QA -- WHEN DID IT START? WHO INVENTED IT, ETC??
This is a question for those of you who are familiar with the history of medical transcription.
When was the QA process started? Who started it? Why aren't doctors and other report dictators put through a QA process? Are there other points about the history of QA for which I have not posed a specific question here? Please feel free to share whatever history you know.
Why do I need to know? Well, I don't need to know, but I am just really curious. While I may write a fictional story about an MT who murders a dictating clinician, I'm not sure if I would include the background information in the story.
As far as I can recall, none of these issues were covered in the Career Step course that I completed 3 years ago.

Has your pay already been cut by MQ? I am an IC for MQ and this has not happened to me yet. Is it a proven fact that this is going to happen or just speculation?

since the beginning of time. It's not just the 'religious fanatics' who have changed. The human race has evolved in their beliefs since Adam and Eve. Christians don't follow the rules of the Old Testament per se, because Jesus fulfilled the laws. There aren't too many religions who still make animal sacrifices. A woman who is having her period is no longer considered unclean. Pork is not considered taboo in the Christian faith, and Catholics aren't limited to eating only fish on Fridays. There were religions who believed in human sacrifice, but I doubt that goes on very much any more.
People have evolved in that we no longer believe the world is flat. We don't believe you are going to get AIDS by touching someone who has it. We no longer believe that a woman who is having her period can't go swimming.
Who believes the same things they believed when they were six? How different are your beliefs than your parents? Not just about religion, but about anything.
I see it as a career BUT so far it hasn't ...sm

Because the state she lives in does not recognize legal separation, she had an agreement drawn up by her corporate atty. This agreement handed the house (quick claim deed) and business over to her, while her "future ex" draws his pension and moved to warmer weather (California). This all happened when things were friendly. Now she wants a divorce so she can get on with her life (with her new beau she is buying the house with). BAD idea since she has not filed for a divorce. i think he is totally obsessed with the case. whether he did it or not, i am not sure. he can't give details?? the puzzle i am not fitting is the 118,000 ransom note that patsy new about. he has admitted it was a hostage gone wrong. i am wondering if maybe somehow he and patsy worked together on this one and now the reasoning it is coming out since patsy is no longer here. he lived in Georgia where they did but jonbenet was a baby at that time. it states that were vaginal abrasions but no semen? maybe that was a cover up to make it look like an intruder but they kinda left out an item called semen to make it real. makes me wonder if her hymen was intact. he is saying he drugged and raped her but yet autopsy says no drugs in her system.

It hasn't been allowed since HIPAA. MT companies who outsource...
get away with it because they don't tell their clients, and internet wav file capabilities enable them to do so without anyone knowing. Some policy and investigation needs to be enforced to stop this. I say start slapping companies who outsource with huge fines and see how long we have to deal with outsourcing problems. What gets me most is "I can't even get a copy of my own medical record from my doc unless he has a release of medical records form signed by myself on file in my chart, but a doctor can dictate John Smith's medical history, send it to a company who in turns sends it out of the country and back here! Ridiculous.

In fact, just the opposite has happened. It's just a pocket-lining scheme, and nothing more. We had plenty of respect and a decent income back in the day when we had no 'CMT' scam going on. It isn't being credentialled I have a problem with - it's the group claiming that it's 'necessary', who is trying to get it mandated, and who would ultimately PROFIT IMMENSELY if that were to come about. But would WE profit? Nuh-uh. It hasn't made being an MT (credentialled or not) a better or more rewarding career, and if fact, just the opposite. So I SERIOUSLY DOUBT that if they were to succeed in making it mandatory that only CMTs could work, that those MTs would derive any viable benefit from obtaining the title. Most likely things would just get worse.

Word hasn't capped after a colon since the 2000 version.
You have to manually cap with shift key or run a macro at the end of the report. You can also hit shift+f3 to change the case to upper.

It seemed a little rushed at the ending. I was also expecting to cry or get emotional or get shocked like I usually do. It was very mundane and the ending was rushed. Looks like everyone died exactly the same way, except the gay boyfriend.

