Transcription Gear user questions - sm

I just started using Transcription Gear/FTP and have some questions.


Do you keep TG open all the time? Do you keep Gear Export open all the time?


How do you send your work to the doctor? Right now I create my document in Word XP, save it, exit, and then go through several steps to paste the document to the medical records FTP site.  Seems like too many steps.


Any help with this and/or with TG in general would be most welcome.

I use Olympus with the company I work for now and it works well. Transcription Gear has a system that I do not think is quite as expensive. There is Bytescribe as well, and Express scribe (which is free). I think almost any handheld recorder will work with these.

Transcription Gear FTP Question
Yes I leave Gearplayer and GearXport open all the time.  That's how i get my files (GearXport). Have used Transcription Gear and GearXport, and recently GearFTP for 8 years now and love it.  I, too, use Word XP but there are help screens in GearXport that help you write rules so that GearXport will direct the files to your clients.  Once the files are transcribed and sent via GearXport, I then send the client a notification to pick their files up on Gear FTP.  You don't have to do all that cutting and pasting.  The rule writing can be quite cumbersome if you have never done it, but the tech support staff at Transcription Gear are very helpful.  This really works great and will save you LOTS of time once you get your rules written.  Best of luck to you!
Transcription Gear/Xport- sm

We have been using this system for about a year, but have a recurring problem: We send our work through FTP but the office cannot always retrieve it and when I talk to the TG tech support they tell me that they can see that my work is "sitting in the FTP site and that the problem is with the doctor's office. This, of course, does not "sit well" with them and we are now setting up a meeting with their IT person, etc. Has anyone had a similar problem?

Hi everybody. . . I have some more questions about at-home transcription.  Can anybody tell me the definitions to some terms or a glossary where I can find them. What is an IC? What is acute care? What are Basic 4 or Big 4 Reports? Also, when a company advertising for an employee says they will provide the software, does this generally mean just the transcription software? Does this tend to include text Expander and spellchecker as well? Thanks!


Questions about EMRs and transcription integration
Hi everyone! I am trying to understand EMR and where transcriptionists fit in, if at all. I see a lot of TASPs that claim they can be integrated with EMR systems. I have no idea what this means. I've even called them and they can't explain it to me. My impression is that EMRs work like big databases with fields that you fill in with information, but I've never actually seen one so I'm not sure. Do any transcriptionists work on an EMR-type systems or know anything about them?

Cardiology and sleep study transcription questions

I am looking for other Transcriptionist that have cardiology and sleep study backgrounds. I am a newer MT, I am starting a cardiology and sleep study transcription account next week and have never transcribed for this specialty.


I wanted to know if there are specific word books or websites that you find helpful and are there specific things that you could tell me about the specialty that may help me?

When you work for yourself which voice player do you prefer?  I have tried all of the above players and all of them seem to distort the voice when you slow it down.  Why can't these players slow down files like the platforms MTSOs use?

Meditech questions and Cerner questions. Any input is appreciated. SM

I just recently went back to work in the office at a local hospital. We type in both Meditech and Cerner, depending on what you are typing, i.e. Medical Records reports versus Radiology.  We are currently typing blindly in Meditech, meaning we do not have access to our line counts and must depend on management to give us our totals.  In Cerner, we are able to run a line count report and print out a hard copy of our line counts at the end of the day.

We are finding that 800 lines in Cerner equals about 500 lines in Meditech.  We are not paid per line nor do we have an incentive plan, but we have a new HIM Director who would like to institute an incentive plan and the administrator over our department has laid down the law and decreed that all transcriptionists not making minimum in the department will be written up.  Problem is according to Meditech line totals that we have been given, that would be about half the department.

The transcription supervisor has taken our concerns to the new director who told the MT Supervisor to hold off on writing people and has taken home all the line counts from the last month and she is going over them with a fine tooth comb this weekend.

