I would like to offer database management to my potential clients. Any notions on what to charge for this? I was thinking of an initial charge to set this up, and if I do their transcription no charge to maintain.
Also want to offer form letter creation and mailing. Has anyone ever done this and what did you charge? Any input is appreciated.

In my experience, certainly your QA may vary and/or be inconsistent all-on-its-own ... but most of these truncated forms, fit into what I was taught was called "imperative case" and follow up is being used as a VERB ...

Searchable database
There's a product called ScriptXSearch, new on the market, that will allow you to easily provide your clients a searchable database. You can find it at www.scriptxsearch.com, or write to rob@reynet.net--he's developing it specifically for ICs and small MTSOs.
The EHR is coming. Physicians offices are going to be the last to move to the EHR, I think, but I'll bet more money will go that way now that the Democrats are in power. Bush proposed it but never really funded it, so it's kind of stalled. Except that it's such a good idea that it's picking up its own momentum.
My thought is that if I offer doctors these small pieces of the medical record maybe they'll take us along in the EHR. A lot of vendors are advertising that their EHR is going to replace transcription. I don't really think that's going to happen, but it may be pretty lean there for a while.
We can't go too far, though, HIPAA says something about if you maintain medical records you have to make them available to the patient and you SURE don't want to get caught up in that. I'm a small serivce and I offer Rob's ScriptXSearch. AAMT had provided a HIPAA agreement that included a clause explaining that we don't maintain medical records, these are preliminary reports.
I do not think they had a database the doctors could search.
I think they just store the reports in a way that can be retrieved. As a subcontractor, in general I keep the files only until I am paid for them. When I complete a file, I send the reports to the doctor and to the MTSO. Some large companies do provide an electronic health record, but I doubt it would be feasible for an IC to provide this. I would say this is one of the areas we cannot compete with the big guys.
Meditech is a hospital database. sm
It allows everyone in the hospital and also physician offices remotely to view patients' medical records. There are upgrades but they are costly. It is not transcription friendly at all. I worked on it for years, and so glad that was on an hourly basis. You are better off transcribing in Word and cutting and pasting into Meditech. It is not user friendly. You cannot download it. The hospital or MTSO must give you a password and you either access it by VPN through the MTSO or directly into the hospital's server. When it goes down, you can lose reports. There is a recover feature, but it doesn't work all the time. Just be sure to save reports midstream. One more thing, there are a lot of screens and some require data be filled in before you actually get to transcribe. JMO. I'm sure I'll get flamed but I've been through upgrade after upgrade and you wind up losing features and having to put canned text back in, which can only be done by the Meditech people. NOT USER FRIENDLY at all in my opinion. It also lags behind your typing. You would be better off finding work that you can type in Word.
Patient Database in MS Word?
We released MPLite less than a month ago, and most of the emails we are getting speak of how having a patient database is saving so much time. Another feature that many emails comment on is how the invoices give the details (patient, Job#, MR, dictator, MT, DOS, Doc Type and count).

escritption database errors
It seems lately that I'm always getting database errors, either while pulling up updated physicians lists or problems with the queue. Sometimes it won't even get past the login screen and requires a reboot of computer because editscript won't shut all the way down, even though there are no indications of it running on my task manager, so I can't restart it without a reboot. At times it is only one account and I can login to another, others it is any account I try to login and pull up queue. At times this decreases my work time by hours. I've redone my whole computer hoping this would resolve the issue if it were my computer. From what I can tell there is no difference between now and whe Escript seemed to be working perfectly well, at least on my end.
Anybody else with this problem or any ideas?
...should be new meds, colleagues not in the database, etc. Damn! nm

This web site has the listings for lots of major companies that we deal with every day. It includes the numbers to actually speak to a human being and not a computer. My daughter forwarded it to me, but I haven't actually had need to use it yet. Thought it might take away a frown or make someone's day a little better.

I wanted to know from anyone who works at Focus if you are able to download work when you want to or is it assigned to you? I thought I heard that you can go onto the site and grab dictations when you wanted to instead of being assigned.

How is keeping a patient database on your personal computer HIPAA compliant?
I would love an effortless line counting program too, but...
Just curious if anyone has investigated that issue with this software.