 Quit acting like an ostrich and pull your head out of the sand. Pick up the paper some time and read it or tune into the news when you can fit it into your busy schedule because it's really important for you to know what's going on in our world today. Of course, your other alternative is to keep doing what you're doing, with your head stuck firmly in the sand, thinking companies are paying bucco bucks/line rates in today's world economy.

Six feet under drama
Nate sure left a mess, so far they've bad mouthed the Iraq war, fantasized brother and sister romance (shudders), where else will they go? Who's going to be a goner in the finale, I feel its coming. They've set it up where anyone can either off themselves or do something crazy and accidentally die.

cold feet
I bought a cheap electric heater and run it pointing towards my feet.
Only one here, but it is 9 feet tall...
and it takes my 6'8" husband to help decorate near the top. Merry Christmas to all.

crow's feet...
I am beginning to notice crow's feet around my eyes  ...help! Any advice on a good eye cream to help prevent these from getting any worse would be sooo appreciated!
swollen feet
I had swollen ankles/feet last year and found out it was my high blood pressure medication causing it, so was switched to another, but then developed a cough, so then switched to Benicar with no problems. Sometimes medications can cause the feet and ankle swelling. My doctor also suggested getting up every 1/2 hour and walking around as all this sitting just does a number on our bodies. Epsom salt soak will help also. Mine occurred during the hot summer months.
RE: swollen feet
Mine swell, too. The only thing I can think of is to drink plenty of water throughout the day and walk around a bit when you can. Also, during your lunch break, prop your feet up. If you soak them, use tepid water, rather than hot.
I warm my feet.
When my hands are cold, I warm my feet. Not sure why that works, but it does! I have an electric heating pad under my feet (type barefoot or in my socks) and it warms me up. I also have a blanket I crocheted and that helps too.
Hair and feet
My feet swell a lot after sitting all day, so I also make sure I get up for at least 5 minutes every hour and stretch. I stretch my arms, shoulders, legs, etc. and walk around to get some circulation going. This does help.
My eyes started going too, so when the headaches were everyday, even just glancing at the monitor, I knew it was time to make the visit to the eye doctor. I found an excellent doctor close by my house that knew what he was doing, as I had been to another doctor that screwed me. There is such a thing as "computer glasses" and the doc spent much time with me asking questions on what I do, how close to the monitor, etc. The first day of using my new glasses, no headaches! I had visited the other doctor and was told I had "computer glasses" but after showing them to the new doc, he said they were nothing but regular glasses with not even a prescription in them.
For your hair falling out, I have scalp psoriasis, so when my stress level increases, my hair falls out even more. I have been under tremendous stress lately and actually have handfuls at once coming out. I have been to the doc, had all the tests done to make sure it wasn't thyroid or anything strange. I would suggest you see your doc and make sure of that too and go from there.

Typing with feet up sm
There were many times when I could have used this advice, would love to see some pics. How I have suffered over the years with tapes! Things are getting better and it's important to take care of our legs or we could get DVT. My PT suggested sitting on a balance ball which she said a whole insurance company does. I was petrified of the balance ball at first but it really is something you can get used to. (I never did use it for work.) I also have seen pics of people working on a treadmill, standing with the laptop on a raised platform. In my younger days I think I could have done this and remained much healthier. Where there is a will there is a way. How I wish some of these options would have been available to me during the years I suffered with winding tapes, sitting for hours and in pain. Just when things are getting better -- along comes outsourcing! Perhaps things will change for the better for the newcomers, I sure hope so. Please keep posting your new ideas, there is a better way. After all, we survived electric typewriters, carbon paper and White Out! Keep on keeping on! I love the idea of working in a recliner, how much better for our backs, just be sure to get up and walk around and exercise now and then to keep the circulation going.
I have a cat who dashes in front of my feet
I'm thinking of using one of my dogs squeaky toys to get his attention when he cuts me off again, that will show him!
