I'm sorry that there appears to be some issues with organization on your end, but our agreement was for transcription services only. The additional secretarial tasks were not discussed at the beginning of the contract nor included in the original agreement and are not compensated for at the current line rate. Therefore I must insist upon an additonal x cpl or an hourly rate of x per hour to compensate for this additional workload.
That's what I would say or something similar. If she gets snippy, just explain you are running a business not a charity.
There is the possibility she'll choose to drop you, but she sounds like she is in desperate circumstances, so I doubt it. She may refuse to pay, in which case you will have to decide whether it is worth it to you to drop her or continue working for her, doing these tasks for free.
