My mom escaped from a psych hosp when I was about 12 years old. It was in the middle of winter. She had no coat, no nothing. Went to someone's house and asked for a ride. They actually drove her to her house. I came home from school and there she was.

Hi All:
I am a little upset about something and needed to vent. First off, I have been with the same company for 12-1/2 years. I was recently promoted to shift manager about 3 months ago. My indian nation tribe decided to build a very large casino across the interstate where we currently are. I had actually put in for a transfer to the new facility, but the general manager of the one I currently work offered me this position. This woman is very critical of people, quick to judge and a very emotional type of woman. I accepted the position because the start out pay was not bad. I would have full benefits at the end of 3 months, which consists of the company paying 100% of my insurance, for my dependents it will cost 25.00 a month.
Well, my mother had been in ICU from May 20 until August 23, which she ended up being taken 2 hours a way from home to an extended-care facility. So, about every 3 weeks or so, I like to have off on a weekend to be able to go down and see her. I have been arranging my time off with the assistant general manager, mainly because she is easy to talk to and most people feel comfortable coming to her when there are any problems, and she KNOWS what she is doing.
I had spoken to the assistant GM 2 weeks ago about me taking off this weekend to go visit my mother. She arranged it to where I could do that. Well, Sunday I became ill with a cold and I called the assistant GM at 6 am and asked her if there was anyone who could cover my shift, there was. On Tuesday, I had to call in because my son was running 103.4 temperature. I didn't use any of my vacation time or sick time for these days, but was planning to fill out the paperwork to do this when I returned yesterday.
When I arrived yesterday, assistant GM, pulled me to the side to let me know that the GM told her to tell me that I was not going to have off this weekend, that the days I called in could be my 2 days off for this week. Actually, it worked out well since I have been ill and my son has been ill, but it is the principle of it. She did it out of spite. Even the assistant GM thinks that. So, I will work 9 days straight in order to have off on next weekend. She also told the assistant GM that if anyone needs any special time off that they have to go through her to get it. She (GM) does not work her fully scheduled hours during the week--in fact, she was just off for a week and a half for her daughter's wedding about 2 weeks ago.
I have not done MT since June, and I have really been thinking about quitting the casino and finding another MT job, but I feel trapped mainly because of the good benefits I get at my current job. I make 28157.00 salary, and my benefits will start on the 24th of this month. Not to mention the fact the length of time I have been there.
Also, during the time the new casino opened and was moving to the new facility, she did not have a shift manager for 3rd shift, so I was working 12 to 16 hour shifts to cover until they got one, which was about a month and a half ago. Because I am on salary, I got no overtime for it. Plus, she knows nothing about the electronic gaming of the casino games, just the bingo.

How long to make decent $$$??
This may seem like a stupid question to all of you "veterans" out there, but how long until a "newbie" can expect to make a decent wage in this business? I have only been employed for about 5 mos, so I am trying to cut myself some slack, but minimum wage is hard to swallow. I do reports from a pool of doctors so I can't seem to utilize the full benefits of expanders, and on my best day, I can type about 110-125 lph. Again, I know I just started, but I can't keep making this kind of wage forever. Are there tricks I am missing? Any insight would really be appreciated!! Thanks in advance!!

I lost my QA job 3 weeks ago because a US company bought the offshore company my original company was using and now brought in their own QA people. Apparently they hope to get these MTs off QA here real soon.... I just gotta laugh... I've been QAing the work for over 2 years and it is the same mistakes day in and day out... they just don't get it and don't care to 'get it'... they get paid no matter what so what is the incentive to learn? These MTs actually argued with me about contractions, etc., like I had no idea what I was doing.... I know you are venting and I hear you and totally understand... it's a sad place MT has gotten to over the last 5-7 years or so in my mind

accurate on what they are saying and if you have done this particular doc for 2-3 years i am sure you will be able to understand. start out at 120%, work there for a few days, increase to 150% and so on. i bypass all the wasted time with hmm, hmm, flip the page, hmm, etc. simply by speeding up the dictation and my fingers fly right along with their words. just FYI, don't immediately jump it too fast where you will spend more time slowing it back down to figure out a word here and there as that defeats your purpose and is more time consuming. adjust the speed periodically where you are comfortable hearing 99% of the words with 100% accuracy and build on that speed.
How long does it take to make decent line count?
I am considering joining an outsourcing company. Can you tell me how long it took you, as an experienced transcriptionist, to get up to snuff that you were making a decent wage? And what is your average rate per hour when paid by the line of 65 cpl?

Hitting the numbers
I have to agree with Busy MT, Anon, and Endiqua. It doesn't matter how you arrive at verbatim as long as it is verbatim. I have a verbatim account typing op reports. I have set up a file with each doctor's name and a sample report, e.g., Dr. Smith, lap chole, appy, hernia repair. When Dr. Smith dictates, I cut and paste whatever procedure he is dictating into a new document and make any changes as he dictates. As someone said most doctors use the same phraseology in each report, making only minor changes during. For example, 5-0 nylon, instead of 5-0 nylon sutures. Just take out and add in without straight typing every single word. It's still verbatim. Using this system, my usual count is about 300-350 lph. I also use my expander extensively.

