Transcription Contract forms or ideas.....

Can anyone refer me to a site that has contract forms for transcriptionists or what do others of you have?  I have one sample, but there are quite a bit of things on it that don't really pertain to the services I am giving.  How do you come up with your own and what should be put in a contract?  Thanks for all of your help and suggestions.

Transcription from Forms
I do the exact same thing transcribing from forms.  You cannot read into these too much.  They are probably already using the forms they filled out as their report, but having you transcribe to make them "look pretty".  No one is perfect.  If the money is right, I would overlook the inadequacies, and of course you should fix dramatic errors, as you would if you listened.  I am not trying to be unkind, but it works both ways.  Sometimes as transcriptions, we inadvertantly type an incorrect spelling, and the dictators don't give up on us, or do they??????  Something to ponder.  Good luck to you!!!  I hope you can find a happy medium in this matter and not lose what sounds like an "easy" account.  However, the reports I transcribe are not from a nurse, they are from PA-C or the physician himself.  I do, however, have a question for you, if you don't mind.  What do you charge, per line, per page, etc?  I charge per page and make out pretty darn good since everything is so canned text!!!!    Take care... 
Anyone willing to share their contract ideas?

I was wondering if there was anyone out there was was willing to share their contract verbiage for YOUR contract that your MTSO signed. There was a wonderful lady who emailed me with some great ideas, but understandably she was not willing to share her contract verbiage because she had paid a lawyer to draft it.  I totally understand that!  I was just wondering if there was anyone who had drafted their own who would share with me (identifying info could be removed, of course)


I would have money to pay a lawyer if my rackin' frackin' MTSO would pay me, but I'm still without the paycheck I was due on Monday!  Anyone who has done this themselves and who would be willing to share, PLEEEEEEASE email me with it and I will be beside myself with thankfulness.

Does any one know who has the Cleveland Clinic Transcription Contract? sm

I saw where they did a face transplant today.  I would love to transcribe that operative report!   Anyone know who has their transcription contract?  I want to work for them - I think, depending on who it is.

Idea for a new transcription service. We are looking for some ideas to firm things up. TIA. sm

Here is the basic structure:


All work kept in the United States.  Recent graduates of either a 2-year community college program or one of the better distance learning courses. 


We are thinking of opening a combination office/training center.  Half will be recent grads.  Half will be seasoned MTs who like to mentor.


The seasoned MTs will get an hourly with a line incentive as there will be time taken away to be second ear for the newbies, more in the beginning, less as time goes on. 


The newbies will have to be in-house at the company for one year on either 1st, 2nd or 3rd shift.


The newbies will receive either by the line or by the hour, have not decided.


What do you think would be a fair hourly for the new grad?  For the seasoned MT who will be transcribing their own work and mentoring. 


We want to be generous without going bankrupt.


Do you all think the idea is a good one? 


If you could work in house at a company with good benefits would you prefer that to working at home?


Just looking for some input.  It looks good to us, but outside opinions really can be eye-openers.


This is an established company, so there would be plenty of work.  All of our MTs are home-based now, but not sure if everyone really wants to be home-based.  We are near a major metropolitan area, so finding the MTs should not be a problem. 


Any suggestions would be appreciated!

Long short forms vs short short forms

A long short form is usually easier to remember. So I would definitely assign the short forms as suggested by Kelli:
SmithCThead
SmithCTabdomen
etc


In most Expander programs you would opt for a short short form as you have to type the whole short form before you can expand.
In Instant Text you don't have to type the whole short form: you type the first two letters of your long short form and then you can type any of the following letters.
So in this case you would type smcth for the first one, smcta, and you may also type smhead, smabd


This is possible when you use the Instant Text marker key to expand. If you use the space bar to expand, you may be better off assigning the initials as suggested in the second response, as you would not be able to "jump ahead".

