Have you noticed that practically all the job postings these days say experience with ESLs. It seems

like without good ESL experience anymore you wont even get a job. I think it is really heading that way. I do ESLs but not a lot so I need to get more experience doing that and take a big cut in pay to learn it but seems like it is almost a necessity.

nowadays, when saying EMR 'point and click' is understood, AVR is automated voice recognition and then there is standard transcription.

When O says 'EMR', he means '

how many lines you actually worked and what you should be paid. I QUIT! If I have to be payroll person, I'll do it with an IC account. Sheesh!!!!
That's not the standard rate. I'd keep that job. I've had to beg practically to get 4.5 cpl
might get 5 cpl as a IC but that's about it. If there are more companies out there paying over that, sign me up and give me names LOL
I tell a complete stranger my whole life history practically.
I'm suddenly realizing that I have isolated myself and that I have no real friends and the only adult conversation I have is with my DH.  While I love by DH dearly and he is my bestest friend, sometimes you need other conversations/relationships. 
Had a resident who practically giggled through his entire dictation....
because his psych patient kept asking him if he wanted to see her underwear (she was in her 70s). I didn't find the woman's comment so funny, but this doc had the weirdest laugh...it was like Ernie from Bert & Ernie on Sesame Street. Weird.
BTDT. I worked for a large national. I had to practically get down
on my knees and beg for a secondary.  Got one, had even less work than my primary.  I went to upper management and got a third account, even less work that primary again, so 3 accounts, and still not getting in but about 600 lines/day. 
Yea, and you ought to try it in a snow storm. Practically white out and they're roaring

I have experienced all of the postings
below and they are my pet peeves too, but I have one doc that sometimes will not use the call in system, but uses his tape recorder to dictate and he does it while he is in a train station waiting for train, riding on the train, and leaving the train with his wife and kids talking right beside him, not to mention the noise from the train station, whistles and all. I get two 30 minute tapes a week of this guy and I seriously have a headache when I am done listening to it. Wish he would STAY HOME!! lol

I read these postings quite often and am
a Transcriptionist of many, many years but from what I read many newbies come on here stating out of school and unable to find a job and asking how to go about it. Jobs are steady being sent overseas and I have seen companies state there is lack of qualified transcriptionists here. I see others complain about lack of work (which is happening to me on this particular day also). I did not feel qualified after a year of hospital inhouse training to go home and transcribe by myself. I think the majority of people are not trained efficiently for years and years. I do not think newbies would be qualified for VR work. I find it difficult after all the years I have behind me trying to still figure a medication out, just different aspects of it. I suppose a newbie could poke along and eventually make it but I think most companies would not want to put up with their training on the job.
It's funny that these postings would come up
right now because I've been considering nursing myself - I regret having not done something about it years ago, and I'll be 50 in August.  Everyone keeps telling me it's not too late - I'm trying to believe that.  I think I would be a good nurse.  I've been an MT for over 20 years and I just can't stop thinking about what "could have been."  My 24-year-old daughter is considering nursing and we have even talked about going to school together. 
Paid per minute postings
Something rarely seen.  Paid per minute of dictation.  There are two rad jobs listed on MT Jobs board.  I have always gotten paid by minute by my primary account.  After working part-time for a few nationals this past year, I have learned getting paid per minute for radiology is not really worth it.  For normal reports you have to process the report for pennies which in some systems (such as Meditech) takes time going through the different screens.  For anyone applying for these positions, I would be interested in learning how much they are paying per minute.

Are you talking about a long distance service you need to use a C-phone?  Or are they questioning the use of your DSL connection?
What is with all the obvious spam postings today?

