I transcribed for a doctor once who was also very picky about commas. He was picky about other things too. The company I worked for sent all his reports as drafts, because he would pick them apart, underline certain things, etc. I soon learned that, after putting commas in any series, including before the "and", I only inserted a comma when he specifically dictated it (and he would). After I "got it", he asked that I be the only person who did his work- just on this issue alone. He is the only MD I ever transcribed who even cared about commas. However, some have been picky about other ridiculous things. We had an account manager who used to go over reports with a fine tooth comb and send corrections. I did not have problems with her over commas, but she wanted us to listen to the dictation and try to make it more concise (define shorter), using abbreviations as much as possible. I told her she had to be kidding. I would make every effort to be grammatically correct (this particular account was an ESL doctor), but I was not going to listen to a dictation and then think up ways to cut my paycheck. The doctors did not even specifically ask for this. I do not think doctors (and some QA) have a clue how we work. However, I do not think they care either.
