Medical Phrase Index is a necessity. sm
It is the easiest to use, covers many many words from many specialities, and I use mine every day.
As far as specialties, Stedman's makes reference books for pretty much every speciality.
Also, I use my Stedman's Medical&Surgical words a lot even though I'm not doing surgery charts; you'd be amazed how much you need to know just for a patient's past history!
I prefer the Drake&Drake Pharmaceutical Word Book, but everybody has their own preferences for drug references.

My number 1 recommendation is the Medical Phrase Index by Lorenzini sm
and would also recommend Stedman's Abbreviations and Labs - these are all more generic since you are doing a variety. It would be best if you could eventually get Stedman's Cardio and other specialities...but I highly recommend Medical Phrase Index for newer transcriptionists - it helped me tremendously in the beginning

I'm signed up with Andrews and am quite a ways into it but they still haven't sent the Lorenzini Medical Phrase Index, and I'm tired of waiting. Tired of them not sending back tests, too. Wish I had gone to M-Tec now.

it's not my fault, the darn quote mark just doesn't post for some reason.
Okay, for some reason, the first quote at the front of qldc does not show up, but in the line autocorrect.entries.add name:=qldc" there needs to be a quote mark around each side of "qldc", not just at the end.
If you want to use this macro -- and find it won't work and are confused my instructions, please write me and I will e-mail the macro to you.
I really don't like the idea of people having to spend their meager pittance of a salary on expensive software to do such a simple thing.

It sounds as if in the transcript you're talking about, the subject isn't clear and I agree it wouldn't be appropriate to add it.
The MT is probably putting it because s/he is (a) trying to make a complete sentence and (b) thinking of the rule of always putting "The patient" and not "Patient" to start a sentence, which is commonly taught. It might be tough to get the idea across, but I would agree in trying!

Mixed Latin and English abbreviations "confuse" the health care worker. Doesn't compute. Next year it will change. Maybe too we can spell things phonetically for the healh care worker who is confused. The new spellings can bring in new revenue to AAMT.

but I can take the occasional complaining... what I do find annoying and WORSE than complaining on this board is FLAMING. Why are some so b*tchy to people they don't even know? I'll never understand it. We're all here to support each other and talk with people in our career, share situations and ask for advice, give advice and encouragement. There's no need at all for insults and cutting remarks.

Is everyone just using spellex or stedmans or are there lower cost alternatives?
How did you find them in the first place? Just curious because I don't want a cheaper product to act cheap also.

It is just not enough for the work. Speech is great on the wrists but you are not just reading what is typed, you are making a lot of changes. I have a hard time doubling a lot of days with all the changes I have to make. Just my opinion.
The same way you do any accent. Do
the best you can and go through the entire report, leaving blanks as necessary. Then go back and listen to the report again. I'm sure you'll get some blanks. Also listen for what you think the blank might be. Don't guess, but have your mind set that you know that if they are dictating CBC that your choices are limited in what the options are so listen for those.
This always works for me. I still may have a blank left, but I usually get most of them filled in by doing the above.

My very least favorites are the ones who 'swallow' a few syllables at the beginning and/or end of their sentences. It makes it nearly impossible to decipher what they might be saying and just really an annoying habit on the dictator's part.
I'll take an ESL over a 'gulper' any time.

This is a big question mark for me also.
I have SBC Unlimited currently & know they have been notorious for dumping their unlimited plans. There again, some of the girls on the boards have been, others haven't been. I'm afraid to risk it at this point. I'm getting ready to try Vonage VOIP - Small Business Unlimited Plan & can only hope it works out. It's going to cost me more, but I'm hoping that the increase in cost will offset my fear of being dumped from their service, out of work & with an astronical phone bill for the minutes over what was allowed. I'm petrified at this point. I haven't found anything that says it limits your usage to 5,000, but who knows what is in the fine print that I'm missing somewhere! I'm still digging & think I may try to call them. I also read in all my insundry print-outs, that some DSL carriers will block the ports needed for VOIP. Guess who my DSL carrier is - SBC !! I have no idea what is going to happen. I may end up retiring & end up going on welfare! If I find out anything further, I'll let you know.

