Title: Healthcare Exam Question Writing


PROJECT DESCRIPTION

The client is based in the United States that is providing training and education to health care professionals.
The client is looking to outsource the process to outsource agents who can write exam questions from the source material (source material can come from journal articles, books, etc.)
1. The client will provide the source material either by email or by providing the web address.
2. The outsource agent will write multiple choice questions from the source material provided. Each question will only have 4 possible answers, with only one answer being correct.
3. The client will designate the number of questions required per course.
4. The outsource agent will format the questions in a format that will be provided.
5. The outsource agent will email the questions to the client for review and editing.
6. The client will email the edited questions back to the outsource agent.
7. The outsource agent will upload the edited questions to the client website.

The client will pay one dollar US ($1) for each question written.

We believe that for a typical set of question will require 15 questions and will most likely take about 3 hours to complete. This equals about $5 US per hour. We anticipate rejecting some of the questions that are developed, though the client will consider paying for the development. The client will work with the selected agents in the future to assist with question development.

Health Information Management is the (sm)
same thing as medical records department in a hospital. I've worked for a hospital that used this fancy (silly?) term. It just means hospital work, but does not include radiology, ER (in my experience) or pathology transcription (IME). It may or may not include cardiology procedure reports, neurology procedure reports (IME).

Treating private health information like a trade is wrong any way you look at it.
Let's see how you feel when you've been violated because your private health information was distributed for any and all purposes.
HIPAA only addresses PHI (protected health information) would is defined as anything that SM

specifically identifies the patient, i.e. name, MRN, SSN, account numbers, etc.  As long as all identifying information is removed from a report, it poses no HIPAA violation to keep it on file.  Furthermore, as an employee of the hospital in the medical transcription department and charged with peforming QA, I have legitimate reasons to access a patient's chart.  HIPAA says so.


Go back and read your HIPAA laws again.

there that went from coding to MT? OR, MTs who went into coding? Love coding, hate coding? Not much talk amongst coders anywhere. I just wonder what it is like and if it is worth considering.

Management IS the devil. By the way, what's it like up there in mananagement, with your 6-figure income, your executive healthcare, and salary-not-hourly status? Especially when you sit in your office all day surfing Amazon & eBay? Must be nice, Beelzebub. But I'll never sell my soul for it.
At-Home Professionals
I thought the training there was excellent.  At the time, I was very frustrated with all the ESL docs they had, but now i'm glad they did that because while other MTs are shying away from ESLs, I'm getting that work and getting paid a premium for it.  It took me a little while to find a job, but I've been working now for over seven years and I never regretted taking those courses.  Your best bet would be to start with local doctors and get some experience first.  Good luck to you!

Don't know about the prospects of looking for a job.  I don't know how much you make doing transcription of what you consider similar.  I think coders probably start out in the mid-20s per year up to 40s with some experience.  There are not as many billing jobs at home as there are remote coding opportunities.  Hope that helps.

school professionals are not always "perfect"

In my own experience, my daughter was upset all the time, crying, acting out, etc.  I was receiving phone calls quite often, but I knew my daughter.  She was in th 4th grade already.  I met with teacher it was only about 2 months into the school year.  It was actually the teacher my daughter had, who was new to the area and the district and seem very timid.  I changed her to a teacher who I knew from before with her brothers and was outgoing, gave lots of praise to all the children and really interested in the kids.  My daughter was back to herself.. 


I have also had problems with the principals, and one was a psychologist.  They will try to "bully" you, so you have to stand up for your children.  I even had them take off a suspension on her record because it was unfair, as I had many times spoke with the principal about some girls teasing my daughter and hitting her many times and my daughter  never did anything and finally was tired of it and hit back and she was suspended.  All the other times the other girls never were suspended but they were just talk to.   The principal never took the time to speak to the kids before this happened.  She just thought my child had issues, but in the end I showed her that my child did not have issues. 


Anyway, my older son has ADHD, so it took a teacher who was patient, caring, and who expand the mind of the children not just classify the children with learning disabilities as "worthless" and put them aside. 


I have walked into class without notice to see what is going on; I have stayed to help out for awhile. 


If you do not get anywhere with the school officials, speak with the superintendent; go up the ladder, make waves.  Make sure to document everything, person you spoke to, time, date, etc.  Believe me this will help enormously.


We have to be there for our children.  We also need to remember they are imperfect, they will also make mistakes and they do need to take the consequences of their actions.

If the trained professionals at the school can't handle him,
why do you think you'll be able to work and homeschool him at the same time?  I'm not slamming you, but I don't think working and trying to educate your child at the same time is the solution.  Maybe try switching teachers or switching schools.  Have you had him evaluated by his pediatrician or obtained a referral to someone else?  What kind of phone calls are they?  Is he acting up?  Have you asked him WHY he's doing these things?  Kids need to be socialized.  They need to learn how to cope with other people.  They also need to learn that being part of a society means that you're not always the center of attention.  My SIL is going through this with her kid.  He's being an absolute beast because he doesn't like the teacher and she has 19 other kids she needs to pay attention to.  He wants attention, any attention, so he's acting out.  I don't know if your situation is similar to this one or not.  I'm just saying that sometimes sticking it out and finding the root of the problem is better than trying to have your attention focused in two different but important places.
Do you think nurses and front office professionals look sloppy?
I don't like to go around looking "sloppy all day" either, but is it really practical to work comfortably in nylons and suit jackets when I am sitting and typing all day? No thanks -- I can't work well that way!