I am not a very trusting soul by nature, so I am doing a little research on my own.  I've been in this business for about 15 years.  I started out in a hospital and have worked for a few services at home and am now back in the hospital setting.  So I know a few things about line count and I consider myself to be pretty well informed on trends and technology in the industry.   I would like to walk into our next department meeting armed with as much knowledge as possible.  So I have come up with some questions that I would like some fellow MTs to answer, give opinions, and maybe even direct me to websites or individuals who could give me a better understanding of Meditech and Cerner.  Here are my questions:

 


  1. Is it possible for an MT to have access to her line count in Meditech or is strictly a management function?  Any MTs that work in Meditech on this board, do you currently have access to your line counts and if so, how do you check them?  Is it an option on the main menu after you log into Meditech?

  2. How is a line defined in Meditech?  (i.e. 65 cpl, with spaces, without spaces)

  3. How is a line defined in Cerner?

  4. What abbreviation Expanders work with both Meditech and Cerner?

  5. Examples of incentive plans (if anyone would like to share that with me that would be great, if not I completely understand).

  6. Measuring productivity by minutes of dictation versus typed lines.


    1. What would be the minimum minutes required for an 8-hour day?

    2. How would one structure an incentive program based on minutes?

I was thinking of presenting the idea that we keep track of productivity using minutes instead of lines since there is such a discrepancy in Meditech.  I also worry that if an incentive plan is put into place and we are still typing blind into Meditech, what's to keep management from shaving off a few lines here and there?


Feel free to email me or post here any responses.  Either is fine.  I really appreciate any input, comments, or just plain old supportive posts would be nice too! 




Anyone using Gear player?

Sounds like a really good package deal, anyone using it?  I am thinking of using the digital recorder and buying the gear player package.

i downloaded the free 5 day trial of gear player to take a test.  Cannot get it to work.  The icon is on my desktop, but when i try to open it to drop the voice file in, it only appears for a few seconds and then nothing.  any ideas??
Gear Player is the best.
I have been using Gear Player for 4 years now and it just keeps getting better. It plays what I need it to play. I had tried StartStop before Gear Player and it was like I was back on Windows 95. Above all I love Gear Players support department, the guys are very friendly and can fix anything if you ever do need to call them.

I bought Gear Player
a few years back and it's been really good. Just throw the file in there and transcribe away. Check it out. Their latest model is probably even better.
Help with Gear Player linking
I'm trying to do a test file, which I haven't done in a long time, and the only footpedal I have is Gear Player.  The sound file was sent on Windows Media Player.  Are these compatible?  Because I can't get this footpedal to link to it.  Bear in mind I'm technically challenged and usually have help setting these things up.  I found a foot pedal test picture that lit up when I pressed each of the three footpedal controls, so the problem seems to be that I don't know how to link it to Windows Media Player, or else this foot pedal is incompatible. 
VPEDAL AND GEAR PLAYER
Help plz.. does anyone use the VPedal (from Career Step) and use it with Gear Player? I can get it to work with Express Scribe, but not Gear Player. It it even compatible? Any suggestions would be helpful.

Gear player or Bytescribe

Gear player vs. bytescribe... which is the more versatile? 


I am currently using start/stop, but the voice files I get it will not let me speed them up.  Dives me INSANE to listen to a sssssllllllllooooowwwwww dictator and not be able to speed them up.... actually would like to go whomp them up the side of the head .. but since that is not an option...


Looks like Gear player you can buy just the software, but Bytescribe you have to pay another 50 bucks and get the foot pedal.. no option that I could find not to.

Gear Player vs Express Scribe
could anyone please explain to me the difference between these two??  Express Scribe is free, but when am I missing something as to why I would pay to have Gear Player?  they seem awfully similar..thanks.
Express Scribe, Gear Player, etc.

I have used Express Scribe for years and love it. I had to restore my Win XP computer and now it will not play the .dss files.  I also have Express on my Vista without problems.  I have done everything that I know to do to get it to work, installed an older version, downloaded the .dss files, etc.  I am currently having to work on Gear Player or use my laptop to play the sound files.  What else is everyone using out there besides Express, Gear, or Bytescribe?? Any help would be appreciated!

new Cox user need help sm
I will be having cox installed next week, I understand I can get a wireless router for one computer but they said I would have to purchase a wireless "card" for my others. I have an Acer and a Gateway, plus a Dell laptop aside from the one I am having Cox installed on. How can I get a wireless card for each of these computers and are they hard to install? They are "older" but still work fine for the kids, etc. Thanks for any help.

