Contact the Attorney General for that state. Send them a letter stating the situation and enclose the bill. Ask if they can do anything to help you. You might get paid then. And, let that physician know you are reporting him to the attorney general of his state. Next, report him to the BBB in that area, as well as the wage and hour division. Next, report to the AMA to have his name reported as nonpayment of HIS bill. Let him know all the agencies he has been reported to before hiring a collection agency, as they will take a portion of the collection.

You may very well get paid when he receives a letter from the attorney general. You might want to call them and get their fax number to expedite the situation.
