Short Stay Summary
For those of you who work in acute care, work type does your facility use for short stay summary?   HP or DS?
We have a separate work type for Short Stay
Usually the summary looks almost like a DS, but sometimes they start out like an H&P, where they appear to be dictating a combination H&P and DS.
At least 1 short break every hour and stay away from IM
s
I used Short Keys, not Short hand for Bayscribe, it worked fine.

Death summary on someone who...
had died for the same disease as my dad, lymphoma. The summary went through the last month of her life and the hospital stay. At that time, my dad was not in the hospital, but was showing these same signs. So, I knew what was forthcoming for him. And, needless to say, it happened the same way with very few changes. Talk about a tear jerker and a tough one to type.
Discharge summary TAT?
What is the minimum and maximum TAT for discharge summaries have you had?

Under the summary points

This statement was my favorite, and I do hope a lot of companies read this survey and learn from it!

"Speech-recognition technology is only a transcription tool, and not a transcription replacement. The job involves too much professional intelligence and sense-making ability to have it migrate completely to speech recognition."

I honestly don't think a company should be able to lower our rate of pay because of VR...We are still responsible for sitting and listening and correcting mistakes...although I have been fortunate enough not to have my company have any accounts that use VR yet...but I have a friend and her company is doing VR, and her rate of pay is ridiculous!

House Passes Budget Bill with Biggest Cuts in Medicaid, Medicare

Cuts $99.3 billion over 10 years - 27% from Medicaid, 23% from Medicare.

Feb. 1, 2006 – It's done. The House has passed and sent to President Bush the budget reconciliation bill that was strongly opposed by most senior citizen advocacy groups and newspaper editorials due to the deep cuts it makes in Medicaid and Medicare. It was a very close vote – 216 to 214. The bill cuts the budget by $38.8 billion over five years – 50 percent of the cuts are in Medicaid and Medicare.

Final House Vote on Budget Bill Could Cut Billions from Senior Programs

Passage expected Wednesday to cut Medicare, Medicaid

Jan. 30, 2006 – The Budget Reconciliation Bill, which many see as making drastic cuts in government programs for senior citizens – Medicaid and Medicare, is expected to hit the House floor on Wednesday for a final vote, the day after the President's State of the Union address. As reported today by the Capitol Hill Watch at KaiserNet.org, the bill will reduce federal spending by $99.3 billion over 10 years, with half of that coming from these senior programs. The Congressional Budget Office analysis says premiums for Medicaid beneficiaries in the bill could cause 110,000 to lose coverage by 2015. Read more...

Senate Budget Chair Calls for More Cuts in Medicare

Republicans to recycle old failed issues in 2006

Jan. 26, 2006 – The Republican chairman of the Senate Budget Committee is calling for more cuts in health care programs in fiscal 2007, which includes Medicare, according to The Daily Health Policy Report by KaiserNetwork.org, which cites CQ Today. The daily news summary says Republicans will also be targeting health care proposals this year, but most are proposals that have failed in the past. Read more...

The House first passed the bill on Dec. 19, 2005, by a vote of 212-206. It went to the Senate, where it passed by only one vote – 51-50. Due to procedural changes in the Senate, it had to go back to the House for this final vote. There was intensive lobbying by the senior citizen advocates and others since the Senate vote on Dec. 21, 2005, but they came only four votes closer.

The final vote was mostly along party lines, with Republicans supporting the bill and Democrats in opposition.

AARP CEO, Bill Novelli, quickly issued the following statement:

"Last night, in his State of the Union Address, the President said, "Keeping America competitive requires affordable health care. Our government has a responsibility to help provide health care for the poor and the elderly, and we are meeting that responsibility."

"Today the U.S. House of Representatives has turned a cold shoulder to that responsibility by further limiting eligibility for Medicaid, a program that serves the neediest -- the disabled, children, the poor and the elderly. It also approved a provision in its budget that will deny long-term care coverage to those who give money to charities, churches and family members in need.

"Working with our members, AARP will continue the fight to have this ill-conceived policy reversed."

The bill would save $99.3 billion over 10 years, with half of that coming from Medicaid and Medicare - 27% from Medicaid and 23% from Medicare.

There's also $1 billion in new spending to extend an income subsidy program for dairy farmers and a reprieve for physicians who had faced a 4 percent cut in Medicare fees, according to the Associated Press.

A major boost to opponents was an estimate issued by the Congressional Budget Office estimating that 45,000 Medicaid beneficiaries would lose their coverage in FY 2010 because of higher premiums in the bill. The CBO report also estimated 65,000 beneficiaries would lose coverage in FY 2015.

Children would account for 60% of the Medicaid beneficiaries who would lose coverage, according to the report.

