As stated below, transcribe what is dictated - sm

no matter how much you don't like it. The doctor/dictator told you what to do, and you chose to ignore him which was wrong. I have dictated a few reports of this nature to, and I just kept on going, type what they tell, put in quotes if they want it that way, etc. Don't dwell on it and just do your job.

I transcribe just what is dictated.

transcribe as dictated...(sm)
Transcribe as dictated: "Patent shotty veins." This is perfectly acceptable as is and does not require rewording.
You can't transcribe what hasn't been dictated. I don't see

OP also stated.....
"obviously upper middle class"....so that does definitely have a bearing on the response made. There are so many variables here as well as trying to compare almost apples and oranges. I'm very sorry that happened to the little girl, but they knew she had been already for for 14 hours; they don't even know where Holloway is; she was in a foreign country when she disappearing and said foreign country is not sharing any information. There is a big difference between these two cases.
With all that being said, I do absolutely agree on one thing; there has been way too much coverage on Natalee Holloway. I don't believe they will ever find her. It was much too easy to throw her weighted body into the ocean or, maybe she was sent into South America to be used as "whatever they wish to make her do". I haven't heard any reports on incoming or outgoing flights from Aruba during that night have you? All it takes is a little single engine plane to do the job.
What I stated before was---
I don't object to people breaking into food stores for water or food, but why do they need to steal TV sets, break into people's homes to steal jewelry, guns, etc. That is STEALING, folks, like it or not. Why do they need to steal TV sets now when there is no power to begin with??? If they were just taking items that they needed to survive, that's one thing, but to take advantage of a horrible situation is reverting to the law of the jungle. The fact that these IDIOTS are SHOOTING AT THE PEOPLE WHO ARE TRYING TO HELP THEM IS A CRIMINAL ACT! As I have said many times before on this board-- a situation like this brings out the best in some people and the worst in others! The ones who are doing all the looting, shooting, rapes, etc. already had that mentality and are just using this as an excuse. They are not helping anybody by doing this, including themselves.
Yes, it should be stated that 500 lph
Not many people can crank out 500 lph and if they do, it's because the account has many normals and templates. That's rare, especially in acute care.
Even if you could transcribe 500 lph, 6 cpl is still WAY TOO LOW. Do not accept that rate of pay unless you feel that's all your work is worth.
Who stated they took the AHP
I am actually a graduate from one of the top 3 schools, never had any problems finding a job after school. It is a disappointing field to get into, people expect you to work like a worker in a sweat shop. I especially feel bad for people who have been in this business for many, many years, have the CMT credential and don't get the pay they deserve. In all actuality, it doesn't matter where you got your training or your credientials, you still don't get paid what you are worth.
However, I am glad I researched the schools 3 years ago before getting sucked into one of the diploma mills.
As stated below...
...the pedal you currently have should be compatible to play .wav files.
I had one the other day that stated
Patient was given an insect at 150 mg per hour for 12 hours. Yuk!
It's Transolutions, as stated above.

Excuse me, but as has been stated
very different. I'm happy that you have a good one, but we don't all get bonuses...even if we have excellent QA. So how 'bout gettin' off the high horse and jumping to conclusions when you may not have all the FACTS?
As I stated right at the beginning....
If it were ME, I would be blaming myself. Each and everyone of us are responsible, one way or another, as to what happens in our lives. Too many people over the past so many years find it much easier to place the blame elsewhere. That is one of the problems with our society. I was "accused" of being an over-protective parent as my child grew up, but guess what, he never, ever even came close to an incident such as that, or close to being kidnapped or any of the other things going on in the world; didn't fall down steps; wasn't involved as a passenger in a car accident, wasn't around drug users or "bad" people, because I PAID ATTENTION...I didn't allow accidents. Accidents are only LACK OF ATTENTION and lack of common foresight!
Also, my "over-protectiveness" did not in any way inhibit my child's growth. He is a Gunnery Sargeant in the Marine Corps and handles great responsibility well and has the foresight and the attention needed to prevent accidents!
As for the driver of that car, I'm not trying to "take their side"....I'm just saying we don't know what the circumstances were....my goodness, we have all run over something in the road at one time or another and when the weather is inclement or it is dark, sometimes you don't know what it is. I would certainly hope that if they knew it was a child, of if they find out through newscoverage that they ran over a child, that they will come forward. I'm sure that at least 99.9999% of people out there will not willfully run over anyone. Perhaps he/she looked down at the dashboard or the left rear window for just that one second and felt a thump as he car moved on. Probably wondered what it was but had no idea whatsoever that it was a child....why would any driver in their right mind think that a 2 year old was out on the highway at 2:00 AM or midnight or whenever it was?????
as stated below, sounds like
office mangers padding their own pockets, NOT MQ policy... I would take it all the way to corporate
You have just stated my feelings exactly. nm

