Are headings capitalized or followed with a colon?

HISTORY OF THE PRESENT ILLNESS:
Is it p.r.n., prn, h.s. or bedtime?
Just your basic formatting and styles. I may just download the ebook. Thanks!

Basically, harder dictators and harder reports with a lot of formatting with tables, etc. SM
give you extra lines because of the time involved in doing those particular reports. Not so much you get paid more per line, you just get extra line credit. For example:
You have a 100 line report, but it is weighted because of a table that you have to input. Then you would maybe get credit for 130 lines for that report.
It's basically a type of Dictaphone
where you you dial into the hospital's dictating unit to pick up your dictation. It has to be programmed for your individual accounts.
Acute care work is operative reports, consultations, H&Ps, emergency room, DS basically the type of
dictation found in a hospital setting as opposed to a clinic setting in which you just type office notes and minor procedures.

Gosh, in the old days, I would have resorted to just putting this all under HEENT if you could. It really doesn't belong under ENT, everyone knows the eye does not belong here. Who knows anymore?
headings
If your company uses templates where headings such as HPI and PE are already there, you may not get credit for those character lines. I had this issue at a former company so that may be part of it. Just a thought!
Just like an H&P, but sometimes without headings, sometimes use the SOAP designations. nm

Autocorrect Bolded Headings
I spent quite a bit of time yesterday entering bolded headings into Autocorrect using the Formatted Text button rather than the Plain Text button. I go into WORD today and all my Formatted bolded entries are gone. What did I do wrong or what can I do to make sure these entries are saved? Would appreciate any help. Thank you.
Here's what I do for the numbering -sm
I got this from another board awhile ago and saved it. I assigned the hot key of ALT+; so I don't have to take my hands off. When I get to #2, I just do ALT and semicolon and the lists goes to the left margin. Here it is:
Record this simple macro and assign it to a hot key:
1. Type #1 and any text you want to, then press Enter.
2. Immediately after #2 appears in your list, start the macro recorder, press the up arrow key, then press Alt + Shift + left arrow and your list will demote to the left margin.
3. Press the down arrow key and your cursor is in place for the second line of text.
4. Stop recording.
When typing lists, run the macro when you get to #2.
numbering
to find how to do this in Word, just type in numbering where you ask for help. It gives a simple good explanation how to do it.
You just type 1 and tab or space over. Type in your text and enter. it should automatically come up. If it does not, then go to tools menu, click autoformat as you type, select automatic bulleted list or automatic numbered lists, Make sure the box is checked. To finish your list, ENTER twice or press backspace to delete the last bullet or number in the list. It works. I tried it to be sure.
Numbering system sm
This would not work well as some people come in way before their appointment and some show up 2 minutes before and have to get back to work, so the early bird would have the other one's number. What about the local pharmacy where they holler out your name and in my case, yelled, "We're almost out of Norvasc but will give you enough to get you over the next few days." Good thing it wasn't something more personal! This privacy stuff is a long road to hoe before it gets straightened out. Same thing with blood work, no privacy at all in getting your info into the computer, yell out your name, ask address, tel. #, age, it's disgusting. The whole room knows why you're getting blood or urine tested. Geez.
Numbering reply
You need to click on Format, then click on bullets and numnbering, then click on outline numbering and you can reset the numbers and alphabet letters any way you want to.
Word numbering
Hi: Try opening Word, Edit and then click on undo bullets and numbering to see if this may be a simple way to stop this. I have Windows XP and I just tried this and it worked. Good luck.
HEADERS....as in what? The things like HISTORY AND PHYSICAL headings?
or the heading at the top of the page that we don't type?
and do you find it a little offputting that they can't put in writing that they pay for spaces? it makes me think all is not kosher.
Enumerated lists and underlining for headings in exam
I was wondering if any of you can help me, and possibly even give me a reference, regarding enumerated lists, specifially in an exam. Example is:
1. General Comments:
2. Response to Medication:
3. Medication Compliance:
Etc.
What I am wondering is the clinic likes the section to be underscored (i.e., General Comments); however, should the underscore include the number or just the words titling that section? I have been doing the latter; however, the person I share the account with underlines both the number and title.
Thanks for your help.

