Dave Ramsey (daveramsey.com) has a wonderful get out of debt program. He also does a radio show and seminars throughout the US. I highly recommend his program. He is a real motivator! While its a bit strict when you first start out, it does work!

I want to learn to type on a steno machine. Have you ever thought about doing closed caption stuff for TV shows and such? I thought it would be fun, but you have to use a steno machine so I was out.
You said you went to school for it...can you tell me about the program? How long was it, etc.? I'd appreciate any info.
Oh, and I wanted to say I work for MQ too and ASR slowed me down. My actual typing speed is only around 110, but my production rate was around 400/lph. So, needless to say, I've been losing money like crazy with 80 to 90% of my production now coming from ASR and that wonderful decreased rate.

The biggest thing I like is---
They are FLEXIBLE. They don't call you up if you are in the bathroom to see why you aren't working. As long as you do your work, they don't bother you or restrict you to a strict schedule. That was a big turn-off for me when I talked with Spheris.

One of my biggest pet peeves, since most of
MT is history.
After doing this for 10 years, 3+ in house, I'm on the brink of hanging it up and forgetting it. The pay keeps getting worse for the MTs (and better for the corporatists), the platforms keep getting slower and more cumbersome, the quality of the dictation does not get any better and tends to get worse overall as the old masters of English retire and more ESLs move in to replace them.
I would strongly advise anyone who is young enough to find something with a future.
CPL History
Just a thought here. It seems we're all so consumed with making lines and working faster and faster, has anyone stopped to wonder about how the 8-10 cpl rate was devised?? It seems that with the amazing fund of knowledge we have to possess to function in this profession, I feel ridiculously underpaid and overworked. The companies are constantly begging for us to do extra work with no incentives - just the statement that "here's a chance to get extra lines". I realize that this is a production-based business, but COME ON!! Am I the only one who feels like the companies are profiting big-time from our skill and knowledge?? There's so much we have to know as well -- A&P, meds, extensive terminology, hardware, equipment, and on and on. Please tell me if I'm overlooking something obvious, because I'm getting really burned out. Thanks guys for listening.

As long as I get the work done, I can do it when I want to without being chained to my computer.
Plus, I don't know why you think ICs net 50% less than if they work on an employee basis. I've worked as both an IC and employee and, although I don't have benefits, I make much more as an IC.

You have to purchase the equipment and pay for installation. The dish weighs about 75 lbs so if you don't want that weight on your roof, you can have it put on a pole in your yard and that's extra. Plus they charge to bury the line from the pole to your house ($3.25 a foot - I buried it myself). All total for me, including first month's service over $700. But, hey, when that's your only option, what do you do? There may be a company or something where you live that 'rents' the equipment for a small extra monthly fee - that's how we do our satellite TV so we don't pay for repairs. Satellite internet is not as fast as DSL or cable, but is still much faster than dialup. Plus, if you're an IC, it's all a tax writeoff. Hughes Net customer service stinks big-time and if you don't like them taking the monthly charge directly out of your checking account, they charge an extra $5 a month to mail you a bill. They know you have no other high-speed options and they have you over a barrel - so they stick it to you every way they can.
But these are hospital reports from one of the biggest
ones here in Atlanta. At 1 time it was no name and change to the patient and now, all 4s, when the dictator dictates their name, you put the name. I know darn good and well they supposedly go along with the HIPPA or whatever the initials are because when it first came out they were given out and we had to read. These are definitely not clinic notes.

I volunteer for a local food back through United Methodist Open Door. They do not provide money for rent or anything, but they will give you some food and a referral to other agencies that provide assistance. They do ask for proof of income, but if you are experiencing a short-term situation, they will help you. They never turn anyone away hungry. I am sure there are such things in your area as well.

Your biggest problem will be with OP Notes.
I would buy the newest editions of Stedman's Surgical Word Book, Stedman's Medical Equipment Word Book, and Tessier's Surgical Word Book. Those three books will be your best friends in hospital transcription.
I would also invest in Stedman's Lab and Pathology Word Book too because you'll run across a lot of lab tests you've never heard of and would probably never hear about in the clinic setting.
How does one distinguish biggest issue
I've been at this a couple of years now and still too slow. Aside from my own issuse of looking things up, getting my Expanders accumulated, etc., I am wondering how to sort out some of the technical issues.
I hear a lot of talk about platforms, but I think the one I am using is ok (what excactly should I be looking for to determine that?). Also, I noticed the computer seems fairly slow in general. I use the Internet for searches, but it seems it takes longer than it should if I put a word into the dictionary search or acronym search, and so on. (I have some books but just as slow with them and need different glasses right now so have trouble reading them.)
I am certain we need more RAM, but it also seems our connection has slowed down as well. Connecting to the server takes longer than it used to and the e-mail opens up more slowly.
On top of all that, the data screen slows me down. There is always something that causes trouble, like, if they don't say a certain thing, then delete something else and check a box ... Or, they will enter a doc's name, but it needs to go in a different field, so we have to move it.
Any thoughts on these things and tips on how to work around it the best I can? Thanks a bunch!!
Nothing like making up history
There are in fact many Hispanics who died and continue to die for this country. I am the daughter of a WWII veteran whose mail was confiscated because he wrote letters to his mother in Spanish. While my dad was literate in both English and Spanish, his mother could only read in Spanish, as was the primary language of New Mexico, Colorado, etc. 50+ years before the Mayflower landed. Also, my brother is a Sgt.Major who retired after 25 years in the army and 3 of his sons are also currently in the army (including Iraq). But as I said before, you can ignore all that 'cause you make up your own history....and telling Native Americans to get over it only displays your ignorance.