Tax forms
This is the first year I have worked as an independent contractor. Do I use Schedule C, profit or loss from business form, Form 8829 and Schedule SE (self-employment tax form) to file? Form 8829 goes into detail about depreciating house, mortgage, etc. and in the past I eliminated that because if you take this off when you sell you have to factor this depreciation amount back in. Of course, in the past I worked as an employee and filled out the other form, employee business expenses.  I hate to pay for a CPA for no more than I worked but maybe I should?
Tax forms

This is not about that form per se, but you might want to consider filing a form 4868 "Application for automatic extension of time to file"  This can be sent any time before April 15 and gives you some more months to get your return ready.  If you owe money, it still has to go in, but at least you won't be one of those folks you see on TV lining up at the Post Office at midnight.

1099 tax forms

Does anyone out there who is an IC know of the deadline date for the 1099 forms to be sent out via "employer"?  As a special input, I did some research on what deciphers an employer-employee versus employer-independent contractor.  In summay there were 20 points made and I fit all criteria of being an "employee" instead of classified as "independent contractor" per the IRS.  I am considering filing the "SS-8" form with the IRS, which is a form for the IRS to give their input on the employer-independent contractor issue to determine how I really should be classified.  The way I see it, my employer refers to me as an IC because it simply benefits her, and on the otherhand, it does nothing for me except harm.  I am making 9 cents a line, cut back from 10 cents a line a few couple months ago, and have am offered no benefits whatsoever.  This happens to be one main point that the IRS displays as I would really be classifed an employee as my employer has control of my pay.  Just thought this was an interesting fact to point out to all as it is something I was not aware of until today upon doing self investigation into it.

abuse comes in many forms

 


This truly is abuse.  You may not have visable body scars but mental abuse is just as bad if not worse as physical abuse.  I'm not sure there is help for your husband but you need to get out of this relationship for your own sanity.  A tiger never changes his stripes.   I had been in relationship that had the same elements.  I worked all day and would teach a class at night and we were to dress in a professional manner and I would be accused of having an affair.  (Huh, like you have the time and energy.)  I had the courage to get out and have now married a man who respects me and treats me as an equal partner.  Is that not what a true marriage is, a partnership.   If you do not have family to help check with you local Women's Help Center possible that could be a start.   I wish you the best and hope you have the strength to more forward and leave him in the dust.


 


Contract/no contract -- same rights
You do not have to have a written contract to take them to court.  All you have to do is to prove you have been providing  work to him, past statements/invoice will do that,  you just fill out the small claims forms and go to court.   But first of all I would personally go in and talk with someone about it.   Could be the office manager/bookkeeper and not the doctor that is not paying you.   He may not know anything about it.   And please do not  make the "joke" as above mentioned, that is very LOW class, tacky and not professional at all.  Especially if you are in a small area -- you need to conduct like a professional.  But call and make an "appointment" to talk with the doctor either in person or through a phone call.  Some doctors do not know what is going on in their office and perhaps the office manager wants to be paid first.  If the doctor does know, then in person or over the telephone inform him that you have no other choice but to pursue the debt just as they would if someone did not pay their bill and if that involves going to small claims court, then that is the route you must take.   Good luck.  I did take one to court without a contract and won.

You can edit your short forms in the Glossary Viewer and have them come up according to the short forms you have chosen:

Does anyone have a "stock" proposal form they use to bid on transcription work? Thanks.
There are various forms of back supports
you can purchase for your chair, or check with your physician for chair types.
I never received 1099 forms when I
was an IC. I worked for a hospital for almost 10 years plus had other docs and an attorney. The attorney did give me one every year and it never conformed to my figures but I never had a problem.
EHR can encompass many forms of dictation....
not just point and click. I know several people who have lost their jobs to "EHR," and it had nothing to do w/point and click. It WILL eventually affect us all; however, the timeframe will be on an individual institutional basis as the cost to convert to the system is quite extraordinary.

If you haven't signed non-compete forms...
with a company you are employed by now, and you leave the company, is it acceptable (legal) to contact a client of theirs and offer your IC services if you really want that account as your own?  Any suggestions from anyone having ever done this before would be much appreciated.
I used a Selectric for my first job and on the subject of whiteout, we used forms that were three
different colors, so each time you made an error, you had to white out on your carbons with all of the matching colors and this doc hated errors and would point out that you had to use the whiteout.  Glad those days are over!
You can change tax forms to have extra taxes

come out of your husband's check.  You can figure out how much tax based on $900/mo.  If you have too little taken out you'll have a penalty (around $50) and then you can adjust the next year if you need to.  I had taxes taken out of my husband's check, but didn't figure enough and I got hit with the penalty and I just adjusted the next year.  I am an employee now so don't have to worry about it this year and have adjusted tax withholdings so we have more $$ in our pocket.