Mark Forstein seems like a very nice guy, but don't expect him to do anything on paper. He likes working on hand shakes so he can switch the deal on you later. He lives in Fort Lauderdale,FL now, and after retiring from Medquist, he tried investing in a Medical Billing Company called R-Solutions, but later took it over after the "brains" revealed that he didn't have the money he made Mark believe he had. After Mark took it over, he basically ran it to the ground. When he took over, it started a slow decline, then his wife took over, at it went out of business within 3 months. Meanwhile, he tried ripping people in debt off by starting a company called Nationwide Consumer Credit. This very shady non-profit business promised to help people out of debt, but the only thing they cared about was taking thier deposit. Rest assured they weren't trying to help people in debt. He set up a parent company called Landmark Credit Services to take the profit. BAD MARK FORSTEIN!!

I think it's the way Indians interchange Vs and Ws. For example, I have heard Indian docs say, "The patient has had chills and Womiting." Many Asians replace Rs where Ls should be. For example, "The patient had a biRateral beRow the knee amputation instead of biLateral beLow.
Some people may consider that just part of the accent. Even so, years ago, one of my favorite dictators was an Indian. He tried very hard to articulate and not mumble. Once you trained yourself that W was a V and "wise-wersa" he was a breeze. I had to toss that in there because he always said it.
For me, it's not any particular accent, it's the speed of
the dictator's talking. Accent or no, when they blast through their report like their pants are on fire, what they get in return for their sloppy dictating habits is a 'swiss cheeze' report full of holes.
I just don't get it sometimes - if teh reports are medicolegal documents, and are important enough to dictate and put in the patient's medical record, I think theyre important enough to dictate more carefully.
My other pet peeve is letters. Most of the time, I find it difficult distinguishing between F and S, B and P, C and D, etc. I think they should come up with some kind of a code when they're going to be dictating letters, such as, instead of saying 'CPT', they could say, 'code cat-pig-tiger'. Or something to that effect. It would save an untold amount of time backing up, relistening, and ultimately leaving blanks in what could have easily been a complete, accurate medical report.

Heat index here Sunday (according to National Weather Service) was 126°! 97° temp and 77% humidity = 126. Not sure how they got that figure, but it was during the county fair parade, and some high school kids marching barely made it the 3 mile route. Kids and I escaped to a movie theater!
Considering the heat index is 107 here today....
Shorts and a tank top work for a lot here today. I myself wear shorts and tank tops that I find from DKNY, Liz Claiborne, etc. So they aren't junior clothing and are appropriate. Just because I am 47 doesn't mean that I need to put a sack over my head or this body which is in great shape, considering I've had four children, and a hysterectomy. Still a size 8 and will continue to be a size 8. :)

this is insane. when you see who is running this - the person whose claim to fame is actually making the medical transcription business what it is today, boasting over 400,000 MTs.
now she is trying to what, cut that workforce down by maybe more than half.
this is an extremely aggressive push for mobile healthcare and point-of service care, which from what I see completely eliminates the need for people at all, period.
does anyone know, please, is there a future for us in this picture. is there anyone working with these people to help preserve our jobs in some form at least?
where do we write, who do we talk to? this person also used to run the AAMT, and now has jumped ship for the opportunity to make probably millions for front-running this technology. does she even consider the 400,000 MTs (in this country alone) losing their livelihood. does she care?

Change provider vs change to business plan
In your place, I would seriously consider changing providers if there were others available in your area, but I'd ask around about their service quality first. It may not be any better. If there are no other providers, then consider changing your service package to a business account rather than residential. They're usually more expensive, but they also tend to get faster service if there's a problem.
Last summer, I had no electricity for 3 days due to severe storms, but there were some 600,000 in the same boat with me. We just had to wait it out. It's a downside of working at home.

Vietnamese accent the worst.
There is one Vietnamese doc who not only dictates like greased lightning, he also stutters, stammers, and changes his mind. I think he may have a bit of a lisp also. This is in addition to the problems that Asians have with Rs and Ls. This doc gives me an ocular migraine almost every time.
Spanish and Middle Eastern accents are a piece of cake compared to this guy.
An accent trick that helps me...
After spending way too long trying to figure out an allergy one night that was pronounced as s/l fee-oh-REE-nul and which turned out to be Fiorinal, I started double-checking and realized that East Asian (India, Pakistan, etc.) and Arabic language dictators will often incorrectly accent the 3rd syllable if the word happens to naturally be accented on the 2nd syllable.
So when I have a problem with a word that's accented on the 3rd syllable, I'll re-emphasize it in my head, changing to hitting it hard on the 2nd, and that will often help. Or just playing around with emphasizing different syllables from how they're doing it. Now if I could only find a way to mentally blank out the 'trilling' (arrgh).