I was SE for most of the year and employee for 2 different companies last 2 months of year..I also am now part-time emp. with a co. and trying to start up my own business which I started up in Dec. and want to take deductions for equipment purchased for that business. Oh my head is hurting.

request to fill out on my own, or is it usually requested by the IRS? Will the company I contract through know that I have requested this form? I, too, am an IC who is treated like an employee. I have worked a set schedule (days and hours), including nights and both weekend days, for a long time with little or no flexibility. The pay is good, though. TIA

It probably means to just type the dictation and make new paragraphs as needed, but to just keep going down and make no headings unless specified.
I think that any form of indiscretion with someone
else would be considered being unfaithful to your spouse. That includes talking on the phone or in person when your spouse is not aware of it, holding hands, kissing, or "going all the way." Anytime something is done with another person and we feel it necessary to keep it from our spouse, there's something going on that doesn't need to be. This, of course, is just my opinion, but I hope it helps.

Seriously, though, just put in your contract you don't do toilets, the same as some don't do windows. I bet there are people who would rather get other things done, and would be willing to do that themselves, especially if the price was just a tad bit lower.

You said you use your den "almost" exclusively for business. Almost doesn't count. The room has to be used EXCLUSIVELY for business in order to be able to take business use of home.

can dump more money into the pockets of the Indians! Did you know that the government will give out $60,000 loans or aid to help Indians/Pakistanians help open up a business in this country!! I couldn't get a business loan for $25,000 and I'm from this country!! Born and raised!! There was a grocery store in our town that was for sale and no one in town could afford to buy or get a loan for it, then out of nowhere, this Indian family moves to town (lily white community, by the way) and bought the store. They told the store patrons that the government gave them the money to open up a business. Can you believe that???!!!

Just put your complaint in writing and send by fax, mail or hand deliver to the Patient Care Representative at the hospital.
The PC rep at the hospital I work for investigates all complaints from beginning to end and will make a follow-up call back to the patient or patient family regarding the issue if further comment is wanted.
I've typed these reports for admin and can be very interesting to say the least.

for healthcare professionals. It has a circulation. It is readily available in schools. All media matters because what is in the media influences the way people think. I think it gives a negative view of us to those who call the shots over us who read it. That's why I think it matters.
Would you capitalize the name of a form the doc's use?

However, the BOS also says to spell out nonspecific numeric expressions, i.e.
"She described hundreds of symptoms. (Not 100s)"

Well on the form in the book from the DMV it is listed and the doctor is required to check. By the way my daugther has a lady doctor, not a man.
We all need to JUST SAY NO to VR in any way, shape, or form
Having to edit some of this garbage is more time consuming than typing the whole report not to mention the looking stuff up and verification of names, etc. It's all a big RIP-OFF.
Regarding letters
We have an account with our national where all the consults are dictated in letter format. So for every single consult we get we have to look up the correct names, spelling, and addresses for the referring physicians because the dictators never give them. Once we get them we can save them in autotext, but it's still a pain to have to look them all up.

some of us CAN'T do much else at this point. I can't even write anymore!! And we blessed, fortunate ICs don't qualify for disability pay or unemployment... so yes, it's upsides and downsides, but in the end, right now I'm facing having to quit MT and I have NO idea what to do for a living, at the tender age of 36, and, with the pay cuts and such, no money to invest in an education. So please try to have some compassion... not all bellyaching is unfounded.

The written word can come back to bite you. Be careful what you say. If they care enough, they'll ask and then you can verbalize. Otherwise, it will become part of your permanent record. Don't say anything to make them angry, One never knows down the road when you will have to list them in your past employment history. Sometimes we would like to pour our hearts out but it would depend on what you have to say. I once wrote a note which was not really that bad but everyone in the company said I left a scathing letter which was totally untrue. The less you put in writing, the better, IMHO.

Use a name that is 4 letters or more and you get the... sm
AutoComplete tip box showing your entry, then you can press F3 or enter to expand it. Make sure the first 4 letters of the name are not the same as the first 4 in another entry or Word won't know which one you want and no tip will appear. You can use less than 4 letters in the name but no tip appears. You'd have to remember it and hit F3.
Just remember that AutoText only works in Word and you might want to think about a true text Expander that works in all of your programs. You can also bog down your normal.dot file if that is where you choose to save AutoText and you will be at high risk of corrupting the file. Make sure you keep a backup.

I have a test for a position and it says to do it in continuous note form, does that mean that they should be all in one document in Word or am I completely wrong on that? Someone please help soon! Thanks!

Are you testing for MT group? I took the test and just typed the patients one after another. I kept spaces between the patients with the patient's name.

In letter form for clinic?
If the clinic is consults in letter form, that slows me down, looking up all the docs and addresses. Not all clinic is like that. But that part may make a difference. I just quit a company cause of that and went back to acute care. I have also worked clinic that we didn't have it in letter form, and it was fine. Just depends on the account.
I would think it be depend on the type of form.

There are several documents accepted but if the employer wants the SS card and/or DL as those documents, they can ask specifically for those.
