I work in the Flint area.  I work full time at a hospital in a different capacity.  About thirteen years ago, I approached one of the physicians on the floor about the possibility of doing transcription for his office, but he didn’t need anyone at that time.  About three months later his office contacted me as they had lost their transcriptionist.   I have worked for him ever since.  I started at 9 cents per line, and at this time I am 12 cents.  I pick up and drop off daily as his office is  near the hospital.  They provide all the paper and ink.  I charge per gross line plus envelopes, which is why I don’t complain about a 3 cent increase over a 13 year period.  He has since added 2 more physicians to his group and I type for all of them plus the various residents that rotate through his office.  They are primarily still using tapes, although I have tried my best to switch them to digital, they are resistant to that idea.   I have a friend who owns her own MT company.  She types as well as 2 other girls.   She has about 10 physician office accounts and they all visit their own docs on an independent basis for pickup and drop off.  The owner is basically also the Editor and she monitors the other 2.  I think she makes 11 cents per line (I’m not sure if its character line or gross line count).  I had typed for her for awhile when she was in a pinch.  I’m not sure how much she pays her girls per line, but she does take a cut. No benefits, but she makes great money.  She has a mix of docs between tapes and digital.  She tried to work FT outside the home and split all her work between the other 2 girls, but she hated it and after a couple of years went back to running her business and typing at home and she loves it and said she would never go back to the rat race… too many politics!  I think if you want to work independently the way to do it is to find your own accounts and not work for other people.  Then you are not doing all the work and getting a cut.  If you land one physician account, then by word of mouth, a little advertising and making the rounds through the doctors offices, and a little networking, you can pick up more.  I have had multiple offers to pick up more offices over the years, but with working FT at the hospital and the one account I already have, that is more than enough work for me.  Also I am picky, I refuse to type for a mumble mouth or for a specialty that I am not comfortable with.  Hope this helps!
