Two meanings.

Some people use the term electronic medical records (EMR) generically to refer to records stored in electronic form rather than in a paper chart.

It is also used to refer to a system in which the provider does not do a dictation but rather sits down at a PC and fills out a computer form by checking off boxes and typing in short phrases as necessary to explain the treatment rendered. No dictation is ever done. The Veterans Administration has used this kind of system for several years. I was personally displaced in 2005 by one of these systems that was installed in an ER I had worked on for a couple years. It is certainly a threat to MT.

Incidentally, a system which does not require a dictation obviously does not require that the provider speak much English, but only read and write English and use a computer mouse. That makes the practice of medicine in the U.S. much easier for a lot of doctors though perhaps not for the excellent dictators.
