They're so used to somebody (like us and/or nurses, etc) picking up after them and waiting on them hand and foot, that they just don't think they need to bother. When I worked on-site in a hospital, it was easier just to get the chart and write their dis summaries for them, but of course we can't do that from out here so I guess we have to use our mind reading skills.

No doctor, nurse or any other health professional bases their treatment on a typed report on the chart - and if any of your employees believe that bunk, they deserve to have you as a boss.

I could never imagine staying in one job for that long if I was as miserable as you appear to have been for quite some time (20 years).
I have only been at this for five years. I have worked in two major hospitals, one clinic, and at home. I have always DETESTED working inside, especially during the day and especially with meaningless office banter and gossip, not to mention the snobbery of most upper management or "administrators".
This led to me to MT at home, which I love. I do not work for a large national with multiple benefits, so what you are offered sounds good to me.
Honestly, as long as you can keep your production going (overtime is not a problem where I work...they would love you for it) and not let the isolation that sometimes is overwhelming (for some...not all)from bothering you, it sounds to me that you enjoy working at home.
IMO, VR and India will not take all of the jobs before you plan to retire. I don't know where you live, but where I am, there are plenty of IC jobs to choose from. I am honestly too lazy to go after them because I like not having to deal with all of that. I like to get lines, get paid, and be able to play with my dogs and wear pajamas. So, in that way, I am spoiled.
I would try to secure another job before quitting my first, though. It can take a week or so to get used to your new company's system, templates, QA people (!), etc... to get your production up.

I spoil them too, but not to the same extent. They are 6 & 8 now (girls). He seems to think he has to buy them something everytime he goes out of town (sometimes this is as much as once a week), so it can add up quick, and they were starting to ask him every night he came home from work "what did you get me", I told him it was way out of hand and had to stop. He had gotten better finally but still not quite where I want him to be. They generally wake themselves up in the morning, but if they are not up yet I wake them up. They pick out their clothes and dress themselves, brush teeth. I get their breakfast though and brush their hair. My 8-y/o does her homework on her own for the most part, I might sit with her while she does it, then check it when she is done. As for chores, nothing on a regular basis, but that is changing shortly, have quite a few things that I do that they can handle just fine. They both clean their rooms (the older one under duress); take turns cleaning their bathroom; and help pick up the LR adn DR (again under duress) when I want to clean/vacuum. We do not do allowances as yet, but are teaching the 8-y/o about $$. She keeps wanting games for the DS Lite, which she paid $60 toward the purchase of to her uncle for her birthday a couple weeks ago; she sells our eggs (have some chickens) and we let her have the $$ which she saves up to get things; granted the egg money won't get her far ($10 a month if that) so I have to keep an eye on how much my DH slips her (which drives me nuts, keep telling him to at least make her do something for the $ and quit just giving it to her). I just don't want my kids thinking they will just be handed $ whenever they want it from dear old dad. Hopefully my visions of the future will not come true.

I am at fault by spoiling her so much; I really love being with her, I've been knitting sweaters and watching the Wizard of Oz with her and reading, she loves for me to read books to her and play with her, she's an only child and both her mom and my son work and they are young so you know how the priorities are at that age, nobody spends time with her but me so she's thrilled when I see her. She cries when her Mom comes to get her and climbs on me saying "No" she doesn't want to go. My son is usually busy playing video games or on the cellphone when she here visiting so she comes to me for attention. I feel bad, but I really have to work, I've been out of work for 2 years now. I appreciate all of your feedback. You guys are great!!

She sounds like a self-centered spoiled employee, IMO. Does she not take pride in her work? If not, she is in the wrong field. I think if a Transcriptionist ever gets to the point the errors don't bother them, they need to consider a profession change. Yes -- they're small mistakes right now, but how long until she transcribes the wrong medication dosage or something really big? As I tell my girls, a decimal in the wrong place can mean life or death for a patient if the doctor does not thoroughly check the work and calls in a wrong dosage.

Most kids are "spoiled" in some way or another. I have two teens who do not ask for any material things. They do not care about name brand clothing or shoes, etc. They are leaders - not followers.
They keep their rooms clean, help out around the house, clean the kitchen, bathrooms, help me do the laundry. Now that school is back in - homework and school work come first. My husband and I spoil our kids on our own just because they are "good kids."