The letter I got clearly stated that you can opt out
of ASR and do strictly MT. I was told when I trained on DEP that at some point there would be a reduction for those doing ME. I see no reason to bail, as my office, at least, was up front with me. It's unfortunate if some offices were not.
It would be x3 or 3 times if stated
that way.
That was NOT how you stated, read
I am glad I don't do you line counts either, but I can bet they are not 2500 in 8 hours!!!
spaces are usually stated -- with or without
it's no trick at all
if they say just 65 characters, i would think spaces are not paid
yes, you could call them and that is what you should do
No I meant what I stated.
Taxes and other deductions come out of the $730/750 a week.
The monetary value of company provided health insurance, PTO, STD, LTD, life insurance, retirement, company matched income tax, etc., adds about another 30% plus value to my base pay.
not one name nor team name was stated....nm

I do believe as stated in this discussion
that we should care for them. I just want to respond to some things said. Yes, our parents have decades to prepare. But my parents worked very hard and were extremely smart with their money and had a good savings when my Dad became ill. However, to put him in a nursing home is thousands of dollars a month. His insurance paid 0 for it and Medicare only paid 20 days. For him to stay it would have drained every pain fast (and it did) and then he would get state aid of course. If you can at all help it, a nursing home is NOT where you want to put a loved one. Some are great if you can afford them, but even the ones you think are good can be very remiss in care. Trust me, I've seen it. They're grossly understaffed and some employees should not be in that field, enough said. I don't and didn't at ANY time feel caring for my Dad was a burden. I did feel it was my responsibility. He's a member of my family. If my brother or sister became ill and had no family to care for them, I would certainly feel it's my responsible to at the very least help them get care but for certain to make sure they're cared for. I can't say how a person should feel towards an abusive parent since thankfully I have no experience in that. No, I wouldn't want to 'burden' my children with caring for me when I'm old. But if I raise them right and I happen to need care, I would hope they would feel privileged and blessed like I did, to help make sure I get what I need.
She stated they were using them in emails,