You can find a used C-phone for $125 if you are lucky, but more around $200-$300 range.  There is a classified board here, you can also check E-bay.  If you need a food pedal only and not the software you can find a used one for $25 if you're luck to about $50.  You can new for about $80.  


A lot of times a company will send you the foot pedal if required, but not always.  Sometimes they will rent you a C-phone, but not always.  I wouldn't buy anything until you have a job.


As far as working to only gross $900/mo you can guestimate how many lines you can do based on xx line rate and monitor it so you don't go over.  I would guestimate higher to give yourself a little room.


Just be upfront with companies that you can only do so much so they know they can't expect you to work extra if things are backlogged. 


 


How do you fill out forms in Adobe Reader 8.
I am referring an MT to a company I work for and to get the referral bonus I have to fill out this form in Adobe Reader and E-mail it to HR.  Well, I don't know how to do that.  One friend said to copy it in Word and fill it out that way.  It won't let me do that.

there is delay due to delayed minimum tax vote on certain forms
The five forms affected by the delay are:

Form 8863, Education Credits.
Form 5695, Residential Energy Credits.
Form 1040A's Schedule 2, Child and Dependent Care Expenses for Form 1040A Filers.
Form 8396, Mortgage Interest Credit and
Form 8859, District of Columbia First-Time Homebuyer Credit.

I avoid numbers in my short forms and use letters
instead when needed. What I do most often is double-key a short form (same two letters) that goes back and adds slashes or dashes for dates and decimals. It's easier for me to key the numbers, then let a command entry edit when needed. You might want to go to Productivity Talk for a lot of great tips on learning IT.

Doesn't it give you the option of printing all the forms and schedules?

I have to disagree. Most hospitals have migrated to EMR & RNs are able to fill in online forms
My husband is an RN at an acute care facility and he loves his job.  He has dual majors.  One in computer science and the other in Nursing.  He decided he didn't want to sit behind a desk plinking on a computer all day and really wanted to do something with his life that would help people.  Being an RN is not just about the money, it is about caring for those in need.  Seriously, think about the reasons you would be entering the nursing field.  If it's just about money, you'll be disappointed.  However, if you are the type of person who likes caring for others, then by all means, go for it.  All the best!
Instant Text creates entries and the short forms from
previously saved documents. You run a compile on the reports and it picks up the most frequent words and phrases. Go to their site and click on the overview link to see how it works.
Bidding on new account - states that they will provide specialized forms such as letterhead, *sm*
progress notes, operative forms or other such printed documents.  The accounts that I have previously worked on did not have "printed" forms for progress notes or ops.  What are they talking about here?  If there is such a thing as a preprinted form, then how do you do the account long distance - I am not near them?  Could the forms be scanned or made up in a template which could save them money by not having to buy them?  Years ago I worked as a legal secretary and we had forms that we made up and filled in on our computer rather than with a typewriter.

Know AIM transcription system, never heard of Hull transcription. Are you sure
you are not thinking about DHull who is a recruiter for MDI-FL?  They use the AIM system.

I'm beginning my carreer as a medical Transcriptionist - which is best Expander program, are there any good ones lower priced??  anyone know if there's a free trial with any of these programs?
Some Ideas - sm
First, keep a list of each job # you do...this is useful in case there is a dispute or lost work. Also, if you later need to request a copy, you can ask your QA dept by job #.

Second, your question: I do about 20 reports a day for around 750 lines. It takes me anywhere from 4 to 10 hours. :( New accounts are really slowing me down lately.

Try these ideas.