For instance, do they do chores that are age appropriate, do their own homework, get themselves up in the morning and ready for the day if they're older than say age 10? Here's how it works in our house:
6-year-old: Responsible for cleaning her room, feeding her fish and bunny rabbit and her bathroom each week. Puts dirty clothes into the hamper and puts up clean clothes except those that have to be hung, which we do since she's not tall enough. Likes to mop the kitchen floor so we let her do that. Does her own homework after I make sure she remembers the directions.
15-year-old: Has to clean her own room and bathroom - alternates the bathroom cleaning with the 6-year-old since they share a bathroom. If she cooks anything that's not a meal for everyone she has to clean up after herself. She's now learning to do laundry - my DH always took care of that for her in the past or her mom did, but I've forced the issue that she can do her own laundry now that she's 15. Also has to do her own homework without help from us. For instance, while doing English she commonly asks for the meaning of a word. I tell her to go to webster.com and look it up or grab the dictionary - which is how we learned as kids. Also her dad (and bio-mom) used to help do her homework by looking up stuff and writing the answers which she'd re-write but that stopped while we were dating as I got him to realize that he wasn't helping her by doing this and she needed to do her own school work and not whine to get him to help. She is also required to get herself up and out the door for school in the morning, on time, and knows that if she oversleeps and misses the bus because she's goofing off then she gets no PC or TV privileges for the day. It only took her 1 time of missing the bus to learn to get up on time.

When I started out we worked on typewriters (do you know what those are?)- some had correcting tapes - you hit the backspace and typed the same mistake, took that off and then you backed spaced again and continued typing. On the first several computers we had no spellchecking at all. Where I learned we had to have less than 3 errors (either medical or English) on a page before we could get production pay. Our work was much, much better than what I see just being posted on here with the misspellings, etc. I use spellcheck now but only because, first of all required and secondly just makes me feel better but not a crutch for me in any way.

I have a dozen yrs in, I am an acute care MT, OP note specialist. I have been shopping for a job a lot here lately and I have had my pick, so I have been very choosy and I have not found quite what I think I would like best...yet.
In this search recently, about half the places that say they require testing have deferred testing me. I have a short written multiple choice test to do this afternoon, which I don't mind because the MTSO said that of the 100 resumes she got, mine was the only one that popped out at her as the ONE MT to talk to about this position. Okay, so I am a sucker and with an ego!
BUT, if someone thinks I am going to do more than 4 notes or use my keyboard to start and stop the dictation, they are can take a hike. I have to use my pedal and it needs to be 4 or less, period, and they need to be short.
My current IC position they required a test. The recruiter said I'll send you the test to do because I have to, but I'll call you tomorrow and hire you regardless. She said she felt she had found the right MT for the position when she read the first OP note I had transcribed, and this is as it should be.
All of you are right about this issue. I also think that anyone who hires MTs regularly can tell by talking to you whether or not you have lied on your resume, whether your English skills are adequate or whether you sound like a total buffoon or whatever else. Besides which, once you are a well-experienced, well-seasoned and high-caliber MT I think you have already proven yourself and a test is worthless.

You're not lazy. I'm old school, 53, 30+ years of transcribing, learned on the job in a hospital, took a few terminology courses and that was it. Best way to learn transcription but this is no more, alas. I believe it's the times we live in, the attitudes about actually having to work daily for a living, management in charge (younger generation?), our love in this country for excess nonsense "education" instead of common sense and experience, etc. I have coworkers who don't even bother calling in sick, they just don't show up and don't even get written up. That in itself blows me away. Nobody trusts anybody's word, of course. Okay, I'm done. I'm just glad I'm where I am and hope they don't change too much more before I can retire, ha-ha.

newbies are doing this. You learn nothing if you don't know how to research and find things out. Besides there are too many questions answered about stuff that folks should know...the basics....when they get out of school and start. I am not talking about bad dictators etc, I am talking about plain every-day medical terms that go with physical examinations, etc. etc. Its obvious people don't want to research, which I AM SORRY is part of the job. Shouldn't be working production if you are not expert at the profession yet.