I see them all, too. Some people seem to be missing headings from the blue bar on the left. nm

MS Word Auto Numbering
If by indenting, you mean making a list and having it indented toward the right, you need to turn on the autonumbering options in Word. Do this by opening Word, go to tools, autocorrect. Click on the tab autoformat as you type. Check ON automatic numbered lists. Now click on the tab Autoformat and check ON lists, say OK at the bottom. Close Word and open it again. When you type 1. and then some text and hit enter aftward, it should now set up your list for you. Make sure that if you are transcribing, the account allows autonumbering, most don't. Hope this helps!
Turning numbering on and off for lists
Does anyone know the keystroke commands or if they even exist to turn numbering off and on for lists? I hate having to use the mouse to point at the menu and going that route!
Help with numbering/indents in Word/EXText
Any of you computer smarties out there who can help me out? I'm new to Word Client and EXText and I'm trying to figure out the least maddening way to set up my lists and numbers. I already have a great macro for when they dictate 1., 2., 3., etc. so no problem there. My problem comes in when some of these guys want 1. and then a., b. c. They want it to be set up like this (hope it comes out right)
1. text text text
a. Text
b. Text
2. Text
a. Text.
I can get my a. b. c. macro to look right when I type it as a stand alone macro, but if I have done my 1.2.3. macro and then try to do the a.b.c. underneath the 1, then it scoots the a. waaaay too far over to the right, to where it is under the second word of the text, not lined up right under the first letter of the text under 1. Does anyone know of a way to do this that doesn't involve a bunch of time and extra key strokes? Hope this makes sense trying to explain it, I'm waiting on deliverye of my Word For Dummies book but then had the thought that I have seen computer-related questions answered here before and thought I would give it a shot. TIA.
on ExText numbering, and subheadings w/indent
after going in and setting up for numbers what I have to do and I have found no other way.
when doctor saysS1. then subset A:
1. Hyperlipidemia.
A.
B.
2. Hypertension.
3.
what you have to do is type 1. what you want afterward (hyperlipidemia), hit enter, you will get 2 lined up under 1., back up, delete it, space to under the h, type A. xxxx, enter, it will put a b under a, then if he say number 2, enter, it will type a c, back space through it and line up under 1 for the #2, type what he says (hyperlipidemia), enter and it will type 3 under the 2.
that is the only way I have found to have the indented numbers and subheadings.
Sorry no more help, but when you get used to it it gets faster. If someone has an easier way I am all ears.
Microsoft Word question (numbering)
I started with a new company today. Anyway, I got my review back. It said I need to turn off my numbering macro, the numbers should be flush left. Well, no matter what I do it always indents. I am working with Word 2000. I tried the help menu which did not help. How do you make it not indent the numbers? TIA.
XML formatting - does anyone know how....sm

formatting
Generally, if you don't have to identify the speakers by name or if there are several speakers -- just start a new paragraph with each change of speaker. Or, if it is a question/answer type of interview, you can put Question and Response as headings on each new paragraph. I've even done them with Male and Female headings to identify speakers when the company didn't want them identified by their names. Good luck.
Formatting
I have a few questions about formatting:
If more than 1 presciption is dictated should it be seperated with a period or a comma? For example: Prozac 100 mg. , Provigil 200 mg.
Thanks
Angie
It's something in Word, not ExText. Check your Format/Insert/Tools headings. nm

Every version of Word seems to do auto numbering differently. sm
The last two versions have a number list style that will keep numbers at the left margin and do the hanging indent automatically. Alt-Shift-left arrow will take your list back to the left margin if you don't use the style but want to use automatic numbering.
Change the alignment to Right in the Bullets & Numbering dialog box.

SH bold formatting and others
In the "Dictionary Text [Add]" dialog box, as you
are entering in the "Text to Type", when you need
to add special formatting, you simply tap Ctrl-K, and
add the keystroke commands applicable to your
word processor to change to the desired font.
For example, Microsoft Word uses Ctrl+B to
enable/disable bold face. So to simulate bold text
with ShortHand in Microsoft Word, you simply need
to simulate the Ctrl+B keystroke. When you are done
your word(s) that are bolded will look like this:
{@KEY--Ctrl+B}your text{@KEY--Ctrl+B}.
When you actually type the key (as you transcribe)
Shorthand will automatically/instantly enter all the
special function keys that you have entered, and
your text will come out formatted.
You can use the same concept to add any formatting codes to your Shorthand text, including underlining, italicizing, changing font styles and/or font sizes. You can also add tabs, change margins, etc.
Try thi out and if you have any problems with this, post another message.