Thanks for replies. As stated, sm
I do plan to ask the MTSO about it. I just wondered if it was more commonplace with smaller-sized MTSOs. Patti, that makes sense - the MTSO is in a small town, so maybe they are used to working on trust.
I've seen a lot of MTs not getting paid lately, and I have been burned in the distant past by a few, myself. I did receive a reply on here about the co., but then I thought, shoot, it could've even been the MTSO answering for all I know!
Thanks. I appreciate the opinions. No, I'm not brainless and I would never make a final decision based solely on opinions on a message board. Only wanted information since I don't have past experience with small MTSOs and how they usually operate. Once I said yes, I'd work for them, I received work right away, same day. I knew they were swamped and figured we'd do contracts later once things settled. Only worked a couple files here and there, then the big project issue came up and I thought hmmmm.... is this wise? LOL Probably being overly paranoid, but it's hard not to be with so many MTs being stiffed and usually by what seem to be smaller companies. Thanks again!
It is being questioned, as I stated, because (sm)
it is unusual. It may not be unusual for you, but it is unusual for many of us (prayer is not unusual, but to hear this being dictated is, and that is coming from someone who prays 5 times daily without fail). She has every right to question any dictation before placing it in the report. If an MT has no right to question any part of a physician's dictation, then what is the MT good for?
He stated he is going to fight that.
Said he is going to fight against that medical records thing. Didn't mean offshoring, does not sound like he has any idea about that happening to our medical records.
As I stated in my post below ...
Qualified is a great thing, but it's pretty clear that the industry does not want to pay for qualified.
The poor "education" that most MTs receive (and rely on without taking their studies further on their own) are accepted by SO MANY companies because, were MTs required to be more educated, the rate of pay would have to go up.
They are able to continue to pay extremely low wages with very unfair terms because we are not, as a whole, educated, represented in a unified way, or regulated.
I can assure you, if quality really WAS of interest to hospitals or MT companies, they would demand a certification program and we would be in a much different boat.
But because it is NOT of real interest or import to hospitals, doctors, or national MT companies, the "regular" MT will never live up to true quality standards.
It doesn't seem as though there's a mismatch between what the buyers expect and what the sellers produce, really. The market has adjusted to the point where both sides are comfortable.
It is what it is. And until one or the other of us cares more, it will stay what it is.
I once had a report where the doc stated
That's funny Cranky. Reminds me of an older doctor I used to transcribe for.
"The patient is feeling so daggone good, he doesn't need me anymore."
"The patient states his stream is so good, he can hit the wall from across the room."
Seeing as this doctor was always dictating some off the wall phrases and at times flirting with the older female patients as he dictated the reports in front of them, I'll never forget the time I sent a report to QA to confirm his stating "Patient comes in today with his rascal." Here I was thinking umm, is he referring to the man's wife?? lol
Duhh, moment for me as he was talking about the man's wheelchair.
That doctor used to have me laughing.
and think, the Dr. may have dictated that
At least you ARE concerned about it. Many wouldn't be. Hang in there!
Has she ever actually dictated a sm
report in the format that she is required to do. I do forms for a cardiologist in this manner, but when I first started working for him, he actually dictated the letters. I think all physicians need to include certain things for a new patient, and then can do follow-up letters without including past medical history, allergies, etc. I wonder if she has any colleagues that would be willing to share samples of their reports, but that specialize in the same field.
He has stated that he hates America
There was an item in the Washington Times. When the story was picked up and he was questioned, he tried to claim he was misquoted, but the reporter who was interviewing him when he said it had the film to prove it. He hates America, so he lives in France, but he apparently doesn't hate our money because he still works here and is happy to take as much money as the Americans he hates are willing to throw at him. He's not getting a penny of my money.
I'm well aware we edit..but someone on here stated
dictator just stated the obvious
"ORAL tobaccoism" -- like what other kind is there !!
oh i suppose it could be cutaneous via patch, but sounded dumb...
Poorly stated, but I think anyone should understand what
you're trying to say. You think that MTs should be compensated more if they don't need QA as compared with the ones who don't take the time to look anything up, don't proofread, are in a hurry and miss words or leave several blanks.
At least I hope that's what you meant. Either way, someone's gonna get their panties in a bunch about the way you stated it.
The point is to commensate the MTs accordingly, instead of treating them all alike whether they do a great job or not. Face it, I make as much (I don't need a lot of QA) as the guy who doesn't look anything up and sends blanks.
thanks for the tip, but as stated, our specifics say double. nm