That sounds beyond lazy to me...
I neglected to mention in my post above that my wonderful husband helps me out all the time and on weekends, he does all the cooking...over and above working 60-70 hours a week... (My post is the one re: 3 kids, DD works PT, DS college, etc.). I am more blessed that I ever imagined...
Not just lazy...DANGEROUS
This is how patient care errors are made. I know they hate dictating, but it is very disrespectful to their patients when the doctors won't even attempt to dictate clearly so that their patient's lives are not in jeopardy. They should be chastized by their hospitals and not allowed to dictate like that!

I have a couple of MT friends. I make about twice what they do, but I "work" roughly the same hours they do. They do a lot of whining about wishing they made what I made. When the work is there to be done, they tell me about the work piled up, but how they don't feel motivated. By the time they get motivated, the work is gone. Then it is whining about finding a new job, but when contacted, they can't get motivated to test and one is always wanting me to look over her tests to see if I think she will pass (I saw that her work was very poor indeed) and I have only done it once, which was enough.
Working at home is not for everyone. It takes a lot to park your buns in front of the computer when the dishes are in the sink and the laundry is piled up, or the kids, DH or whomever wants your time. It is SO EASY to not work. I get distracted by the housework, especially today because my daughter was home from college this weekend and every single coffee cup and bowl are dirty! I did her laundry, not mine over the weekend...so I feel distracted.
The OP is right. Use the answering machine or voicemail, don't answer the door (I never do) and get down to it. If you are like me and the housework drives you nuts, you can do what I do. I type 3 reports and go do a short, under 5-minute task, and sit back down. I find I can get the kitchen 90% clean in a day by doing this and I catch up my laundry this way too. Then I can focus because I like a really clean house to support me when I am working. It is a support because I don't think about it when it is clean.
My last rant is about those $tupid TV ads about working at home as an MT. Gimme a break! It is NOT like that.

but I swear when I opened up the testing and it was 6 reports including discharge summaries, right then and there, I just thought to my "NO way." I guess I just don't have the drive or energy to do testing for a job that takes a hour or more. Part of me realizes that they need this information, but the other part, just thinks it's ridiculous to expect people to spend so much time on a test. Trust me, most of the time, when you see one report, it's quite apparent if the person has any skills or not. Sooooo, I guess I'm lazy because I'm NOT doing this test. Anyone else get these feelings...Maybe I'll regret it and maybe not. Time will test because a lot of other people were sent the same test so I'll wait for their report LOL

Where I feel crazy, I set a goal of around 300 lines an hour (ha! not with these dictators but 250 is still good), every two hours make a snack to get me moving. I love juicing veggies and making fruit smoothies in the blender, coffee makes me nuts. I also do calisthenics (sp?) every couple of hours for about three minutes to keep my blood flowing and keep toned. I walk the dog two times a day for about 10 minutes to get energized from the sun and keep a good color. I take an hour break to watch good ol Judge Judy, and fix myself something nice n yummy, and after Judy I crank out my last hour called the 'power hour' where I take no prisoners and smoke comes off my keyboard! Like a race to the finish line.
I can't see how you could do the 20 and the 10 minute clean break, that would slow me down! Maybe I should take cleaning breaks, because I never want to clean when I have a block of time off and the dirt just collects.

Only a person who consistently leaves blanks because they are lazy should be docked.
Or maybe they should just be fired. The first place I ever worked docked us for errors that we should not have made. I was only docked a couple of times, and it was only a small amount, but it made me pay attention and be more careful, and I think it made be a better MT.