Just wanting opinions. I find some days, my fingers/brain are really "on" with transcribing, but then I have other days, where reaching my line count is a struggle. Obviously, I'd prefer to all "on" days and I'm wondering if any of you have suggestions on being "on" more than off.

If in MS Word, sometimes hitting the "Undo" arrow (sm)
above can make something you didn't want go away. I've also discovered that if you are working online, like in email for example, hitting "Control" and the "Z" key at the same time works just like "undo". It's especially helpful for when you've just typed a long email, are just about to sign off, and you hit the wrong key and it DISAPPEARS! If you immediately STOP, don't touch anything else, and then hit "Control-Z", if you're online it SHOULD come back! :)
This way works, too, but using the ;yo or ;py cuts down on hitting the spacebar

shortcuts for windows. much faster and costs less. if you put a whole report in it, it will take a few seconds too spill out, but still faster than shorthand.

I use shortcuts to the max but what do you do if the doctor dictates in abbreviations? For example continually says CHF for congestive heart failure, etc. Do you type the abbrev? or spell it out?

Anyway I have never really used a lot of shortcuts (just the Autocorrect in Word) but now that I have to do more lines to make the same amt of money I need some tips on creating shortcuts so I can produce more lines an hour. with Auto Correct. We have a few doctors who have their own "normals", but 99% straight typing, proofing as I go.

I am curious as to why you would need a book on shortcuts or macros. Everybody is different, thinks different, types certain keys with ease more than others, etc. You have to do what works for YOU. I've been an MT for 15 years or so and I add shortcuts to AutoCorrect daily. For me, if I type the first three letters or consonants of a word I remember it easily. For a phrsae such as "heart and mediastinal contours are within normal limits" I'd use "ham". "Please see discussion above" = "pda", etc. Some doctors dictate the same thing for normal exams. I use the doctor's first three letters of their last name then the first few letters of whatever exam they are dictating. Becoming proficient at using macros and shortcuts is seeing what works for the way you think and type. Make shortcuts for EVERYTHING! "There is" - "ths", "are" = "rr" (it's easier/quicker to type two identical letters rather than take your fingers off a key. For shortcuts that are pleurals, I use the last letter twice: "demonstrate" = "dem", "demonstrates" = "demm". The less you move your fingers off the keys, the faster you will go. The more you use your techniques, the easier it becomes for you to develop your own shortcuts with your own technique that is easy for you to remember. Macro every month of the year for dates, macro the year "2007", whatever can be turned into less Keystrokes is beneficial.

I know how you feel, but unfortunately you must go by the rules of whomever is paying you or the client of theirs. I have had clients that marked my reports incorrect when using the BOS, they want it their way or the highway. Don't take it personally, just go along to get alone. Been there, done it, hate it!!
I use WP5.1 with XP and Shortcuts
and so I can use the expansions i saved in Shortcuts for DOS in WP5.1 with XP but I cannot access Shortcuts nor can I add things to Shortcuts via XP (unless I get the Shortcuts for XP) and I cannot access MACROS but can use the MACROS in WP5.1 that I saved for a long long time....
hope you all understand my post....I use WP5.1 with Shortcuts for DOS in WIN XP but cannot add anything to Shortcuts nor add anything to my WP5.1 macros but can use all I saved prior to going on XP last year. It all works out....

as 1y, 2y, 3y, all the way up to 100, where it expands to 1-year-old for 1y. I know most don't do that, but it got around the space problem you are talking about.

build your shortcuts to the max
it's ALL about your shortcuts. you have to shorten every word, phrase, not just medical ones, regular English too, assuming you transcribe for someone who SPEAKS English!!
I have been using the same shortcut program for 10+ years and I STILL add things to it almost daily. 1st rule is if the shortcut doesn't just pop in your head, keep typing and don't think about it. it has to come to you automatically. then you have to be creative. example - I have this one doc who constantly says "once this was done" so I have that in as OTWD. "The patient tolerated the procedure well" is PATOL. I have things like ADP for adenopathy, then you have to expand that on further to be LADP for lymphadenopathy, AADP for axillary adenopathy, etc. if you have one doc who says some phrase constantly, even if it is only one doc, if you type him every day, make it a shortcut. I have typed in "codes" for so long they are drilled in my brain. I'll look at a license plate and it will say RSD456, my brain says "reflex sympathetic dystrophy." the longer you use it the bigger it will get and pretty soon you're typing all shortcuts and not much else.

Use Shorthand9, I think.... I used it with Meditech all of the time for all of my shortcut needs.

I work at a hospital and my main doctor "Ah's" a lot and changes her mind and has no particular format. She just dictates and I have to put where it belongs. We get along fantastic, but I am lucky is I get 120-150 lph. With Psychiatry, I can get 200. However, I have an account on the side in Psychiatry where he speaks very fast and I can keep up so it comes out to about 300. I would be interested in getting some ideas on more shortcuts - as I am sure everyone would if someone would like to share them with us. Do you have a printed list? Or where did you come up with these? Any help is appreciated. TIA.

I know there are so many different ways to save Keystrokes and then there are those that "just seem to make sense" but don't necessarily follow any particular rule. One of my favorites is using the first two letters of each word. It strikes me funny sometimes at some of the combinations that result this way. I think two of my funniest are boob for bowel obstruction and pusy for pubic symphysis. Any others that you have? I'm looking for humor this Monday.