he hasn't actually said, "i killed her." He has only stated, "i was with her when she d
i think he is totally obsessed with the case. whether he did it or not, i am not sure. he can't give details?? the puzzle i am not fitting is the 118,000 ransom note that patsy new about. he has admitted it was a hostage gone wrong. i am wondering if maybe somehow he and patsy worked together on this one and now the reasoning it is coming out since patsy is no longer here. he lived in Georgia where they did but jonbenet was a baby at that time. it states that were vaginal abrasions but no semen? maybe that was a cover up to make it look like an intruder but they kinda left out an item called semen to make it real. makes me wonder if her hymen was intact. he is saying he drugged and raped her but yet autopsy says no drugs in her system.
I picked up a brochure and it stated that --sm
the student version did not come with outlook 2007, which I happen to like. Guess it will be the next up version for me. sigh.
I am very well aware of the differences between the 2. As I stated below
in my post, I use GuildFTP server software. It is on my computer and the files are stored on my computer. It is completely secure. I pay absolutely no fees, because if I did I would have to pay them to myself. The files are sent from the physician's offices to my server which is in my living room. It does not go through anyone else to get to me. I have a static IP address which their FTP software (usually cuteFTP) uses to connect to it.
I stated the same thing in another post...sm
I am an IC directly for a small rural hospital in a generally rural state. Most of the hospitals in my surrounding area have either inhouse MT or remote MT and the medical records dept manages them. I started inhouse and then have been home for almost 13 years for the same hospital. I have also done part time work for the nationals. I basically get paid about the same, maybe more, without all the headaches of having to go through a national company. I raised my rates 2-3 years ago and the hospital looked into a national company but found I was much, much cheaper and stayed with me. This year, I again raised my rates and they agreed with no questions. Local MT jobs are out there. You just need to hound these people until how beneficial for everyone it would be to hire locally.
But you stated you questioned your lead SM
and nowhere in your original post did I read that anyone checked with the doctor. That is the problem we have. If you or the lead had actually checked and he said to change it, fine. But neither of you checked. You asked the lead, the lead said change it and you changed it.
As stated, just send everything. But agree with
x
As the other poster stated, I'd be careful sm
in getting to comfortable here. I don't think it's any surprise that half the page on the jobseekers board is filled with their ads. I rather suspect you'll be doing "editing" and VR work rather than true QA - HUGE difference. Of course, 7-8 cpl for VR or editing is fantastic, but that is what would have raised a big 'ol red flag right there - who pays this for VR or QA work? Not many. Good luck though, but definitely keep your eyes open.
ROFL at what this MD just dictated...sm
NEUROLOGIC: He does have diminished memory. I did start him on Aricept for this last year, but he has forgotten to take it.
They pay per minute dictated. Sux.
Especially when they assign the work. It's not fun when all your work is less than 1 minute, two line reports, ESL docs, etc.
dictated, not read
I'd take it as a compliment.
If the docs dictated like that, they would never get anything done, lol

H&Ps dictated by a very thorough dictator. nm
Doesn't have to be dictated...but he does

Exams Dictated But Not Done
I transcribe for a local multispecialty clinic and have seen many of my own reports. It just gripes my butt when the doctor dictates a standard physical exam and I know for a fact that he/she hasn't even touched me. Did not check my heart, lungs, abdomen, neuro, extremities, etc. The only things that are legit are the vital signs. It's like if I come in for a sore throat he looks at my throat but nothing else. My fibromyalgia clinic NP only talks to me - no exam at all - yet she dictates an exam, review of systems (that we didn't even talk about). She will say conjunctivae and lids are clear; PERLA, etc., and she hasn't even gotten within 6 feet of my eyes. This just doesn't seem right. And when you have labs or other tests, they NEVER call you back with the results. My PCP never suggests any preventive testing - I have to ask for it myself. I know they're not all like that but my eyes are opened much wider than they used to be. It's all about getting it done in under 15 minutes and on to the patient in the next room.
I always leave these as dictated.
Let's say you're Asian and have a doc who seems to possibily have a bit of an ''attitude'' about your ethnicity. In perusing your own medical records, you see that this doc uses the term Oriental instead of Asian. There's some confirmation that your suspicions were correct, allowing you to make a more informed decision about your medical care and who you want providing it. Especially true with the word Negro, IMO, which I haven't heard in quite some time but have actually had dictated and left as is, even if I had the leeway to change it.
IOW, I think it's a patient's right to know if a doc is using words like that, so I don't change them.
Doctor who dictated during the PE sm
I had a doc I used to do who treated a lot of brain injuries and strokes. He always dictated during the interview and PE. He would say, in front of the patient...the pt is completely obtunded...the pt is befuddled...the pt is extremely obese, just enormous...the pt's mom is once again out of touch with the pt's needs...and so it went.
The noise was sometimes out of hand, he was a grumbling gravely dictator and that IS all part of the job. It is the thought of being his patient and having him be so brutally honest, well it smacked of unprofessional behavior.
I had a podiatrist who was "too busy" to dictate the day of service, so he would dictate on Sundays. One weekend, he dictated 8 reports...from the bathroom...in the span of about 20 minutes. I got to hear the toilet flush no fewer than 6 times in 5 dictated reports. He kept pausing to concentrate and it wasn't on the report he was dictating!
If the noise is too bad, it gets a blank. I don't care what they say and I don't care where they are when the dictate, but please don't make me know that you have the crap$ while you are on the phone with me. Plainly, I don't want to know.
I would type it as dictated. nm
