Should be a high suicide rate too funny. The MQ style says spell it out in diagnosis and impressions. As far as the blanks, QA them, let them deal with it. I find that I leave the QA marker, finish the report and go back with a fresh ear and some new information sometimes you can fill em in. If not where I am no blanks allowed straight to QA. Don't go killing yourself :-)
