In high school, the peer pressure is to be thin, pretty and popular. If you are even just a little bit overweight or "healthy" looking, the pressure gets to you to become "skinny". It is a complex problem. For me, it was a low self-esteem and control issue. When things around me were out of control, including a dysfunctional family, the only thing I could control was my eating, so I chose not to eat and become thin, thinking that everyone would like me more, etc.
Your stepdaughter needs to feel in control and good about herself - the only way to do that is to complement her and encourage her. Tell her how pretty she is and thin she is, etc. Sometimes and most times, that is not enough. She needs to talk to someone, maybe a counselor she feels comfortable with at school. I can tell you that she needs to do it willingly or it won't work.

On a good note, I overcame my anorexia and am at a stable healthy weight. I can not have a scale in the house and can never weigh myself - haven't for 20 years. That is my obsession. If I get on that scale, I will tend to become obsessed with losing weight again, and revert back to my anorexic behavior.
You need to get to the root of the problem which is usually a deep emotional one.
