NOBODY should have to worry about this junk, cuz it is just that, JUNK. FYI, in 20+ years of transcription, I have never had a single entity, be it a hospital, a clinic, a private practice, or any combination thereof, bring to my attention a report that I did with a hypenation error or a spacing error. Nobody EVER so much as gives the time of day to that sort of thing, and nobody ever brought to my attention anyone ELSE'S error of same, either. (Clarification: When I say "nobody," I mean the original dictator or a nurse or another doctor.) In 20 years' time, I can recall maybe 1 or 2 "supervisors" who were all happy about being in a position to put work under a microscope and find such an error and ask that it be changed. But anyone who matters? No, no, a thousand times NO. They JUST DON'T CARE. The sooner you realize that, the less wound up you will be, and the easier your job will be. Spell your words right and transcribe what is said, and as well get the numerics correct in your labs (this includes decimal points), and you can wash your hands of the inconsequential spaces and hyphens as so frantically cited by the above posters. That is my two cents. My EXPERIENCED two cents.