I picked up an exercise bike at a yard sale for $1 and a mini trampoline for $5.  Whenever I need a break to recharge, I get on the bike or trampoline for 5-10 minutes.  They say that little spurts of exercise throughout the day are as effective as exercising all at once.  Plus, it'll help clear your lungs out.  Another thing to try is to just step outside for a moment and take some really deep breaths of fresh air in.  You used to inhale deeply with smoking, so now you inhale deeply without the carcinogens.  You can also try yoga or tai chi.  Very calming on the nerves.  I also switched to herbal tea, so I'll take a break and fill up my little teapot for one.  Replace your bad habit with a good habit.  Just don't replace smoking with eating.  If you need the hand to mouth thing, try air popped popcorn, cut up fruit or vegetables, a small amount of nuts or seeds, or hot peppers.  I keep a jar of Bit o' Honey and fireball candies on my desk, so I'll have one of those a day.


It's important to reward yourself, so take the money you would ordinarily have spent on cigarettes and get something for yourself.  I like to buy books, bubble bath, stinky candles, mocha lattes, restaurant gift certificates.  You can also put the money you're saving into a big jar to watch it add up for a BIG reward.  I saved enough to buy a dishwasher, which is kind of silly for a reward, but I really wanted one.

several ideas on where to go

Number one was psychology, but that will take too long, going part time, and knowing in the next five years MT'ing might not exist.  You need a bachelor's to get foot in the door.  I wish they had schooling completely on line for this degree, but they do not. 


Second is something with cooking.  Would like to work with people on diet and weight loss, since it is the way i've lived my own life for years and I believe strongly in it.  The chef/cook field is predominantly male, so it will be hard to get a foot in the door there.  I can be a certified dietician all on line within a year, but the pay isn't all that good.


Ultimate goal would be a personal chef or working for myself doing SOMETHING.  I think thats everyones dream.

are right up my alley of interest.  I have a community college in my area that offers a certificate in cooking.  I've sent away for the packet, and I feel this is the direction I want to take, since I actually enjoy cooking and working with food.  I'm just in a premid life crisis.  Thanks for your advice, i'm glad I posted today.

For the business expense part I would buy a printer that uses a different ink cartridge than what you have now. The best one you can afford. I would tell them that my home printer won't handle that much printing (they might even be able to get you a good deal on one from their supplier). In the meantime I would go to Kinkos or printer and have them print the reports and SHRINK WRAP or package the bill with the reports while you are waiting on your new printer. javascript:editor_insertHTML('text','');

Two ideas

My daughter LOVES trains, and you can find scenic train trips all over.  We did one in Romney, WV that was just wonderful.  The train was called Potomoc Eagle, and we did see bald eagles on our trip. 


Not sure where you are in WV, but what about Hershey, PA?  Probably not the cheapest vacation, but lots to do.  Besides HersheyPark itself (very kid friendly) there is a zoo, a garden and more.  I'm sure there are packages available.  


I'm anxious to see what others come up with too.  We are in PA and looking for some close trips as well.  Which beach do you go to?  We found a beautiful not-so-commercial beach at Lewes, DE last year and can't wait to go back!

I really appreciate your taking the time to answer my post and give your input. I already know that I will have signed on for real at a new job before I ever walk out the door at my present one. And although I want SO badly to tell them to "take their job and shove it", I guess I have to not burn my bridges, either, just in case. I had thought about doing some part time work, but I'm usually so burned out at the end of the week that I don't know if I could do that or not. I run around like crazy on Saturdays, just trying to do all the household chores, shopping, etc. that doesn't get done during the week.

I seriously considered training for another career about 10 years ago, when we were told our jobs would "be taken over by VR" eventually. But I got into MT because I don't fit well into most other professions. I've had a couple of my own businesses. Too much worry and having to be available 24-7. I've gotten lazy in that respect. The same with going back to school. I'm just hoping I can keep on scratching out a living with what I know how to do, until I have enough saved to at least retire to part-time work to supplement my 401K & whatever Social Security is still left in 2025. So, I'll keep my fingers crossed!

Some ideas
All teachers don't come out of the same mold, but my friend who is a teacher says the best gift she can receive is a gift card from a grocery store. She likes all gift cards, but it's easier to use the grocery store ones. We all have to eat!! An Amazon.com gift card is also good.

