this happened at the hospital i work for. we had vendors assigned to some of the er and ds reports. one of the clerks was getting tired of correcting this one person's reports because this person kept putting in the wrong document template (nothing to do with the body of the report itself), and my boss finally requested this person be off the account. thing is, i dont know if the vendor kept this person on and just changed the ID code, but the mistake kept happening with this new id. my boss, thinking it was the same person, again asked for this person to get off the account.

other than that, i only know of one person who was requested off the account for consistent quality mistakes.

good luck to you; people dont realize how frustrating this job can be for us.