I do not think think her postings are 'preachy'. They are just matter of fact...
without the input of 'unnecessary' emotions.
Recent postings about "no work and so forth"

Quite a few posters who have been MTs for a number of years (myself included) have come under attack recently for comments posted here  -- what those of us who have done this for years are seeing is "I only want to work certain hours, won't do weekends, don't want to be on call so on and so forth"  Guess what?  I got an email from my supervisor on Monday asking me to cover a certain work type while that person was on vacation  -- I did it!!! -- She also called me today (Sunday and asked me to do 3 STATS  -- I cover STATS on Wednesday)  -- however, I did it because it it my JOB  -- since I started this job in October, I have NEVER run ouf of work  -- if the work type that I do runs out, I do to a differnt work type, it is similar to learning a new account which I have done in the past as well.  All I want to say, is be grateful right now that you have a job and if the "work does not fit YOUR FAMILY SCHEDULE" then learn to adjust  -- think PRIORITIES

Regarding previous postings about large kittie cats--sm
There was a thread of posts about a week ago, or a little longer, concerning large kittie cats and litter boxes.  There was one poster who had suggested using a large storage tub with a *door* cut into it and replacing it every six months.  I would like to thank that poster tremendously!!!  I have a large boy kittie who stands up to *P* and just sprayed every where.  My ex and I had built a makeshift kittie litter box with plastic sides glued to it, so the cat wouldn't spray the walls, etc.  My kittie would not go inside the covered litter boxes, either, so it was a mess.  The makeshift litter box helped alot, but I really liked the idea of the storage tub and went to Walmart last weekend.  My son said that my big kittie would not like it and would not use it, because my big boy kittie is afraid of EVERYthing.  Well, I cut the *door* in the side of it, filled it with litter, and about two hours later, my big boy kittie was sticking his head in the door, checking it out, and then using it!!  I was SO happy!  He LOVES it!  and so do I !  It was the best $7.00 I ever spent!  So, to the lady who suggested it.....THANK YOU SO MUCH!  If I could reach you, I would hug you!  
Next time I'll run my postings past you for approval then, okay, posting police?

Working 6/hour days, 5 days/week I make
$42,000.00, but the work is there to make more if I want to.  I'm in the southeast. 
2000 low days, 4000 busy days
Did 43,000 lines last month.   6 doctors. 
You can "make a living" if you work 16-hr days, 7 days
and if you rarely buy anything but food and the barest essentials in clothing. My balancing act is so precarious that all it'll take is one of life's little disasters (rent increase, sick pet, major car repair) to pull the rug out from under me. Not a good feeling at all.
Experience on top, current experience first. Education second. Leave out ALL fluff.
Recruiters don't need your life story. They need to know if you can do the job. If you want, put your current employer, then state "I have 20 years in the profession doing....." Keep it simple; keep it clean. If you want to go into more detail, do it during the interview. A HUGE red flag is to see that you've worked for 10 different companies, for months at a time. I know that someone who has worked for the same company for 2 years or more is going to have some degree of loyalty and will work through issues rather than cut and run.

that 2 of them are being XTRA XTRA careful when dictating risks and benefits (the plasticsmen) of late......being more lengthy about the risks and benefits....looks like they are CVTA (covering their a**es*) *lol*

I entered 3 entries into the Shorthand and expanded them and noticed that my autocorrect works as well.  I am able to use both of them together. Does anyone use both of these at the same time???

I ended up returning mine to the store. Love the layout of the keyboard, and all those features are great; they just need to work on getting a better touch to it. The MS natural was stiff too.

Have you noticed how they are down to 3 girls and 3 guys? Last year it came down to a guy/girl, year before that 2 girls, and the year before that 2 guys. I didn't watch the first year with Kelly Clarkson so I don't know who the other contestant was in the final 2 that year. Will the pattern start over this year?

On PAs - has anyone noticed (sm)

that more and more PAs are dictating for the physicians, and they are just awful!  They can't pronounce things, go 90 miles an hour, and I'm not impressed with their yawning, laughing, and various other things they do that are aggravating.  I am sick of them - even the ESLs are better than some of them.  JMO

The new *report message* button at the top of an opened message?  I just  noticed this when reading some rather snide responses to one of my posts earlier.  Hmmmm.....this might be a neat little feature.  Thanks again to the Admin. for looking out for the posters who like to frequent here without having to be harassed, polked fun of, and told to *get a life* when something does not meet with someone elses *approval*.

that when you are first hired by a company, they go on to state about all of these certain rules and requirements (the recruiter tells you the same) such as schedule, how long you should be on QA, if you do not do these by a certain time you could be termiated, etc.  As soon as you start working, it is totally different.  They are much more laid back and none of those things they said are really applied.  I am glad that they are more laid back than what you are expecting, but this is just something I have noticed now with 2 companies.  Have any of you noticed this?

