The first time I saw an orthopedist about my feet, I was complaining about bunions. He asked where the worst pain was. I said, well actually the worst pain is a sharp pain that shoots straight up through my foot. I am not sure it was nerve pain, but it was sharp. He had me stand up and watched how much my foot flattened out as I put weight on it. He said my foot moved too much (I only weighed about 115, so that's a weak foot!), and my ligaments were too loose. He said the motion caused the bunions as well as the shooting pain (so the bunions did not cause the shooting pain and that pain would not be helped by bunionectomy). He said I needed to wear arch supports all the time and keep my weight down.
