How much experience do you have? What specialties do you currently transcribe?
As for why would they make the test "hard" when you can look up previous dictations for phrasing? Well, that's something you may find surprising. The test is "hard" (or just not simple) to make sure you can handle yourself if you were hired. Looking up previous reports for phrasing isn't really the norm as it would be at an in-house position. Some companies have the capability, but most do not. I think you need to first decipher if the test is actually difficult for you, or your lack of a foot pedal and headset are holding you back. The tests shouldn't be that horribly difficult.
You sound desperate to leave your hospital position. Are you fully prepared to take a cut in pay, what probably will be a deep cut, at least initially. The "working from home" train isn't the breeze and solution that some think it is. Be very careful and really do your homework on this before giving up that in-house job. You may find yourself kicking yourself 60 days later.
Really give it thought and investigate before going with the "I want to work from home" theme.