Meditech Formatting Keys
I think it was control F8 for spellcheck, but you must be at the top of the document within the text portion of the report. I have an old cardboard that fit over my F keys. Here is what is says: Control: SI, Add Line, Left Justify, Conv Case, Transpose, Del BOW, Del BOW, blank, S3, Word Attr, S19. Then for Shift: Bold, Italic, Underline, Set Attr, Center, Join, Help, Docum, Set Search, Search, Replace, MAGIC. Then across the bottom it says: Begin Block, End Block, Format, Get, Recall, Prev Field, Begin, End, Lookup, Delete Line, Exit, OK. I hope this makes sense. I basically had this placed overtop my F keys, and then I used this as a guide. I think the post you were asking about was for the spellcheck. I only worked in MAGIC Meditech, not in CLIENT, which is apparently word-based, so I'm not sure if these functions work the same in CLIENT. Good luck. E-mail me if ya' like - I am long time Meditech user (10+ years), but it has been a few months, and I haven't had to use it.
Need help setting up formatting in MS word - sm
Computer crashed and died and now setting up my backup computer. I need to fix it so I don't have 2 spaces after a period. It is doing it automatically and I cannot find where to "fix" this. This is Word 2000 that I am using on a computer with XP Pro. Any help with this would be appreciated, it is driving me nuts (I am ending up with 3 spaces after a period as I am so used to spacing it myself).
Needing assistance with formatting
Asked to go to word, tools autoformant and disable autonumber but cannot locate this. Can anyone help?
You don't need to close Word after checking the auto numbering option. It works right away. nm

Clinic Note formatting question...
If the patient has a drug allergy do I make a new heading for Allergies or just include it in the HPI as dictated? Also do I put the medication in all caps w/in the HPI if it is left there.
I know I should probably know this, but I can not remember tonight.

Did you look at the margins and formatting and see how your basic template is set up? Never had this

Need advice please re: formatting transcription test
It's been a long time since I had to do this. I've been given a couple of report types to do. The first appears to be a discharge summary. Should I just type in the headings the way I "think" they should be based on how I was taught in the past??
Also, is it best to transcribe verbatim or try to make the test report look as professional as possible (like we would be expected to when on the job)? As far as listed items are concerned, i.e. medications.....The old MT in me wants to number them. Is that acceptable? I'm really concerned with trying to format the report based on how I "think" they would want me to.
I would appreciate any helpful advice anyone is willing to share!!
I agree 100%. Test questions on formatting or
anything else should not be asked on a forum of any kind. You need to research and find the answers for yourself on a test and not rely on people here to do the work for you.
I work for a hospital that is pretty liberal as far as formatting goes. SM
They have a basic template that they like us to adhere to as far as headings, font, font size, etc. However, when it comes to more detailed formatting, they really have no set standards and leave it to our best judgment.
My question is regarding the medication heading. Do you list the medications in a numbered list or in paragraph form? I have a friend who is a nurse and she says the clinical staff, i.e. Nurses and Doctors like to have the medications in a numbered list because it makes finding them in a report a whole lot easier. So would you list them even if the dictator is not saying "number 1, 2, 3,..."?
Macros are better suited to repetitive tasks like formatting.
I have a macro to create a different second page of a letter with header info., another one that will update formatting from documents created using a different Normal template so they conform with mine, another one for highlighting, etc. They really can be timesavers.
It means you have special formatting applied to those paragraphs sm
like keep lines together or keep with next. You can turn those squares off with the show/hide button on the toolbar or hit Ctrl+Shift +8 to toggle marks on or off -- that's probably what you hit to turn them on.
Formatting of the document dictates the line count, not a different computer.

I do basically the same as you, but I will
as I type in Word, and this helps with not having to go back and read the entire document. I find that if I notice it right away, correct it, then I can move on, and it also helps if I need to re-listen to the term or phrase. I have also noticed where my auto correct did not expand correctly, and even though my final spell check would most likely pick this up, I tend to be a little more careful with that as well. I am so afraid to go with an Expander because I'm sort of afraid I'll lose that "specialist" touch you speak of.
They are basically just saying
we will make your transcription look good like she looks good.
I don't see what the big deal is. Sex/beauty sells everything lately. It's just the world we live in, unfortunately. Every commercial on TV almost has some type of sexual undertones to it and some very obvious.
Basically, yes and no.
That doesn't help much, does it?
They all pretty much perform the same functions, but some have different types of connectors that hook to the computer and/or equipment.
I've got 2...a USB that hooks to my computer, and one that hooks directly into a Dictaphone. I know some other pedals connect to game ports. Sometimes the software allows you to set up a variety of pedals with different connectors.
It is basically just something you have to
make yourself do. I learned this way as when I started as an MT we were on typewriters, so you had no choice.
