I can tell you we see such stuff it HAS to be guessing. We see made up surgical instruments, physician names that don't come close to the correct spelling (not bothered to be looked up). The "don't guess" comment is basically to say look it up! Verify everything you put on paper because remember, you are dealing with an actual sick person and if you put something wrong, you can hurt them and they do get hurt.
