Obviously in an H&P or an ER report, the fact of the rape would be essential knowledge for the physicians and health care workers.
But there are VERY few x-rays where the specific fact that the patient was raped rather than simply assaulted would need to be noted in the history.
I normally transcribe verbatim and even when there is an error I will make a notation for the doctor rather than change anything.
In this case, my lead happened to call on another matter as I was transcribing this report so I discussed my concerns with her. She works more directly with the doctors than I do so I followed her advice in the matter.
