I hate chart notes so that is a simple choice for me.

Chart Notes

Was wondering if anyone could help me out.  I need to see some example of different ways to set up chart notes.  If anyone knows of a place on the net i can view these of would send me some blanked out that would be great!!!!!

cut apart chart notes
Told you I was an old timer.  I print on sticky paper so that they can stick the chart note into the chart one after another on a piece of paper.  It is an 8.5 x 11 label.   I print sometimes up to 3 to 4 patients  on one sheet as I can and  I cut apart the different patient chart notes so the gals in the office don't have to do them and they can be filed right onto the charts.   Just an extra service I provide.   Takes me 5 minutes to do about 20 pages for all my accounts.

Wow, 600 lines from 30 minutes of clinic/chart notes - sm
The most I would squeeze out of that would be 350 probably. You must have one fast talking doctor. You are also very fast if you can do 600 lines in one hour, or else you have it macroed/expanded to death and there is very little actual typing so that is why you can do so much in such a small amount of time.
I hate those but I hate trauma notes on children even more sm
I used to work for a large hospital that saw a lot of children for various horrible things.

The very last one I did was a 12yo boy who had hung himself while his mother was at work. She thought he was in school. The doctor cried, I cried (as I usually did) and I just couldn't handle that anymore. That was the longest report I swear. Short in lines but the length was almost an hour. The doc didn't pause the machine, she cried, horrible gut wrenching sobs, and I did too, right along with her.

Does anyone have any information about Fast Chart? I applied for a position there and am curious to know if it is a good company.

It could be better, but could be worse.  Pay is low, but there seems to be plenty of work and pay is on time.  They seem very friendly and tend to leave you alone to do your job.


QA may or may not be "friendly" but do not tell the QA anything that you don't want repeated to the owner.  Pay attention to details and always confirm, in writing by email or otherwise, your understanding of anything important that is discussed in a phone call, especially with regards to the account assignment, pay and contract.  For instance, you may be told that an account with XXX specialty is not available and then find them advertising within just a few days for that same specialty.


They seem to be a bit disorganized, with account specs being rather vague and incomplete.


They are very flexible and allow you to determine your own schedule monthly; however, they do require weekend work.


IMHO, Fast Chart is a pretty good company compared to some of the other choices in the industry right now.

I have been at Fast Chart for a little over 2 years.  I did start off at 6 cent typing 3 cent editing and worked my way up quickly.  They pay headers, footers and spaces.  The accounts are easy and the people are always available to answer questions.  I know they do not send anything to india.  The owner is adamant about keeping work in the US.  The edit script platform I am on is so user friendly.  I am so glad to have a job with consistent work and receive my money on time.  I say go for it.

Actually, I understood it a little differently.  Fast Chart seems to have two fees.  The $7.50 per pay period is if you do not commit to at least 13,000 lines per pay period.  There's another larger fee if you do not reach your promised line count, based on the percent of your promised lines that you DO produce.


As an IC, wouldn't these fees be somehow tax deductible?

Yep.  And a friend of mine told me that she was chastised by the company President for communicating too much with the QA and other administrators by email.  She asked too many questions.  I guess they prefer her to figure it all out by herself and not bother them.

.09 cents for a 65-character line = $138.46 per 100,000 characters typed, and .10 for a 75-character line = $133.33 per 100,000 characters.

So .09 and 65 characters is the better deal, benefits are icing on the cake.
Does I-chart have a good expander?
Anyone familiar with Dictaphone I-chart.  I'm thinking of working for a company that uses this.  Is it good for productivity.  I have Word 2000.  Is it compatible?  Thanks in advance!
Looking for conversion/comparison chart sm

I'm getting paid gross lines right now and my company is going to be using new software which will use 65-char lines.  I'm looking for some sort of chart that shows the difference in $.

Question on transcription by chart...

I have a doc I have been typing for for years.  He HATES dictating lol  Rt now he has stacks of patients on his desk and I am getting ready to go on vacation, but there is no way he will dictate before I leave.  These would just be short orthopedic FUs,  Im wondering if any of you have gone by the chart or the diagnosis and the prior note and typed a note for the patient.  Does this make sense?  lol  Anyway any suggestions would be great.

their book. In fact, I love the BOS2. It is not a regular grammar book at all - there are hundreds of issues in it that are point specific to MTing and medical field. I have dozens of regular grammar books, too, for other purposes, and they are not similar at all. I purchased the BOS2 when out job interviewing for a few months. Every single employer wanted me to have my own company, and they all tested off of it. Several used it in their daily work, and I have found it priceless over all. I think its even on sale now, but $89 is not a lot for a professional tool that we should use if required, or if a newbie. MTs who don't like to buy books are not very wise in the big picture. Its like shooting yourself in the foot.

Then you absolutely must stay where USA has jurisdiction and that is absolutely not in a foreign country.  Those are the rules and laws and everyone knows that up front.  The troops, the FBI, the CIA, the US police do not help you when you get drunk and/or lost in a foreign country.     But you know that don't you?

If you are a good MT, which most of us are, and all of a sudden you start getting a bunch of crap.  I was very happy to be with a company that afforded you the luxury of being an MT, but yet a "hospital employee".  This was a very innovative thing and I jumped on the band wagon.  All the money is in ops, surgeons, they are predictable, macro-friendly, organized, don't flip through the charts and make me the most money.  After being with this company for over 3 months, at my skill level, and only to get cued 10 ops, most of which were rejected and cherry picked, that is a HUGE sign.  I did not tell "them" that is why I quit, they need to figure it out for themselves.  I personally liked the team leader, but she is obviously pooling work out to her favorites, and not giving new MTs a chance - hence they will still "be with the original team", not realizing that people unfortunately get sick, die, move on, and the "original team" will not last forever.  I am so glad to be out of there.  Don't know if new company is going to be any better, have to wait and see.

We do all make choices, but in some instances there are very few choices.  Yes, we should first avail ourselves of education.  Basic education is available to everyone in this country.  After high school, education becomes harder to obtain, but it is available to everyone.  You do have to have either transportation or a computer, and you have to have time to participate, but if you want it bad enough you can find a way.  All that being said, if the company you work for suddenly closes, and there are few jobs available where you live, you can find yourself in a bad situation that you did not create.  I think government assistance is important for people that find themselves in a temporary bad situation, but should not be allowed to be a way of life, except in the case of disability.

It is not your chair, keyboard, et cetera causing you to be so uncomfortable...getting up at least every 2 hours and walking the floors for 5 minutes works wonders in rejuvenating the body back to comfort level. It works for me and I didn't have to spend any more on equipment, hmmmm, not sure about the carpet yet though...LOL

I'm not able to use Express Scribe due to company policy against outside players. The player is the one that is built in to InScribe. The sound quality is excellent, but there's no way to access the player that I can see, except as mentioned above, and those options have been turned off.

if an injection is needed into the arm, for influenza, tetanus, whatever, that could cause soreness, they might look to see which is dominant and inject into the opposite.

If you can't make enough money to even survive as a speech rec editor, then you should choose option C. That is the point.

But, the big nationals, or should I say big globals, are not the only game in town, and if you choose option A and learn how to edit efficiently and improve your production then good for you. Editing and straight transcription require different skill sets some folks just simply cannot make the transition.
