sounds like the nurse when I had with my baby 40 years ago! I had a wonderful doctor (NOT) follow me through the whole pregnancy but never showed up for the delivery. He sent his new assistant to cover him. I was in labor over 24 hours and the wonderful nurse I had (NOT! NOT!) stated "Its more fun going in than coming out, isn't it? That'll teach you!" Hey, I was 21 and married! I would have punched her right in her big mouth if I wouldn't have had another contraction.
I had nothing for pain for the first 20 hours. They knocked me out and when I woke up a day later, they had butchered me from front to back. I had to stay with my MIL for 2 weeks until I could even get up and walk! I should have had a cesarean but they never even thought to do that.
Thank heavens, things are a lot better today. My 1 DIL was home in 1 day, had an epidural. My other DIL also home in 1 day with nothing for pain. Both had nice robust healthy babies.
BTW, You better believe when the bill came, I did not pay it. I made sure I never got pregnant again. My son also wound up with learning disabilities and ADHD because they used forceps on him. Told both the hospital and doctor that I had a good mind to sue them. They never sent another bill.

I know a doc (anesthesiologist) who always keeps a cricoidotomy kit with him.