His child is cruel toward animals, chances are this child is being mistreated in some way by someone. This is a red flag that something in not right in this child's life regarding the adults. Children are NOT naturally violent--it is learned behavior.
Be friendly, but takes things very, very slow and think of the safety of yourself, your children, and your pet (s). Better safe than sorry.

You bring up a great question. Personally, I don't think any of that is their business - it doesn't affect how they will educate their child. I would not answer any of that - just put family medical history unknown.
Where do you live? I have taught in 3 states and none of the school districts I taught in, or the private school where I worked, asked.
You know, in this country it is illegal to ask someone for their national origin - how can they get by with asking about such personal information as your family Hx?

My biggest problem is the babysitting requests...
This is my biggest problem.. Every mother within a 20-mile radius thinks I am their emergency backup babysitter. I lost a friend over this. She asked me to babysit, I said no, she showed up at my door anyway and begged me to take her kid, and I refused. She hasn't spoken to me since. Several other mothers have expected the same thing, some that I don't even really know, and I have had to be very firm and stand my ground.
The biggest mistake parents are making...
is the use of Pull-Ups. Ever since they came out with absorbent (and now humungous sized) disposable diapers and Pull-Ups, kids are training later and later. What discomfort do they have to endure wearing these wonderful dry feeling comfortable garments? No more Pull-Ups and no more diapers. Tell him that the law is that they won't sell diapers to people who don't have children under the age of 3. That takes care of the visual of diapers in the store.
Just go cold turkey. Put him in underwear (and not thick absorbent training pants either) and get ready to do a lot of laundry for a while. Take him out for some really cute underwear with the character of his choice on them, and just be positive. If he has an accident just say "uh oh, you forgot to go to the bathroom" and just hand him clean dry clothing to change into. He'll get REAL TIRED of all the changing after about 2 days. There is no way on earth that a child of 3-1/2 has no physical control over their toileting, unless there's something wrong with them physically. So it's just a matter of him taking the initiative. EMR - Just found out today that one of my biggest accounts has gone to EMR. SM
They have started using an EMR system this week and are phasing out of my service. This is a big chunk of my work. Guess this is a sign of what is coming. Wonder if it is just a matter of time before the rest of the accounts follow suit!!!

It simply should not be allowed. It is hard enough to understand a lot of these people, and then they sound like they're speaking from the bottom of a pit with the annoying mechanical, buzzing sound quality as they "push to talk". Not to mention all the extraneous noises on top of that -- drives me nuts!!

You want the history of how a line is defined?
I have a feeling you're going to need it.
Sheet of paper = 8-1/2 x 11 inches. Courier font = 10 Keystrokes per inch
Type in Courier font from the left edge to the right edge (no margins) and you'll get 85 keystrokes. Create 1" margins on left and right side and you deduct 10 keystrokes per side. Therefore, 85 keystrokes less 10 keystrokes for left margin and 10 keystrokes for right margin leaves you 65 keystrokes a 1" left margin to a 1" right margin. That is considered a "real line," as opposed to a gross line where "anything on a line is a line." It doesn't matter if you type a whole sentence or just word. If it's on a line, it's counted as a line.
When we started using computers, people switched from Courier font to other fonts, but Times Roman 12 pt was the favored. So, to calculate a line when it's not running from left margin to right margin (Times Roman 12 pt is a smaller font than Courier (see below), you simply follow the rule: 65 keystrokes equals a line. This way, it doesn't matter what font you use, you're rate of pay will be the same. So, if you're getting 6 cents a line for a 65 char line (presuming that includes spaces), you'll be paid the same no matter if your font is:

All of the lines are different lengths, but you will be paid the same no matter which font you use. You don't have to worry if the person who hires you wants you to use Gothic (huge font) and Mary Contrary to use Arial Narrow (very thin font). You will both be paid the same - 65 char/line.
You also need to do is find out if spaces are included. If spaces are included then every KEYSTROKE is counted. If not, then only what you actually see (the letters) are counted. So, be sure to ask if spaces are included.
But, as far as getting 6 cents for a 65-char line ... I'm going to presume you're new to the business (what they call a newbie). As such, 6 cents per line is decent. Just make sure you get raises over time.
However, if you have at least 2 years of experience doing acute care and they offered you 6 cents per line, you really should refuse the offer ... unless it's either that or the bread lines.

Even in these hard times, her professional skills and personal integrity are worth more than that. For heaven's sake get another job! There are still better jobs out there. If it were not for my extremely painful lower back, I'd find some PT work to do, just to keep my hand in, and even a retired MT could surely find a job with say, Medquist or some huge MTSO like that. I know what it's like to have one's professional self-esteem eroded and destroyed. Not worth it, not at all. Exit ASAP!
