Have made a decision that
when dictation is too fast to decipher and doc is an ESL, will leave blanks without hesitation. I am responsible for accurately transcribing what I hear. I will no longer take responsibility for dictation of that which cannot be dictated. If they ask why so many blanks, I will tell them.
I am trancribing for a new doctor. No sample notes. NO IDEA WHAT HE IS SAYING. But, will no longer make it my responsibility.
Guess I have snapped!

Well I got told today right before I left work that my transcription i do for my department is being sent away anyways. That pretty much made my decision for me. I am getting a lot of support and even from my boss right now. I am going to go ahead and take the at home job and use my left over vacation that i get once I quit and save that for my leave when i have the baby. I feel happy yet scared but i just can't work there anymore. My whole reason for having that job was to do the transcription and it has just dwindled for the past few years and now its going to be gone!!! Most of it is going to this company i am going to be working for. hey maybe I will get my own transcription back that i am losing!!! Thanks for listening and please keep giving me your take on things!

And I am happy with what I have realized. I realized that what makes me nervous is the seeming unwillingness of my present situation as an IC where this company (perhaps all companies are this way when you are an IC) will actually make sure to keep me working. Their goal seems TAT which of course is to keep the client happy. That means they want very little jobs on the system at any given time, and they will hire as many MTs to keep that true around the clock. At least that is my take on the situation.
So, I made the decision to be an employee at a company which is more of a mom and pop type company. They are hiring on for 2 new accounts and have the full time amount of work slated for each MT they hire, straight transcription. I will keep the IC on the side for now, to make sure this company as an employee works out, but since the IC company does not care how much work they have for me, I have little worry how much I do for them. So, there will be no pressure on my conscience. If they don't bug me, I will do as little or as much as I want on the side and make sure to keep the employee job happy (which has incentives and bonuses). Either way, I guess what I was seeking was a more stable situation. I know I won't find the exact answer in this economy, you are all correct, but I do believe my worry was about mistrusting the company where I am an IC, and even if they were the best company in the world, they probably are not a good match for me if I do not trust them. Thanks for reading this long post. And a million thanks for your advice and suggestions.
I've made a decision to leave MT...
And to pursue my RN degree...I've had quite enough of being treated as a 2nd class worker...enough!!! I'm tired of paying for xxxx doctor's mistakes. I'm tired of being "talked" to by "supervisors and QA" who know even less than I do. And basically, I'm tired of making squat for wages while I work my fingers to the bone...
So, anyone done this transition before? I'm sure there are some out there who have and have been quite successful at it. If so, I'd love some words of encouragement. Unfortunately, I'm stuck doing this junk until I'm done with school, which will probably be a few years......but hey, I've done it this long, what's a few more, huh?

If the best an in-house position can offer is $12 an hour, I might as well make $12 an hour and NOT cripple my hands any further. I will just type part time on the side. Maybe that will pay the bills. This profession has gone down the tubes and I am getting out.