Things like not wanting to go out for groceries until it was absolutely necessary or if a friend wanted to go out and do things with me.  I just figured it had to do with exhaustion/depression.  s

the only thing they did for me was sell my name and address to offshore companies, who in turn flooded me with unwanted solicitation. I asked that my name be deleted from mailing list several times. I was told this was their "main source of revenue." Hmph.
Here is what I have noticed on
When I first started on ES I had one nice steady account. I worked on it with one other MT. The facility was new to the platform, and I guess it was kind of a first run trial thing because it was just a few dictators. Once they all got on speech rec, it was awesome. The program even learned the basic formatting, would add appropriate headings, etc. (The line counts were great). There was very little editing needed.

Then they started adding dictators. Still no problem until it got to be too much for just the 2 of us. Each time a new MT started on the account, we could tell. It wasn't that they weren't necessarily doing things incorrectly, just differently. The speech rec program would start formatting things differently because it was learning from the new MTs too. Then it got bad. Things like date format or section formatting, which were client preferences and had to be done a certain way, suddenly had to be constantly fixed. And the speech rec itself seemed to go downhill and not recognize everything it used to.

In short, I think it depends on how many MTs work on the account, how strictly they adhere to client preference, and what their skill level is.




I would rather do some ESLs that some of the
sloppy southern drawls that you cannot ever understand. Wish those people would learn that not everyone is in love with a southerner.

What is an ESL ..... and I agree with you completely about the time spent on GRAMMAR!!!!

They can take the smallest so-called "errors" and put a black check by your name. We are medical transcriptionists, NOT English teaches.

Any GOOD MT has equally good grammar. Plus there should be less emphasis on grammar and more emphasis on the medical terminology. Do you REALLY think the doctor cares if a comma was put in or left out? NOT !!!!

I wasted a lot of time agonizing over GRAMMAR when I could have been getting in more lines and doing what is best for the patient .... accurate medical terminology.
Bad ESLs

Wouldn't it make sense for companies with bad ESLs, to hire US MTs who speak the dictator's mother language to do the transcription?

If it makes you feel any better, I once worked part-time for a doctor in his office. I wanted to get back into a hospital setting so I gave him my notice. He couldn't understand what I loved about being an MT. He said, "At least I have the benefit of reading their lips." AND I was once transcribing for this absolutely miserable ESL one day, pretty much the way you described, and waiting for him to hang up. Instead he was chatting with someone when I heard "I don't know how they do it, but I give them a lot of credit." I had to rewind to make sure he was talking about transcription and he was. Not all of them are that sweet. I once had to show an English book and the BOS to an extremely arrogant ESL oncologist. I hope you had a little chuckle here and feel better!

That's why AAMT is no longer a valid entity for us. The standards have been lowered to accommodate the ESLS because they can't make a decent sentence and don't know how to convey what they did or did not do to the patient. How the hell are we supposed to read their minds???? Those who can speak English and demand verbatim get what they deserve. Just the other day, the doc gave a female who had a vasectomy. He was a verbatim ESL and he got exactly what he dictated. When enough of them get sued or reprimanded, maybe standards will return, but I have no doubt in my mind that ESLs have lowered the standards at the hospitals and that's why we no longer change the dictation to make it sound like they at least passed English class. When I started it was not verbatim. We edited to change context and not content. Hospitals should make it clear they either speak clearly, or write clearly, and failing both of those, they should hire a PA or someone who can dictate like it should be, and some of the PA's aren't much better.

Had it up to here with ESLs....
I used to be one of those conscientious MTs who went back and re-listened sometimes 2 and 3 times, because I hate to send blanks to QA. After this morning, NO MORE !!!! This particular ESL doesn't even say the complete word, only the first syllable which is mumbled and scrunched into the first syllable of the next word. From this moment on, I go through it once, proof it and off it goes. If it's shot full of holes, then so be it!!! I'm telling ya, Wal-Mart is looking better and better. UGH !!!!!
