually I just dwell...

If it is your heart and you wait you could die. If it's not your heart, you'll at least know that. I don't think the heart is anything to fool around with. Too many younger people are dying from heart disease.

Most MATURE men don't stick to the code. They tell their wives. They want their wives to stay out of it of course, but I think they also realize that in telling their wives that it is KNOWN that the wives will probably blab it to the point that it will become public knowledge (blabbing is the female unwritten code, LOL).
I don't really think the OP should contact this *woman*; however, I think she should ask her husband to tell his friend to inform her to not involve him AND to not give out his number or involve him in friends affairs ever again. If OPs hubby won't do that then there are already existing problems.

And, here I sit after my job was outsourced to C-Bay even though my "manager" claimed she did not want to outsource.
You need to consider the fact that your manager probably does not make those decisions, but the higher ups that are above him/her do! This is the very first signs of outsourcing (new system) to comply with the outsourcing company.
The best way to get a new system set up for outsourcing is by using the staff on hand. No, I doubt you'll get a raise, but you may get a big surprise once you set that system all up for that outsourced company.
It happened to me, and I had a manager just like you are stating. If I were you, I wouldn't make any waves regarding a raise right now. If you are hourly, then I wouldn't worry too much about the line count and when asked why your line count isn't up to par, then reiterate the fact that you've been instituting a new system.
The hospital I worked at also asked the MTs to help because the managers had no clue. So sad...
Oh, and they did all of this after hiring a very older lady (partially retired) from a hospital that was outsourced so she had to quit. That lady lost her job at the hospital I worked at about a month after I left. I left before the outsource because I refused to help outsource the work I had done for the past 17 years.
P/S: Just keep your eyes and ears open very wide!!!!!!

My son beta tests these programs and this one is not ready yet. It conflicts with other programs at present, especially any older programs (pre XP). Better to wait until it has been out for quite a few months and the bugs are worked out.

I'm sitting here thinking it is way past time to leave this business. It is too complicated. It has not become more standardized. If anything it is more loopy - each place has a different platform, hospital rules they want you to follow for each thing. Used to you could turn on a typewriter, put paper in the machine, type, put a sticker on it, and you were done. Now it takes sometimes 3 months to get up to speed - NOT because you don't know medical terminology - you have to learn a specific platform (go buy special parts to use it), figure out new expanders, get special internet thingys, get wave pedals, throw away $$$ super stations and C-phones. It's a joke. They want us to cover all that just for the right to make squat for money.

Ditto....granted I am not working much these days, but am currently trying out a new way of working
when I do work which seems to be helping. I am timing myself and keeping a log of how long it takes to to type however many minutes. I am averaging anywhere from 13-18 minutes of dictation an hour now doing this. Granted the time fluctuates between who I am typing, and if I have to look up names, addresses, etc., just depends on the division I am doing at the time. But work that used to take me 3 hours to do is now taking me under 2 hours, I am also trying to put in more macros as I go along, which slows me down initially but pays off in the long run of course. I was working "all the time" before but took forever to get done since I was not applying myself. This new "attitude" has helped me a lot. My goal, in the Fall, is to do 90 minutes a day consistently at 6 hours, and then maybe get up to 120 minutes a day at 8 hours, still while having at least half of the day free (do 60 minutes at night, and the other 60 by Noon). Thereby doing 1200-1400 lines a day. I have never really buckled down and done more than 8000 lines per pay period, so it will be a nice change. Maybe you can do something like that and have a specific knock off time as was suggested below.

that holiday you refuse to work? Grocery stores, food joints & seems any store is open on holidays and you expect them to be fully staffed, so why shouldn't we?
Yes, I took off for the first time on Christmas day, in 18 years and it was great, but I worked Christmas eve and this whole past weekend.

How did you go about this? Are you a parent, and if so did you wait until kids were older? Not trying to be nosy, but I have been thinking about this a lot. My kids are still relatively young, but as they get closer to both being in school full-time, I have been thinking about going back to school and studying something in the healthcare field. I think becoming a doctor is too far out of my reach, although in another lifetime, that would have been my dream. But, maybe a PA, or some type of therapist, or a nuclear medicine tech?

The hospital where I worked before going to MQ 10 years ago would always play it up big time. I miss that, but MQ has made a bit of a dent this year (well, I say a bit because I haven't won anything LOL). Food and a new reference book -- can't beat that!!! (well, um, maybe the Tiffany bracelet would :-)

What a wonderful day...NOT
My one doc got laryngitis on Tuesday...he's still dictating but only in a whisper. Yet, when he had part of his voice, I couldn't understand a darn thing! Whispering was definitely better. LOL
I took a break from my other job to run to the store...15 minutes max. Put our puppy outside because I don't quite trust her in the house by herself yet. Came out of the store and my BATTERY WAS DEAD!!! Didn't have the strength to open the hood of the truck all the way (I'm no spring chicken), then couldn't get it closed because I'm too short.
Thanks heavens for cell phones. Beeped hubby. He was 50 miles away. Called 1 son. He was 30 miles away. Called foster son. Wow. He was home!!!...but sick with the flu and he only lived a mile away. After waiting 45 minutes, he finally arrived. Surely he would be able to get it going...nope not even with the extra battery. Truck still sitting in the parking lot. Poor pooch was so very happy to see me come home.
On top of this, it's my birthday {{{shudder}}}
I think I'll just stay home and keep working from now on.

response to people who are new to the profession. It's good to see someone give some great helpful advice to someone who wants to get into the field. Good for you! I was new once and I remember how hard it was to enter the profession. It's still hard to obtain new accounts because this is such and invisible job, but I love working at home. I bet all those newbies appreciate your advice!

Never been happier to switch myself. All in all i am saving over $100 a month and the only time my net went out for longer than an hour was when a line was cut and AT&T customers were out too. It is a shame that AT&T was allowed to buy up the smaller companies. Just ruined the whole thing.

to produce like that. I agree somewhat with the poster who does doctor office notes. I worked for a medium-sized clinic and did very well. I had one psychologist who would dictate an eight to ten page initial assessment, and then when the testing was done, he would have you copy the assessment (mind you being paid for those lines again), and add the testing information. I made out very well with him. I also had a lot of standards under the Objective and Treatment Plan, which helped with my line counts. I just saved my format in autocorrect and popped it in with the information already there. When I transcribed the initial assessment, I saved the whole thing in autocorrect under the patient's last name, and when the testing came through just popped it up and added to it. Very easy lines.
