The only way I did it,

with the medical secretarial associate degree classes I took back in the 1980s, was to work for a company that had a mentor to help me. I worked at an office with her at the next desk for months. At first she looked at and listened at every report besides answering questions and listening to tough spots in the dictation.
Without having gone to a great school like Andrews or M-Tec, I don't know how anybody can make a living unless they have a mentor like I did. That's why I promote Andrews and M-Tec, to help people avoid going through what you are. Acute care is tough with all the different specialties and terms you only run into doing hospital work. Talk to your supervisor about it. It might help you to just do consults and discharge summaries and not all the other work types. One good thing about that is you can often access the patient's other reports, such as the H&P, and get clues about the patient's drugs reason for admission.
I also recommend buying Stedman's specialty books if you haven't built up your library yet.
I'm not saying clinic work is easy, but it certainly is a whole other animal than acute care.
Good luck.