I refuse to wear sweats or jammies because I want my work to reflect my professionalism and I think the way I feel about myself reflects that -- hence, scrubs, which medical professionals wear. I am a medical professional and I don't think there is anything sloppy about wearing tasteful, professional, and comfortable scrubs.

Since you have recently become an advocate for these "well-educated" professionals....
please justify the 20 minutes I had to sit and listen through all because the doc left his hand held recorder on. I actually had to sit there and listen to him urinate AND defecate! Immediately after hearing the toilet flush, I could hear the door open and then several footsteps that led him to meet, greet, and converse some more. I was more appalled at the fact that such a "well-educated" medical professional didn't care enough to wash his germ infested hands after his restroom break. Unbelievable.

A doctor may say the patient has hypertension as a diagnosis, but the reality is there are a bunch of different codes for hypertension.  There is no such thing in coding as plain old hypertension.


Same with certain procedures.  There is a code for this procedure and a code for that procedure.  BUT if you do this procedure WITH that procedure, then there is a whole nother code for that.


In order to have coding fully automated you would have to have software that could scan a patient's eletronic medical record record and then decide which diagnosis of hypertension best meets this patient's history and then assign the correct code.  Plus with the enormous fines for medicare fraud and upcoding, no hospital is going to trust a computer to coding.  Coders will probably end up editing codes same as we edit transcription from VR.  Our jobs don't just disappear, they evolve.

Since ADVANCE is free, there would
be little immediate impact if we cancel our subscriptions. Contact the advertisers and express your displeasure. They're the ones who provide the income stream for ADVANCE. They need to know that the people they're trying to reach are displeased and that their companies are associated with an extremely unpopular position.
how to advance in the MT field ...

I am wondering what it takes to advance to another position in the MT field.  I've been an MT for about 13 years, acute care for 12 of those years.  I'm tired of typing and have done QA as well, but the pay is horrible.  What does it take to move up the ladder to another position, such as MT supervisor or account manager?  I'm just curious. I know there are probably many more qualified people than I out there for any such position, but curiosity has got me as to the general qualifications and average salaries of each.  Anyone know?  Thanks....

The article says: "we have more and more companies paying premium for credentialed MTs." Is that true? The author is president-elect of AHDI. You all know what that is? Never heard of em. Or is it that new organization that used to be called something else, what was it... I think it had the words Medical Transcription in it. Maybe we should write a letter to tell about the reality of working out here.

Thanks for sharing this article in Advance - don't get magazine anymore since I dropped my AHDI membership. Proud to be part of ATA - we MTs deserve an assocation that will advocate for us.

about how wonderful it is to train Indians to do our job.  OH YAAAYYY, LET'S HERE IT FOR SPHERIS AND HEALTHSCRIBE   Let's give a big whoopie and cheer them on while they continue to post for transcriptionists on this board and yet outsource and TRAIN Indians to do our job.  I guess I'm angry about the article on other levels, such as the fact that it seems that Advance magazine is constantly promoting offshore transcription.  This is not the first time they have printed articles like this. 


 


Our hospital used Spheris and Cbay and we spent hours up hours redoing the reports, yet these companies got paid in full.  Where is the logic in that?  Thank God, administration wised up and brought the work back in.  Now we can go back to doing what we've always done ... OUR OWN TRANSCRIPTION ACCURATELY!

I think if enough people did, then maybe they will stop glamourizing the overseas work. This is the third article in a year's time that they have done. A few months back Advance actually interviewed an Indian owner, picture and all, about his company!!!


If we cancel our subscriptions, Advance won't be able to
sell as much advertising.  THAT is where they make their money, not by promoting offshoring.  If they can't distribute XX numbers of copies per month, the advertisers will go elsewhere.  Boycott ADVANCE and spread the word.  Shut the traitors down.

early this summer -- explained the 3-prong test (instead of the old 20+ questions).

If the service insists you use their software, if the service tells you when you can invoice them (instead of the 2 of you deciding together), if the service, not the client, decides turnaround time and quality, and if the service says you can't subcontract the work and insists on a noncompete clause, then you are an employee.

You can file a form with the IRS if you want your status clarified. Also contact Dept of Labor office in your state.

If you don't have an attorney and accountant on retainer, then you aren't professional enough to be an IC.

Also check the Nolo Press site as they have some excellent reference books regarding IC/employment issues.

Current ADVANCE issue is MT pay

Cover story:  Are MTs Paid Fairly.  This story needs rebuttals.  (I don't know if I spelled that right -- too mad to look it up.)  There is another article about MTing as well as some good letters to the editor.  No, I didn't write them.  Not promoting self.


Look, this magazine wants feedback.  Controversy creates publicity which creates readership.  Please think about sending your own letters to the editor.  Write what you write here.  Let more people involved in our field know your thoughts whether they be MTs or not. 


No, I don't work for ADVANCE.  I'm just an MT just like most of you.  I just want our voices to be heard by more people who call the shots that affect us.  This is one venue.  Even if you don't like the publication, fight back and make your voices heard!!!  What a lot of you write here would be fantastic letters to the Editor which would reach a wider spectrum of our associates than this forum for MTs.  We've got to keep the topic of the MT situation HOT in the media.  You can submit articles to them as well.  Get busy, PLEASE!!

I was wondering if any of you have read the latest article in Advance about MTs? It's called The MT Critic.  Color me stupid but I did not know MTs had to go to medical school to do their jobs?  How many of you know the average size of the left atrium of the heart? How many of you would contradict the doctor's vital signs because the patient had a fractured ankle and you just knew that because of his pain, those vitals could not possibly be correct?  It is no wonder we are not attracting qualified people to our profession.  What are your thoughts?
