As a rule, I don't count off for blanks.

However, if an op report has blank after blank, I know that person doesn't have a thorough knowledge of op reports.

If they leave blanks throughout a cardiology report at all of the more difficult cardiology terms, I know they don't know cardiology, etc.

But, if there's a blank here and there and I can tell the MT isn't willing to guess--I'm going to give them kudos for knowing when to not guess.

You want to know what the WORST thing is, though? When words are transcribed that don't exist. Can't even get a Google hit, much less find it in a book...or the one Google hit that comes up is found in surgicalresidentslounge.com.

Make up words on a test that you send to me, and you're not going one step further down the road to working with my company. That's one area I'm pretty hard line about.