My husband doesn't want to work. Never has and probably never will and it took me 16 years to figure out that nothing was ever going to change. He has been in and out of college over the years with a number of different majors and when he is on the verge of actually finishing something, he up and quits, because I believe he doesn't want to actually go out and get a job. He loves the going to school part, the studying, etc., but the actual finishing and putting his education to work, that's too overwhelming.
For years I made excuses for him, supported him, and actually believed that as he got older he would mature and finally find his way. He's 42 now and runs off every day with one of his unemployed friends to play frisbee golf or go hiking or water skiing or fishing while I'm sitting at home at my desk typing until my nose bleeds!
I was one of those women afraid of being by myself. I recently had a major depressive snap where I cried nonstop for three weeks. I found a great doctor who became a great friend and she helped me find the right medicine to get my head clear and then she helped me realize that I don't have to be afraid of anything. I was already supporting myself and my kids without anyone elses' help. What was so frightening about leaving my husband behind?
So quietly I made my plans to leave. I let my husband know that I was going to move back home to the area where all my family still lives when school let out this summer. He didn't believe me because I had said it all before. So I just made sure he knew I was thinking about it again. Then May came along and miracously a job opened up at the hospital in the small community I wanted to move back to. This hospital never has openings because they virtually have no employee turnover whatsoever. Everyone's been there at least 5 or more years. I took it as a sign and made my move. Within two days of submitting my resume, I had an interview set up. Without even waiting to see if I had the job, I packed up my kids and our stuff and told my husband "I'm going. I love you, but if you want to be with me you need to get a job and actually be my partner not one of my kids." And I left.
I got the job, thankfully. I've been here a month and I know without a doubt that it was the best thing I've done in a long time for me and my kids. My teenage son had built up a lot of resentment towards his father and their was a lot of anger and tension between the two of them before we moved. Now, my son is happier than I've seen him in years. He's more social, made a lot of friends, and even has a girlfriend now. My daughter misses her dad, but she has always been remarkably wise for her young age and is very open about saying how calm everything is up here.
So there's my story. I would never encourage anyone to divorce or leave their husband, and I haven't actually taken the step to file for divorce yet myself. But sometimes separation is liberating and therapeutic. It has been for me. I can't remember the last time I felt so peaceful.
I Do know what I'm talking about...........my point is that
I have done it and PERSONALLY it was not a smart decision because it causes you to have to pay a lot more out of pocket taxes! I don't care how you word it or what you call it. Obviously, I have ran into one of those posters who are very unhappy with their life and come to this board to have someone to argue with and put down to make theirself feel better. By the way, duh is something that I heard from children under 10 and actually haven't even heard it from that age group in about 5 years - grow up and get a life. If you choose to work IC status then that is your choice. I was just simply stating my personal experience and why I agreed with the first poster that it isn't worth it for my time.
No you don't know what you're talking about. The point is
no matter what you call it or how you slice it, the only difference in tax payment between an IC and an employee is the 7.5% self employment taxes and also, less the deductible expenses. I don't know who does your taxes but I bet it isn't you because you don't know the first thing about them. It only costs 7.5% more to be IC status. period.

glaring mistake, complaining about people making mistakes when they should not. And honestly now, I know 100% that you kicked yourself when you saw your huge error. Nice to play the oh this is an informal board game, but honestly, you lost credibility when you botched your own post. When I was a waitress during college and had to stay for a dead shift, I think I remember something about getting paid minimum wage if I had no sales/no tips. We normally made $2.13/hr as the base pay so without any sales we were below minimum.

I'm glad you brought that up. You are absolutely right. What a major slap in the face to the city. So here's what these brilliant show people did: not only did they NOT use local talent, they didn't even use AMERICAN talent, and they didn't even use YOUNG American talent . . . they used an OFFSHORE DECREPIT old name. (Can't even use the word "talent.") I should clarify about "young," because had they used Smokey or Aretha well they are not exactly teenyboppers but they are COOL AND AMERICAN AND WAAAAAAAAAAY TALENTED! They are giants and legends and Mickey could not hold a candle to anything Motown had to offer. YOU MADE AN EXCELLENT POINT!!!

However, the doctor should be aware of what the dictations contain before they go onto a chart, right? This is so scary.
Another thing that's just wrong is the fact that these administrators are dictating to the docs how medical records should be managed and many of them are not competent.

That is very true. I didn't think of that as I have never done these types of reports before. I can only imagine how frustrating it must be. I do regular VR editing and even then it is impossible sometimes to hear what the doctor is saying and he is at least TRYING to talk clear (if he is a decent dictator). I can only imagine trying to listen to a bunch of mush-mouths talking all at once. :)

Personally, I have lost a few accounts to India. The funny thing is that I was typing for a large cardiology office who decided to go to India except for one doc, who is from India. He saw firsthand how many mistakes were on the reports, and decided to fight for me to continue doing his work. I try to explain to the offices that regardless of the "cheaper" rates, how cheap is it, really? I mean, by the time they have to go through and edit the report (thoroughly), present it back to the doc for his review, and then get it back to India to correct the numerous mistakes; what's it really worth?

