that are actually about 5 minutes, once you get past the ummms, and ahhhs, and talking to the nurse behind you, and listening to other people's conversation, and changing your mind 5 times before the completion of a sentence.  Other then that, I can't think of a thing....ahhh, ummm, go back and change....no....under ROS put....

Agree with you. Thirty minutes is long enough.
I will do testing if it has 1 or 2 voice files, but when they send me 4-6+ and then tell me testing take 90 minutes, forgot that. Who has time. Time is money!!!
Paid by the minute... new to me... does $1.14/minute sound reasonable?

Missing dictations
I also get a patient list from the doctors office. I check off which ones I got voice files for, i.e. which ones I typed. I don't keep track of which names or not checked off. How do I know if that patient just didn't show up for the appointment, walked out without being seen, or if the doctor put off dictating. That's administrative work that the doctors' secretaries do at my client's office. If they can't find something they think he dictated and ask me to look for something or check on something I'll do it because it only takes a few minutes but I don't do all the stuff you're doing.

Practice dictations
I am at a hiatus in my transcription career, but I do not want to lose what I know, or lose my speed.  Is there somewhere you can purchase practice dictation tapes, or digital downloads?  I would prefer digital downloads of voice files if there is such a thing.  Just wondering.
missed dictations anyone?
Does anyone have any real experiences to share with the missed dictations? I would like to buy them but I don't want to if my accuracy might decrease with them due to missed dication. although, I think any of my background noise would be louder than any dictator. But I guess on the other end is where the problem lies.
STAT dictations

Re:  Stat dictations.  Does anyone out there charge extra for STAT dictations that come in during the day so that we have to put aside what we are working on and type a STAT letter/document and send in?  This sort of thing can keep one behind in Turn Around Time if it happens on a regular basis.  If anyone charges extra for STAT dictations coming in, what would be a good fee to charge for these besides the line count?

Downloading dictations (sm)

If you key in alt-ctrl-Z while the first one is loading, it will prevent the other two from downloading from the queue.  When you are ready to submit and do another file, you will need to key in that sequence again to allow the next one to come in.

When I got a lot of crappy dictations in my queue, I complained about this and asked WHO assigns the work, who gets the crap and who gets the good ones.
I was told that it is NOT assigned at ALL, the oldest is picked first out of the pool and so on, and that it is all random and nobody is favored.
Now I read in this post that the Leads (team leads ?) distribute the reports. Who are they? The owners, the supervisors or the QAs?
I had once an argument with a QA about grammar and I was right and she was wrong, and from that moment on I got the crappiest crap of the crap into my queue.

Where to find some Sample Dictations?

Anyone know where I can find a few sample dictations, like WAV files, on the internet, along with the completed typed reports.  Need to test job applicants.  Happy Holidays!

You left out blank dictations
The ones with no dictations - I have a feeling doctors probably pay the MTSO by dictated minutes - but how about MTSO paying the Transcriptionist who has to waste their time listening to them, signing them off and sending them to QA multiple times a day?

Right on, JMO. Also, when I have the ability to choose dictations SM
from a list (which I don't have at my current work, darn), I like to gather up all the dictations I can find for bad dictators and do them together to learn that dictator right up front, instead of struggling over months. I'll also use them to put together a template, which if dictation's really horrible will include a whole pack of phrases he/she has been known to use, a grab bag of possibilities to scan over. May sound like a bother to some, but it's a real serenity-saver.

In dictations, I always hear "as well as" when "and" would do. sm
"At this point in time." What does that mean? One says "At this particular point in time." Raises the line count but it is annoying.
Saving dictations on flash drive??
Hi again,  I figured out the settings, and how to recover the files marked done.  Question though on saving the actual dictation.  My files come through email, I was saving in My Wav files and an Archive File; however, the doc tends to send me a lot of duplicates which the Archive obviously does not like. But, also, since I have had a computer crash on me and lost some things, I am thinking of saving my actual dictations on a flash drive and eliminating the Archive file. How long would you suggest I keep them for?  Once typed they go in the Recover folder, how long should I keep those? I found in the help information that we can order foot pedals through Express Scribe and they will be sure that they work with their program, so I ordered one today - figure can't work without a pedal so best have a spare on hand.  Anything I might be missing? Tips or ideas in using the program.

Are hospital dictations difficult to type? I have always typed various specialities for clinics. The company I just started working for lost the account I was hired for. They did offer me a position to type for the hospital dictations. Do you think it will be a rough transition for me? Any advice please! Thanks.

Legal dictations usually are multiple speakers
But, legal dictations usually have a longer TAT. 
I think doctors in general just don't care about their dictations... sm

I know there are some who are really great and are careful or even just considerate.  But I think for the most part they just think it's a pain to do and want to rush through it and get it done... let the lowly MT deal with whatever garbage comes out the other end.  Maybe I'm a bit bitter, but at this point that's how it feels.

I learned to put my phones in front of my ears inbetween dictations so I don't get blasted.

