I am no doctor, by any means, but for one thing, he seems pretty young for liver disease, even with the drinking. The pain also seems a bit low for the liver, as well, as the liver is more up towards the ribcage area and you mentioned right of midline near the waist. I would suspect more gallbladder difficulties, myself, or possibly even an inflamed appendix. But those are just my thoughts. I wish you luck.