Is there anyone who has had phone cards work for them?  The ones I have seen are at least 4 cents a minute.  But, has anyone found that phone cards is the answer to ULD?  I am new to this and scrambling to find something to replace MJ.

VR is only as good as the user,

and it won't pick up background noise if you have a noise-cancelling microphone (like I do).

Here's a report I dictated yesterday. I think it had one error (it typed breed instead of bleed), but that was because it was the end of the day and I was getting tired. Judge VR for yourself (and see my note after the report)-


The patient is a 57-year-old male with a history of hypertension, who was found on the floor by his son.  The son heard the patient fall and then heard banging on the floor.  At the time of initial evaluation, the patient was verbalizing although the patient could not understand what he was trying to say.  The patient had a left hemiparesis.  The paramedics were called but by the time they arrived the patient was unresponsive, however, he was reported to have a gag reflex and he was breathing spontaneously.  The patient was transported to our facility for further evaluation and management.

On arrival in the emergency department the patient was found to be entirely unresponsive, therefore, he underwent tracheal intubation for airway protection and to provide ventilatory support.  He was found to be hypertensive at the time of his evaluation in the emergency department.  The patient subsequently underwent a CT scan of his head.  This revealed a hemorrhage into the basal ganglia and ventricles.  There was mild ventriculomegaly and the basal cisterns were tight.  Additional evaluation in the emergency department demonstrated that the patient had left ventricular hypertrophy on electrocardiogram but without evidence of acute myocardial ischemia.  He was noted to have severe renal failure with a BUN of 95 and a creatinine of 9.8.  The patient's blood sugar was 229. The patient had placement of an EVD and was transferred to the intensive care unit for further management.

Since admission to the intensive care unit the patient has remained hypertensive.  His systolic blood pressure has been as high as 190.  His current blood pressure is 140/80.  The patient is receiving a propofol infusion.  He has not required any additional hypertensive therapy at the present time.  The propofol was used because the patient did become agitated, although he was not awake enough to adequately protect his airway.

Since admission the patient's intracranial pressure has varied between 8 and 16.  He has required CSF drainage of about 10 mL per hour.  His cerebral perfusion pressures have remained satisfactory.  He has been in sinus rhythm with a heart rate of 80.  He has not had any ectopy despite the acute intraventricular bleed.

The patient remains intubated and mechanically ventilated.  With slow ventilatory support, his gas exchange has been satisfactory.  We are providing assist-control ventilation to optimize his gas exchange, but we will attempt to keep his pCO2 between 30 and 35.  Lung fields are clear and there is no evidence that he had aspiration at the time of his bleed.

The patient was also noted to be hyperglycemic.  He is receiving an insulin infusion to optimize his glucose control, and the insulin infusion will be adjusted as needed.

Because of the patient's severe uremia he will require dialysis.  Currently, his potassium is not elevated and his intravascular level appears satisfactory.

The nephrologists have spoken to the patient's personal physician.  By history, the patient has had progressive renal failure although he has not require dialysis.  He is also known to have hypercholesterolemia and cardiomyopathy.  He has had deteriorating mental status over the past few weeks, which may reflect his progressive uremia.  He will require dialysis in addition to continued ventilatory support and management of his intracranial pressure.


P.S.  For the record, I dictated this report, if you will excuse the cliche, like a bat out of hell. All I know is I completed 36:23 of dictation in about 1 hour 15 min, and it had 626 lines.

I've taken phentermine off and on for the last 15 years after quiting smoking and gaining 25 pounds. The only problem is,  once you stop taking it the weight comes back on. I have had no health problems from this medication, SO FAR.
Is there a way to search by user name? (sm)
Is there a way to search MTStars by user name?  For instance, if I want to search my past posts?  I tried using the search at the top but it didn't work. 
I have WP 10 and I like it. User friendly. sm

I've used MS Word too, but there really isn't that much difference and its easy to learn.