Now you're being "nice". I am NOT worried about others. Why would I want to work an 8 hour shift and produce 1000 lines if I could be missing SOMETHING and increase my production? How mean-spirited you are. I would LOVE to produce more and work less, so that I could spend more time with my loved ones. And to read on this forum the astonishing line counts most MTs are claiming to make regularly - well, I'm either doing some really, really wrong, which I would like to fix, or I'm not doing something that I need to be doing, which I would like to fix! Thus, my sincere question. Truly I believe it is time to leave this board, though I used to LOVE it so much, cause you just can't get an honest thread running anymore about anything - even trivial threads turn ugly by 2 to 3 posts, let alone INFORMATIVE ones like how honestly asking my fellow MTs who are huge producers HOW they do it? As in straight typing or using normals and standards? I try to hang on this board and stay "positive" despite the negatives, cause I really like it here and I really like the admin, etc, but I'm not a glutton for punishment. I know a lot of the old regular MTs who actually carried on intelligent conversations have gone to the board where you have to pay $$$ to post. Apparently, one can still have kind meaningful conversations over there - guess the $$ part weeds out the trolls? Don't know, but its sure sad. Please, don't misinterpret this as me thinking anyone would give a hoot if I left here - cause I know now there will be the normal "good - who gives a crap? good riddance" flames, but really, the MTs who are so trollish are just ruining it for themselves. Thus is their lot in life, though.

I can take an ESL more than I can take an American who speaks fast just to show off to his buddies or because he has a hot date and is dictating from his car or worse yet, if you can't understand him, he has an excuse why his report is incorrect, he can blame you for it. They do it, believe me, and they are the ones who should be shot down. Give me an ESL any day who says thank you very much and is trying hard to learn the language. Some of these fast-talking big shots could care less about us. I had one guy who could fit 15 discharge summaries on one side of a 15 minute tape and everyone would send him to me because I was the outside service. When he was spoken to, he just threw his head back and laughed. Ignorant!

don't you just love it when you have to turn up the volume to hear what the doctor is saying and then in the middle of the dictation he finally puts his mouth to the mic/phone, blasting your ears; so you turn it down then by the end of the report you have to turn it back up.......no wonder I have ringing in my ears sometime 
Bad dictators
Chewing gum very loudly, blowing bubbles while dictating, drinking something and then chewing on the ice....all in the average day lately. Don't these people stop to think that someone has to listen to this? That they are chomping incessantly into someone's ears. No common sense. Maybe once, someone in authority should remind them that these are legal documents and need to be transcribed as accurately as possible, so no chewing, drinking, talking while yawning, etc.

I have got to one-up y'all... there is a woman doctor in our group who actually takes her dictaphone INTO THE BATHROOM with her and, yes, that's right, uses the potty right while she's dictating...
ESL dictators
Maybe the ESL docs, knowing that the Transcriptionist must be having a difficult time with the language barrier, should hire an English speaking person to do their dictations. It would free up their time for being a doctor and seeing patient's, give a more accurate transcription, and a better quality report. Wha'cha think???
ESL dictators
Seems many MTs have this complaint. We should be told the correct percentage of ESL dictators BEFORE accepting a job. Imagine all the money a company would save by going over details with each new applicant honestly and thoroughly in the beginning instead of making the job sound good and then have a disappointed new employee, who will quit and the company will have to advertise and hire again. Advertising and training on a computer system is costly, including the time involved etc. The more a company can do to retain its employees, the more money it will save in the long run. Also, pay based on production, not quality of work, is counter productive. Imagine if each Transcriptionist was paid a salary and did quality work and no QA people were needed, except to pull random reports of new employees during the first 30-60 days or so. I feel there is too much QA and not enough emphasis put on quality reports from the beginning. Many transcriptionists feel, the QA people will fill in my blanks, yet they may be able to find the word/words themselves if production was not such a huge factor. But too many employees are trying to meet a line count. With a salary, you can establish a budget and know what to expect in your paycheck. I've transcribed for 25 years. I love my work. Since outsourcing and pay by production have been instituted, I feel pride in transcribing has suffered. Give an employee a job they can feel great about, benefits, and treat them with honesty, fairness, and respect and they will be glad to work for that kind of company. Hopefully, there are companies out there like this It's up to us to find the good companies.
Of note, 10 years ago transcriptionists were being paid around 10-11 cpl and $3.50-$4.00 a page. What happened, outsourcing!! We used to get a higher salary than coders, but not now. We need to keep our work in the USA.

I had my share of bad dictators this afternoon too! Is it full moon? I am sure I had your doctor's wife. LOL Can we band together and chart a bus to deliver them to the firing squad?? LOL At least maybe would could let the air out of the tires on their golf carts! Do these people listen to how bad they are???????