I do approximately 15-20 minutes of dictation per hour and/or 150-200 lines per hour.  So figure from there.

I was fast already, so faster, not so sure that I could be much faster without compromising my own comfort level with quality, and I don't want to listen to my docs at a distorted speed rate.

I also have an Advant Prime clicky-kinda keyboard and am just as fast on it but prefer this Atech4 as it is quiet and has programmable keys.

Don't assume that just become someone expresses an opinion other than the original poster, they're management or American administrator of overseas transcription companies. Sometimes American workers need to think for themselves and other American workers will ask questions designed to make them think.


Otherwise, the rest of your post was well-thought out, although I would say that my impression from reading most of the nonsense about a strike, etc., was the intent was not to make the public or politicians aware of the situation with HIPAA violations but rather to make life difficult for employers, even to the point of risking patient care--neither of which should ever be consider humorous if you're trying to make a point in a responsible and professional manner.


GBP per minute

I missed this somewhere along the line.  I saw an add on another site for a job that paid xxx per GBP.  What does GBP mean?

I'll bet the speediest speed demon dictators are those who are charged by the minute and pay their own bill!  If only they were charged by the line and charged extra for fast talking, I'll bet they would slow down rather quickly!
cpl pay vs per minute
What is the opinion here on pay by line versus pay by the minute?  Pros vs cons and why?  I can figure out for myself that, for example if you are paid 10 cents a line for a 65 cpl and they say nothing but one sentence for 10 minutes and you listen to shuffling of papers you would be much better off by the minute.  I am just curious here what the opinion is and why which one is better. After all these years of being an MT I know companies are always trying to figure out how to make money for them without passing on the rewards to the MT.

I get paid by the minute but I do Basic four and radiology.  Fast/Slow dictators usually balance each other.  Now, if I did just radiology, I would get paid more by the line or report because of the very fast dictation.
They pay per minute dictated. Sux.
Especially when they assign the work. It's not fun when all your work is less than 1 minute, two line reports, ESL docs, etc.
It commonly known that 1.00 a minute is very low...sm
and that is only 60.00 for the hour voice audio... so 20.00 is horrible!!  These guys think we are stupid!!

ten 25-minute reports, or 250 1-minute reports, or 125 2-minute reports, etc., etc., etc. (as the king would say).
Just need to vent a minute...

Why, oh why is it that when a doctor is dictating a list of dianoses, etc. can't they say #1 when they start the list.  Usually I don't let this bother me, but it has happened with almost every report I have transcribed today and it's bugging me!!


If you're gonna dictate a list, start out saying #1.  Don't say the first one and then say #2.  Drives me nuts!


On the reverse I have also had some today that will say "ADMIT DIAGNOSES:  #1:  XYZ".  And then move on without a #2.  If there is only one thing, it doesn't need a number!!


Sorry, this has just been a long day with multiple reports like this and it was getting on my nerves.  I just needed to vent a little.  I feel better now and it's quitting time!

That's why I think it's better to pay by the word or characters, really--pay for what you actually type. I have clients that do the same thing clhmt said--I have one who can do 30 reports in 10 minutes. Other who can't do 1 report in 30 minutes!
Pay by minute of dictation

I've been offered a position where the line count is calculated by multiplying the minutes of dictation transcribed by 12.5 (the assumed average number of lines in a minute of dictation). Has anyone had experience being paid in this manner, and how do you feel it compares to the 65-character line?

You are getting some good advice on here. As for the company, please, don't worry about them, quitting will not hurt their feelings! Believe me, they wouldn't worry about you, not really. What if something horrible happened and you HAD to quit? They would understand. Hey, instead something wonderful happened. You are very nice to think about the company, and that says you are so nice, you really deserve what you want in your heart. This is your gift. No guilt please! No I don't-- going on the preface that 1 minute- sm
is 10 lines, it would take over 8 hours to do 5000 lines; just listening, that is not stopping to look something up, to retype, fix, etc. So unless you speed the tape up a lot (and can still understand it) I see no way of doing 5000 lines under 10-12 hours. I could see 3000 lines but not 5000.

at 9 cents per minute -- buy on E-bay
I have 4 docs that do an average of 2  to 2.5 hours per week each on tapes sometimes more, sometimes less but lets average 8 hours per week.  That is $43.20 per week I pay you.  My psychologist does 3 to 4 hours in addition to this.   So there is another $20+.   I just bought hand held Olympus recorder on E-Bay for $31.50 and even full price at Amazon was under $100.  So one month of paying you would pay for my 4 hand helds.  I have thought about this but just cannot afford the $150 to $200 it would cost me per month.   And yes, I have had very good luck with buying on E-bay for transcribers, recorders, etc.  So look on there or do a Google for digital recorders by brand, Olympus, etc. and it shows you which store has them for which amount.  The bells and whistles are nice but a basic recorder that holds 2 to 4 hours of dictation is well below $100 and that is all you need to provide them with.    Good luck.   Just setting up an FTP and so don't have much info on that yet but searching it out.    Patti
