I have worked on several different hospital contracts helping the facility transition to EMR. The EMR as "O" is talking about is the computerization of the medical record vs. the paper chart. When we transition a facility we have to scan all paper records into the computer. Each record is given a line in the EMR for that particular sheet of paper. Those lines are then assigned the proper numbers (facility specific) for the type of record it is. The record is then coded and analyzed for completeness. Any deficiencies are flagged for the specific doctor (depending again on the facilities protocol). The deficiency then goes into that doctor's que to be corrected,dictated,signed-off. The doctors are counsled by the head of their department ie: Surgery, Neuro, OB/GYN, etc. for deficiencies that are not cleared in a timely manner. When a doctor has too many deficiencies in his que someone will go in and re-analyze them to make sure that they have not been corrected and have not been cleared out of his que. JCAHO will look at this information when they are doing accrediation. This is what "O" wants, a complete MR on every patient that is available electronically, instead of paper charts in every facility a patient has ever been to. So if a patient goes into the ER all records or all past medical care can be accessed immediately. It has nothing to do with VR. I have yet to see a point and click system that can duplicate a complex surgical procedure dictation or a complication, that technology is just not there.