Isn't that both amusing and interesting? In all my years in the MT field, I've only found a few physicians who actually care about their transcription and where it goes with many stating "we'll just have to like it." I guess when it comes to a hospital or clinic purchasing new MRI or CT scanners transcription just takes the last back seat in the yearly budget. It's always been about money and that will never change. With each year that passes, I only hope that someday we will all hear of a reverse trend and India will have to re-outsource our work back to its "country of origin!"

I found it difficult to try to include all types of transcription. Per project transcription is so much different than regular or medical transcription. I will change the 50% fee up front to state that it is for per project transcription (not for medical). I do not want to accept a transcription project from someone I am not familiar with without covering myself with getting at least 50% of it up front. For that type of transcription I do not think it is an unreasonable request.
I've also tweaked it using suggestions from the other MTs here. I added no job too big or too small, took out some of the petty charges to make that simpler, added a couple of pics (will tweak that more later), and included a paragraph about my rates as compared to other companies who may send work overseas. I lowered my rates 1 cpl and added that they were negotiable, but generally I think that what I am asking is fair and I do not want to lower my rate, as I want to stress quality and would rather have a few accounts paying that rate and be able to handle them well than be overwhelmed with cheap paying accounts. I feel like if more MTs stuck with charging what they are truly worth, it would help all MTs and the MT profession, so I will do this as best I can. I also did change it to a standard 24-hour TAT. For that I had been thinking of projects possibly taking longer, but I could put that on the estimate if need be.

I was there once. Where I lived at the time was a small town. Those commercials make it seem like it is so easy to get work as an MT. Not 15 years ago in a town with 1 small hospital and maybe 6 docs it wasn't.

The Smartype Expander doesn't work in Bayscribe, so I type in Word and then cut and paste. Plus, if you have all your stuff, say in a Bayscribe expander, what happens when you leave that job and go onto another one. Do you lose your shortcuts? How does that part work?

That was on MedRite when I was there, but do they charge computer rental with their new platform (DQS?)? If they don't get that rental money anymore, they might be eliminating SE's. Just a thought.

The entertainment value is what I question. Who is being entertained? I'm mildly amused by the ridiculous nature of the shows and the strange ideas they come up with, but I don't live for them. I will admit, I do like Survivor. Don't they lose their punch when the people come back and tell what "really" happened? I hate that.

you would probably be better off just buying a new computer. Many offer their credit card and 6 months no interest. That is how I bought my Dell 2 years ago, paid it off in 6 months, no interest. I love those deals, we buy all our major purchases that way, have never paid interest yet.

I was wondering if maybe some of the work is actually being sent overseas for cheaper labor for a few days or so to help save the company money in this ailing economy. Have known this to happen with a couple other companies. But, as you said, if something major were to happen, there would be an MT influx into other companies and maybe back into the hospitals. I have mixed feelings on this...it's nice to work at home, but hospitals usually have decent/reasonably priced benefits that do not depend on line output.

Good grief. The point is that it
isn't 'whining' to state that MTs have to have much experience & knowledge, and there are a lot of physical effects and mental strain, and considering all of that, the pay is really lacking in our profession. Those are the facts. An IC at 8 cpl is AWFUL pay, by the way. Just because you don't think you deserve any better than that doesn't mean the rest of us feel that way.

Sometimes we would get compliments back then. I remember the first place I ever worked at. We had a neurosurgeon who was known to be horrible (and was). We kept samples in order to be able to do his dictation. I came in one day and there was a handwritten note on my typewriter from this doctor for a job well-done! I still have it all these years later. Today it seems like the "human factor" is gone from this profession and it has all become about production and money and the patients have been forgotten!

I guess I have to figure out what is most feasible for me. Luckily, I have had the same transcriptionists with me for a long time and they are all quality workers, but I still like to quickly check all reports before they go to the client.

I knew they made good money...
I used to work for Kaiser in California. I really wanted to get on with the transcription department but there was NEVER an opening. I really miss working for Kaiser. I have not been able to find a hospital like that since I left. I now live in GA. The Kaiser here is not the same -- much smaller operation here. But I knew it, I knew it!
