I saw it for the first time at a local clinic. My daughter had an appt, the nurse took her back as usual, took history, vitals, all the norm. When she got done, she went over to a computer, typed in about 2 sentences, and checked a few boxes, and poof she was done. No need for a Transcriptionist there at all. I was surprised. I had never seen it before that and could not believe how simple it was.
