Where and what did you once transcribe? Of course, every report has to be a separate document and every report has to have a header per facility's protocol and specifications. Why would you put H&P, op note, consult, discharge summary all into one document! What a mess that would be. What do you mean by "procedural details"? Your message doesn't sound as if you've ever transcribed in the good old US of A.

kid's crazy waiting to see mars
do you have kids stay up late just to see mars... oh!! my poor little girl waited for this planet since the start of august, and she is hoping to see it tonight or tomorrow as big as moon. she even marked it on the calendar... and what's funny about it is that we have never spoted the mars... not once. grrrrr!!!!