The report also estimates that 13 million Medicaid beneficiaries would have new or higher copayments for services such as physician visits and hospital care. In addition, 13 million Medicaid beneficiaries would pay more for prescription drugs by 2010, and 20 million would pay more by 2015, the report states.

According to the report, "About 80% of the savings from higher cost-sharing would be due to decreased use of services." The report estimates that 1.3 million Medicaid beneficiaries would have to pay premiums and that 1.6 million would lose benefits, most likely for dental, vision and mental health services.

In addition, the report estimates that 15% of Medicaid long-term care beneficiaries would have their coverage delayed because of additional restrictions on asset transfers.

The asset transfer provisions in the bill would impose punitive new restrictions on the ability of the elderly to transfer assets before qualifying for Medicaid coverage of nursing home care. (Click here to read these provisions.)

Years ago I did a death summary by a GI doc.
I don't think GI docs are used to having patient's die and I don't know if it was his first patient to die not, but he was crying so hard at the end of thedictation it was hard for him to dictate.  I think he was in his second year of practice, so around 30.  I can't remember he if made a statement or not, but I'll always remember him crying.
Ditto, short ERs or short OPs....short anything! sm
And I especially don't like long OPs, they are the most boring for me, and the doctors pause a lot cause they have to think more. I just want to be done with it! Next!!! ; )
Long short forms vs short short forms

Carla,

A long short form is usually easier to remember. So I would definitely assign the short forms as suggested by Kelli:
SmithCThead
SmithCTabdomen
etc

In most Expander programs you would opt for a short short form as you have to type the whole short form before you can expand.
In Instant Text you don't have to type the whole short form: you type the first two letters of your long short form and then you can type any of the following letters.
So in this case you would type smcth for the first one, smcta, and you may also type smhead, smabd

This is possible when you use the Instant Text marker key to expand. If you use the space bar to expand, you may be better off assigning the initials as suggested in the second response, as you would not be able to "jump ahead".

I would have to separate by category!

first 3 that come to mind are:  The Notebook, Hope Floats and Sweet Home Alabama (SHA didn't get rave reviews, but I just loved that movie!)

ET, Titanic and Forrest Gump are like classics and you HAVE to love them!

Independence Day, MIB, most of the Marvel (comics) movies, Van Helsing and both Underworlds top my list too!

Oh, and then there is the animated stuff... the list is never ending! (and we own over 400 DVDs!)

Not on a separate line, per se
but separated by paragraphs. It looks something like:

Name #1
Address
City, State, Zip

Name #2
Address
City, State, Zip

...and so on.
MS Word not separate (SM)
MS bundles Word with their MS Office product. Previous companies I have worked for had MS Word alone on a generic disk. As far as I know, none of them are in jail yet.

OpenOffice.org provides a program that rivals MS Office. You can download it for FREE from their site. If you are doing this for work however, I don't know that it would be compatible with platforms you would be using - I'd say probably not.

Should you need to purchase MS Office so you can have MS Word, check out newegg.com. I know with operating systems such as Windows XP, you can buy versions that computer builders use - no fancy packages, just a disk and a license - for significantly less than what you'd pay for an off-the-shelf product at say Staples. Don't know if you can do the same with Office. Check out CNet.com for reviews. CNet.com also has forums where you can post a question and an Editor will answer you.

Good Luck!
I have a separate office
Although it's open and doesn't have doors.
I use a separate room
I have a 2 bedroom place, hubby and I use the master room and I transcribe in the "office." This keeps me, for the most part, without distractions other than those I create myself. I set up a place for my cats to sleep so they don't sleep on the computer desk, and have found that this works the best for me...very short attention span when I tried it in the living room. Just don't move work into the bedroom you sleep in, might cause sleep problems.
Are they separate checks (sm)
totalling 12,000+.  You'd have to check the court system, but that may actually be over the amount for small claims court and you'd have to go to the next level.  Has she made ANY attempt to pay any of it?  Do you have any "demands" for payment in writing?  If not, I think I'd get that in the mail today.  You need to establish a papertrail.  Formal demand asking for payment in a set period of time by cash or money order.  Send it cert mail with return receipt.
Yes I did have a separate firewall....sm
I have also used the AVG for years. My friend has also used it for years. I know a lot of people who have used it and continue to with no problems. I had no problems until someone hacked into my computer. When I took it to the computer guy, he said you are using a free AVG version. He said that is the problem. He said why do you think they are free? They don't fully protect your computer. I have no idea with a firewall on how they still got into my computer but they did. My friend had someone hack into hers while using AVG free version and separate firewall and stole her identity. I am just trying to keep the expensive mistake I made from happening to other people. It is costing me $250 to have the computer repaired.

