I, too, had worked in-house for most of my career (I started transcribing at home about 8 months ago) and at one hospital where I worked they hired a Transcriptionist who had primarily done at-home transcription. Perhaps it was just her personality, but even though the reading rooms were only a few steps away from the transcription room, she would not go in there and ask the radiologists when she had a question about a report. The rest of us had a very good relationship with the radiologists and when we had questions they would take the time to explain the answer and even show us the films so we could see what they were talking about.
Experience like that is invaluable when you get out on your own. It isn't enough to know how to spell the medical words. You need to know which words fit in which context. What we do isn't typing. It's transcribing. And there is a world of difference.