Her second favorite is homemade cookies or other edible products.

Trinkets and personal gifts are great, but she says she remembers students by their personal qualities (or lack thereof lol) more than any personal gift.

We have decided to make a picture collage of the puppies that survived and "mom and dad", and include it with a basket of goodies that can be eaten while on the run with an assortment of coffees, teas, individual bottles of flavored water, etc.

Although we wanted something unique, the goodie basket seemed to be at the top of everyone's list... so if you can't beat 'em, join 'em.

Some ideas...
I'm in the same boat, but decided to get over it as July has come and is on its way out and the work is still very skimpy. Fortunately, my rent isn't due until the middle of the month and I'll figure something out about the rest of the bills even if it means getting reaaal good at Ebay.

Some suggestions:

--Check the local temp agencies, perhaps they have something, med. transcription or not, that might be available to help tide you over.

--Consider hiring on with another "back up". That's not being disloyal. It's being smart and sane. What good are you getting frustrated, upset, being tense about where the rent's coming from if not your current employer? What good are you to your family who is depending on you or your employer if you're all out on the street?

--Whether you tell your employer what you're doing (to literally keep yourself afloat) or not is up to you and depends very much on your relationship. Think this particular over very, very carefullly regardless of how much better it might make you feel regarding loyalty.

Hang on, hang in. We've both no other choice. We'll get through this.


Need some ideas
A friend of mine has throat cancer and is undergoing radiation.  His throat hurts and he has no teeth and I am trying to come up with some high calorie food for him to eat, he says he can't taste much and has been doing milk shakes and mashed patotoes, any other ideas.  Really need to be high calorie because he does not want to have a PEG and if he can keep up his weight and still swallow then he won't have to have one.  Thanks for any suggestions
Some Ideas for You RE: LX-219

 


First, sorry about the funny way my answer is formatted below, I'm no good in this little box.


Are you dialing into a Lanier system?  LX-219 and other Lanier equipment only works with Lanier. 


 Ranzy Luster in Texas fixed my LX-219 with large display for a flat rate of $100, gave a 90-day warrantee with the repair.  He sometimes advertises on the MT Daily site.  He has been around a long time.  His phone number is 254-235-3241 and his e-mail is rluster@transcribe solutions.com.  I felt he treated me very well and he was fast.  I should say if the large display is not working, I don't believe he works on that, but if you can see anything in the large display, I believe he will work on it.  Ask him.


Look in your User Guide for the LX-219 and try performing the call-echo-cancel sequence.  If you are calling into a Lanier system, then it is not recognizing your machine.  Lanier machines send a proprietary tone in to connect with the host modem. 


Sometimes the phone is set for dictate instead of transcribe.  You can try this: Press  *99 (that's star-99) then the 1 key


If set for transcription it will say 20 through 39. 


My old Lanier service tech said he always used 20 as his default when setting machines for transcription mode. 


When you dial into Lanier, the LX-219 must be programmed with YOUR ID number.  So, program one key with the number you are dialing, then another key with your ID. 


You can test how this works by dialing the phone manually, touching all the buttons just like the regular phone, then input your ID number, the system should recognize you. 


I haven't used my LX-219 machines in a long time, I use a Lanier TXC card to download large amounts of dictation to a sound card in my computer, but the above were some hints I wrote for myself in the handbook for the LX-219 a couple of years ago. 


Is your C-phone located near any other equipment?  That can cause static.  Move it away by itself. 


I hope something here helps.  Give Ranzy a try.  He might have some suggestions for you.

Grilled cajun chicken or shrimp over Caesar salad. Marinated steak over salad. Marinated chicken or shrimp kabobs. Italian sausage kabobs with peppers and onions and cherry tomatoes. A crustless quiche. Shrimp scampi. Tasty veggies..brocolli with cheese sauce, cauliflower with cream sauce, glazed carrots. Cole Slaw. Stacked thin burger patties filled with layers of swiss cheese, mushrooms, onions, or whatever you like. I find if it's tasty or fun to eat you don't miss the carbs as much ;)