Two separate issues

Her medical records were indeed compromised several years ago and they did a sting with the doctor to find out who it was.  The person responsible, ironically, died of cancer before she could be prosecuted.  Farrah talks about it in the documentary.  The documentary itself is Farrah and Alanna filming her treatment and journey over the last year or so.

Yes, it sure is. I was told on 3 separate occasions that

I use a C phone and 2 separate lines.. sm
I have DSL and I was not able to have my DSL on the same line as my C phone though now I cannot remember why it did not work. I do know that if it had worked I would not have had a phone to use to call out or receive calls while I was working (which was fine by me.. my family knows to call my cell). At any rate, because of the fact the C phone and DSL did not co-exist I had to use a separate line for my C phone and then my DSL is on another line and that frees up my main phone line for calls in and out.
Are the numbers in a separate column? sm
If yes, go to the Data menu and choose sort, then sort by the letter of the column.
Also fortunate enough to have a separate room. And

corrections or addendums. We are not even allowed to change any report once it has been sent, and chances are that we are not the original transcriptionists who transcribed it. It is simply a new report.

I don't really mind at all when they say "go back up to History of Present Illness and put this in". It is just part of the job, not a big deal at all.
If it were me, I would have a separate hourly rate that
I would charge for things such as that, and keep a total of the time you spend doing this, but definitely bill them!!
Is it possible to use 2 separate monitors with just one computer?
I'd have two monitors but only one computer.  I guess there would have to be somewhere to plug it in...and I only see one place to plug in a monitor but was hoping someone knew of some kind of adapter or a way to get around this?  Thanks!!

Info on using 2 computers to separate jobs

Does anyone else use 2 computers to work different jobs? I'm having conflicts between 2 companies so decided to buy another computer to separate the them. I  plan to use my monitor and cable on this computer too.

I  was told by the one company that I cannot use a router. Does anyone know why? I'm not very computer literate anymore so... how else can this be done?

I certainly don't relish the thought of unplugging and plugging my monitor and cable every day and definitely do not want the expense of another cable line. I am not reimbursed for my cable or equipment by either company. Thanks for any info you can provide.

Is each address on a separate line? And do you have Excel?

My office is in a separate room, yet close enough
I can hear what is going on. I have a shock collar that I put on my dogs to keep them quiet when they are outside (I don't have to shock them but very rare occasion). I work late afternoon/early evening so that part of my schedule, pets and husband are in bed and I have total peace and quiet.
separate bedroom makes sense
at my last 2 places that I lived I always had my office in a separate room but Iwas on 1 floor and I guess I felt more "together" that way. I am going to give my upstairs extra room a chance.  At least I will have a window that sees a pretty scene small creek, tall trees.  The only downfall my little dog cannot look out the window there as the windows are too high!  Thanks all.
No, having a separate office doesn't make
me concentrate any better. The part that is nice about it is when I'm not working, I can close the door and not think about work. When I can't concentrate, I usually put in some soft music, light a stinky candle, make a cup of tea and force myself to type for at least one hour before taking a quickie break. I pretty much make a deal with myself as a reward to get it done.
It sounds like you are talking about a separate platform
in addition to the Express Scribe. Maybe part of those steps would work for the OP, though.
When I left my former employer, they cut me a separate check. (nm)

You're talking about a separate program add-in.
In the standard MS Word program, there is no such creature as "word count" on the Tools menu. Either you're not useing MS Word or your company added this program. The other poster is telling you how to get the line count using the MS Word statistics/properties for a document.

If your company has added this program feature, they can configure it any way then want to and you're at their mercy.

Please make sure in the future when you say you're working in "Word" that it is MS Word as that is the assumption that will be made by anyone reading your post.

Your Shorthand expanders can become separate glossary. s/m KyITuser

Instant text actually comes loaded with dozens of different glossaries stored into a separate glossary folder.  For instance, to name a few:  Medwords.glo, (glo is a filename for glossary), Legal.glo, ER.glo, GenMed.glo, etc.  I could go on.  After I imported my PRD+ expanders, I created a folder for it and just named it (after my first name), Toni.glo.  So when I pull up IT, I just click on my Toni.glo file and go to work.

Ironically, I'm looking into ShortHand because even though IT has SO, SO many different functions...so many that I will never learn in this lifetime, that in itself is the only con...other than the price $189.00 and no trial period is offered!

Personally, the best advice I can give is to visit the productivity board and browse through it.  Don't get me wrong, IT has to be the most advanced expansion program on the market, but again, being so advanced is it's only pitfall.  If you are like me, an IC paid on production, I just don't have the time to sit around and play with it.  I hope I didn't confuse you even more.  Good luck!
